PRACTICE MANAGEMENT TIP OF THE DAY
Why and How to Stay on Top of Your Online Reputation - Part 1

1. Patients aren’t the only ones finding your reviews.

Physician reviews sites are designed with patients in mind, but patients aren’t the only people who see them. Your referral partners can also see what patients say about you—and how your reviews compare with a competing physician who just moved into town. Reviews can also influence job seekers and even potential business associates such as landlords, lenders, and payers. Basically, anyone with access to a browser and a search engine can potentially be influenced by what’s said about you on a ratings site.

QUERIES (CLINICAL)
Query: Restless Legs Syndrome (RLS)

I had a simple case scheduled in the OR this week and ran into a complication I had not seen previously. I suspect we have all had restless patients on the table, but I had a patient so agitated that we had to convert to LMA anesthesia before any prep or draping due to the almost violent movement of both of her arms and legs. Other than some IV sedation, there was no noxious stimuli to explain her reaction.

Once in PACU, the patient explained that she has suffered from this problem for years. She says she cannot sleep at night because of it despite her regular use of ropinirole. Since her primary seems to have ignored the severity of this problem, I want to help my patient by referring her for a specialist consult. I'm not sure where to send her - neurology, pain management? I admit to having very limited knowledge of RLS.  Any input is appreciated.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Preventing Suicides of Podiatric Residents
From: Patrick DeHeer, DPM

According to a recent study, “In the United States, approximately one physician dies by suicide every day. Training physicians are at particularly high risk, with suicidal ideation increasing more than 4-fold during the first 3 months of an internship year. Despite this increase, to our knowledge, very few efforts have been made to prevent the escalation of suicidal thoughts among training physicians. This study demonstrates that a free, easily accessible, brief [web-based cognitive behavioral therapy (wCBT)] program is associated with reduced likelihood of suicidal ideation among medical interns. Prevention programs with these characteristics could be easily disseminated to medical training programs across the country."

We created a survey based on the survey used by the St. Vincent internal medicine and family practice residencies to examine their residents' well-being. I believe the updated study is critically important. The podiatric profession can either continue to stick our heads in the sand on this topic or examine how it is affecting our future by helping to disseminate this survey.

APMA has agreed to help get the survey out to podiatric residents, and I hope your various organizations will join APMA's efforts to examine our own profession. I will be using my bully pulpit to get the word out. I hope we can help to disseminate our survey to the podiatric residency community. We plan to collect the data and present it to key stakeholders to hopefully develop a podiatric-specific wBCT.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Maintaining Podiatry’s Identity
From: Barry Swedlow, DPM

I am a retired DPM, board certified in podiatric medicine and orthopedics, and a licensed acupuncturist. I have used many modalities in my practice of 44 years. I use standard podiatric modalities and include chi gung massage, acupuncture, and homeopathy. It is my feeling that podiatric medicine should be an overriding profession with surgery as a subspecialty. We've had this discussion over the years. The inclusion of podiatric surgery subordinating primary care would be a death knell to the profession. It will eventually lead to the absorption of the profession by the orthopedists and lead to the loss of our unique abilities to the physical therapy and like-minded professions.

We have special gifts that help our patients and should not give up our non-surgical expertise to others. These have been the results of years of palliative care that should ever be our kin and ken. Tools this valuable part of our armamentarium will, to my view, be a detriment to our professional survival. We are all podiatrists; the advances of our surgical brothers and sisters are not to be denigrated, but to lose our imprimatur as palliative practitioners would, in my opinion, be a true and lasting loss to our profession and its patients.
