MEDICARE NEWS
Traditional Medicare to Add Prior Authorizations in NJ,OH,TX, AZ, and WA
 
Medicare will be requiring pre-treatment approvals in its fee-for-service program in a bid to root out unnecessary care, federal regulators announced Friday. The change will apply to 17 items and services, including skin substitutes, deep brain stimulation for Parkinson's disease, impotence treatment, and arthroscopy for knee osteoarthritis. The program will start Jan. 1, 2026 and run through the end of 2031. It will only apply to providers and patients in New Jersey, Ohio, Oklahoma, Texas, Arizona, and Washington.
 
Overuse of skin substitutes to help heal wounds has especially come under fire in recent years. Medicare spent more than $10 billion on the products in 2024 — more than double what was spent the year before, according to the New York Times. Providers in the geographic areas can choose whether or not they want to submit an authorization request before delivering a service. But if they decide not to, they'll be subject to post-claim review and risk not getting paid for a service that was already delivered.

FL Podiatrist Highlights the Importance of DNA-Based Diagnostics
 
A talk presented at the 2025 SDPA Annual Summer Dermatology Conference in Washington, DC, featured speaker William Scherer, DPM, MS, who spoke about an often under-discussed topic in dermatology: interdigital foot skin infections. Dr. Scherer was asked about the most common misdiagnoses seen in this area, in the portion of his talk distinguishing fungal from bacterial infections in the foot’s web spaces.

In his session, Dr. Scherer had highlighted the importance of DNA-based diagnostics. He was asked about how, in his experience, the use of PCR assays transformed the accuracy and speed of diagnosing interdigital infections compared to traditional methods like fungal culture. “PCR is a huge game changer for all of medicine, this is now looking at the DNA specimens,” he said. “Number one, it's rapid, it's quick, and it's super accurate.

A Balanced Portfolio Will Help You Weather Any Financial Storm: NY Podiatrist
 
Brad Perry, DPM remembers Oct. 19, 1987 like it was yesterday. The new retiree from Oceanside, NY, was a young podiatrist, starting his career and beginning to save money in a few mutual funds. But the sea of red on Black Monday was too much. The Dow Jones Industrial Average fell 22.6% in a single day, wiping out almost $2 trillion of wealth. "I panicked and sold everything,” remembers Dr. Perry, 66. “I just couldn’t deal with it. It was too much pressure.”
 
Perry definitely learned his lesson from that market collapse almost 40 years ago. These days, after a successful career that included becoming a medical director at pharmaceutical giant Merck, he isn’t doing any panic-selling. He has enough cash to get through a couple of years of crisis, and has a balanced portfolio allocation sturdy enough to weather market storms. In fact, he is sanguine enough to be heading off to a cruise in Portugal with his wife — despite the current market insanity. “Never in all my years have I seen a time like this,” he says. “It’s kind of crazy, and it’s going to be a rocky road for a while.”

Send a quick survey within 24 hours of a visit asking:
  • 1. Was your concern addressed?
  • 2. Do you understand your care plan?
  • 3. How can we improve?
Short, timely surveys can lead to faster insight, improved responsiveness and higher overall patient satisfaction.

I believe Dr. Richie’s response focuses on personal criticisms rather than addressing the substantive issues raised. It is concerning that someone who built their career by modifying an existing brace design, trademarking it under his own name, and commercializing it within the profession would question another’s business practices. My position has consistently been that physicians’ time and expertise deserve appropriate compensation.
 
Dr. Richie appears to misunderstand the distinction between not-for-profit institutions and public entities that operate without profit margins. Not-for-profit organizations can generate revenue; the key difference is how surplus funds are utilized (bonuses, salaries, retreats and paying guest faculty) rather than distributed. Rather than engaging in personal attacks, I was attempting to offer constructive solutions to address identified problems within our field. I believe our professional discourse would benefit from focusing on the merits of proposed solutions rather than personal criticisms.
 
Should Dr. Richie wish to evaluate our respective contributions to this profession’s advancement without profit motive, I welcome that discussion. I have dedicated over twenty years to serving on multiple professional boards and lectures to teach others how to benefit and earn a good living ethically. As anyone involved in public speaking and education can attest, clinical practice typically offers greater financial returns than teaching and professional service. I encourage Dr. Richie to engage with the substantive issues at hand in future discussions.
