APMA STATE COMPONENT NEWS
CPMA Gifted $500K Legacy Grant by CA Podiatrist

Following the close of the California Podiatric Medical Association’s (CPMA) 2022 House of Delegates on June 22 in Anaheim, CA, the doctor delegates and alternates joined the newly elected 2022-2023 CPMA Board of Directors, guests, colleagues, family, and friends for an evening of fellowship, camaraderie, reconnecting, and renewing bonds of friendship at the Annual President’s Reception. During the celebration, CPMA Life Member Bruce Lawrence, DPM made a momentous announcement noting that he had been moved and inspired by two articles in the current issue of CPMA's member magazine The California Podiatric Physician.

(L-R) Drs. Jon Hultman and Bruce Lawrence

The specific articles were: "The Long Path to Parity: Past, Present, and Future" by CPMA Executive Director Jon A. Hultman, DPM, MBA, and the other was "The Quest" by CPMA President Diane D. Branks, DPM, which he and his spouse Janet felt provided perfect reasons and timing to create and gift the California Podiatric Medical Association with the Legacy Fund. The $500K endowment is the most significant individual gift the Association has received in its 110-year history.

PODIATRISTS IN THE NEWS
CA Podiatrist Discusses How to Get Soft Feet

According to Ebonie Vincent, DPM, a board-certified podiatrist at OC Podiatry and star of TLC’s My Feet Are Killing Me“The soles of your feet don’t have oil glands like other parts of your body, which is one of the reasons why they’re prone to dryness.” Add to that the constant friction you get from walking and the added exposure to the elements during the summer months, and it’s a perfect storm for dry, flaky heels. That’s why exfoliating them regularly (even daily) is key for keeping a smooth surface. Dr. Vincent recommends using a loofa and soapy water or a pumice stone in the shower, adding that, “You want to be gentle with your pressure. Don’t scrub too hard, which is our tendency. Harder isn’t better here.”

Dr. Ebonie Vincent

While we’re on the topic of exfoliation, we had to ask her thoughts on those viral “Baby Foot” peels that make your feet shed for days to reveal, as its name implies, infantile skin. “Your skin naturally sheds as is, so when you use a peel like that, you’re increasing the number of layers you’re shedding, which sounds good in theory, but can actually strip the pH and moisture from your skin and take off too many layers, which can cause sensitivity,” says Vincent. “While they’re okay on occasion for those with very dry feet, they’re not something you should be using regularly.” You’re better off going the gentle, regular exfoliation route over the quick fix.

PODIATRISTS AND THE LAW - PART 1
NY Podiatrist Pleads Guilty to Defrauding Amtrak of $1.6 Million

Michael DeNicola, DPM, admitted to his role in a healthcare fraud scheme, distribution of opioids and illegal possession of a firearm, the Justice Department said June 29. From 2019 to 2021, Dr. DeNicola, 59, and his conspirators billed the Amtrak healthcare plan for fraudulent claims, the department said. The claims were for services that were either not provided or were medically unnecessary. The scheme defrauded Amtrak of more than $1.6 million. Dr. DeNicola also gave oxycodone prescriptions to a person in exchange for the use of their patient and insurance information, the department said.

PODIATRISTS AND THE LAW - PART 2
PA Podiatrist Faces More Sex Charges From 13 New Accusers

A doctor with offices in Butler and Armstrong counties is facing more sex charges after a hearing Tuesday. Dr. Matthew Sabo was arrested in January after a patient told state police Sabo had inappropriately touched her several times during routine podiatry exams. Sabo and his attorney learned 13 more accusers have come forward, leading to a number of counts of sexual extortion and indecent assault. Sabo continues to deny the allegations.

PRACTICE MANAGEMENT TIP OF THE DAY
Tips to Get a Raise from Your Payers

Negotiating a successful payer contract increase is possible for those willing to craft a thoughtful, data-driven argument for why they deserve more money.

9. Add services to your fee schedule

Many physicians perform COVID-19 immunizations and laboratory tests, the codes for which may not be on their fee schedule with a specific pre-negotiated contracted rate, Harding says. Instead, they’re paid a percentage of charges, which is often lower. “All you need to do is add these services to your fee schedule and you’ll be paid more money for performing the same service,” he says.

QUERIES (CLINICAL)
Query: Metatarsal Stress Fractures and Under Armour Shoes

A year ago, our local college switched to Under Armour shoes for both men's and woman's basketball. Since that time, we have had a significant increase in metatarsal stress fractures. We have looked at training, nutrition, as well as other causes, but keep coming back to the shoes. I would like to hear from others involved in college athletics to see if they have seen similar issues.

RESPONSES/COMMENTS (NON-CLINICAL) PART 1
RE: Shoes are Getting a Bad Rap
From: Dennis Shavelson, DPM

In a society with paved streets, clustered homes, and physical activities that use special surfaces, equipment, and extreme movements, shoes maintain quality of life, prevent injury and disease, help with healing those with chronic foot conditions, and offer support for inadequate biomechanics or excess rearfoot pronation, while enabling us to perform in hazardous conditions.

Unhealthy foot biomechanics are the root cause of most of our standing and moving issues. Harmful and unnecessary impact and stress on parts of our feet, ankles, knees, hips, pelvises, and low backs that eventually lead to pain, deformity, degeneration, injury, and poor performance over time. These are caused by Earth’s gravity and civilized hard ground surfaces. Shoes are the #1 device discouraging all that from happening.

Shoes protect against sharp objects. Shoes cushion feet and postures. Shoes offer protection from impact stress. Shoes help feet heal from infections, injury, and disease. Shoes keep us active. Shoes do things that support and stabilize our feet and postures. Shoes can incorporate mass produced or custom modifications that promote better alignment in your feet, ankles, knees, hips, and lower back. Shoes allow us to live a longer healthier life. And yes, until the foundational biomechanics and bio-architecture is corrected with or without surgery, shoes promote bunions and hammered digits to form. Isn’t that a better price to pay than a knee or hip replacement, a spinal fusion, generalized weakness, and a collapsed posture or a fall?

RESPONSES/COMMENTS (NON-CLINICAL) PART 2
RE: Podiatric EMR for Dummies
From: Tip Sullivan, DPM

There are so many EMR solutions out there! It is overwhelming for an older practitioner to make an informed decision about which one to use. Many espouse podiatry-specific programs but just don’t deliver the assistance to help the “more mature” practitioner develop the skills to effectively use them. After attempting to incorporate EMR into my practice, I find that it takes me about twice as long to see and treat a patient than old charting methods. The company that I was using, which will remain nameless, simply did not “hold my hand” and guide me through the transition as they promised. A very frustrating situation for myself and my whole office staff.

am committed to make the jump into the present with EMR and would appreciate a book, “Podiatry EMR for Dummies” if anyone out there wants to write one. I am asking my colleagues to make suggestions and/or comments on their experience in conversion to EMR and perhaps suggest a company that will support an EMR dummy!

RESPONSES/COMMENTS (PODIATRY MANAGEMENT ARTICLES)
From: Doug Richie, DPM

Yesterday, I was quoted by Dr. DiPaolo in a post on PM News referring to a recent roundtable published in Podiatry Management (June/July 2022). He stated that, according to Dr. D. Richie, "in the literature, steroid injections are always cited as the most effective treatment of plantar heel pain.” This quote did not include my complete statement and fails to recognize that I was specifically comparing the effectiveness of NSAIDs to corticosteroids.

My actual statement reads: "That is why in the literature, steroid injections, not NSAIDs, are always cited as the most effective treatment of plantar heel pain." My response in the roundtable was to a question specific to the use of NSAIDs in treating plantar heel pain. In retrospect, it would have been more appropriate for me to clarify that corticosteroids are more effective than NSAIDs, but are not the single most effective treatment overall for plantar heel pain compared to other options.
