ACFAS Exhibits at AANP National Conference
Almost 5,000 nurse practitioners “took a new look at foot and ankle surgeons” last week at the American Association of Nurse Practitioners’ (AANP) 2016 National Conference in San Antonio. ACFAS President Sean T. Grambart, DPM, FACFAS and staff were on hand in the College’s booth during the conference and spoke to nurse practitioners from across the country on the value of referring patients to foot and ankle surgeons.

Dr. Grambart also spoke with AANP’s Orthopaedic Group Chair and nurse practitioner Michael Zychowicz on ways for AANP and ACFAS to collaborate on future referral and educational programs. The next stop on the campaign trail for “Take a New Look at Foot and Ankle Surgeons” is at the American Association of Diabetes Educators in San Diego in August and the American Academy of Family Physicians in September in Orlando.

Flip-flops are Definitely a Flop: PA Podiatrist
Rubber thongs may be your favorite grab-and-go summer footwear, but for the long haul, you need a sandal with more support. “While flip-flops may be the ‘go to,’ quick and convenient summer shoegear for some, they are not ideal for prolonged wear,” says Dr. Barbara A. Davis, a podiatrist at Gilbert Podiatry Associates in Gilbert.

“Flip-flops do not provide the adequate support needed, which may lead to flat feet and related problems such as bunions, plantar fasciitis, tendinitis, etc. Their thin soles do not provide adequate shock absorption.” Davis says that this could lead to stress being transmitted to the musculoskeletal system, which could cause knee, hip, and back pain.

Step 3: Don’t let the “1 star” reviews downturn your reputation.
To deal with any negative trends, it’s important for physicians to maintain a fair perspective. These days, it’s the reviews that can bring patients to your practice or cause it to dry up. The real challenge is to deal with the unfavorable/negative reviews from unhappy patients. If left unattended, these can cause havoc on your online reputation.

The impending MACRA implementation may be the canary in the coal mine for private practices of all types. MACRA is certainly a cause for concern. However, the Death Star scenario proposed by some may not prove to be entirely accurate.

Dr. Borreggine's predictive words are telling to say the least. Right now, PAs and NPs are being trained to handle the routine foot complaints most of us see on a daily basis. Will they do it better? Probably not, but insurance carriers don't care. It will be "adequate." Money, whether we like it or not, is the driving force in all of it. Greedy insurers, unethical lawyers and doctors are all driven by it. That fact will never change. The issue will be whether or not you as a physician are willing to accept it.
My bet is podiatrists will accept whatever is paid out to them. Years ago, the late and insightful Dr. Neal Frankel told me that he met with the CEO of a major insurance carrier who told him, "Why should we pay podiatrists more when we know they'll take less?" MACRA is coming with private insurance carriers to follow. You can either bend over and take it or stand up and rekindle some of the pride and integrity you had when you decided to enter podiatry school and get into this "business."

In response to Dr. Joseph Borreggine's comments about podiatry being an "unnecessary" profession and that hospitals have no desire to hire podiatrists, I am a hospital-employed podiatrist, as are many others in my area. I currently practice with 40 other providers including PCPs, neurologists, and general/orthopedic/GI/neurosurgeons. I perform very limited "routine foot care/nail care", maybe 2-3 patients per day out of 30. The main focus of my practice is the day-to-day things we all see, including fasciitis, neuromas, paronychias, etc., as well as a very strong surgical practice (8-10 per week).
I consistently rank in the top 3 producers in the group and typically rank as the number 1 producer in the group at least 6 months per year. So, with all due respect, I have to completely disagree with Dr. Borreggine's comments. I think hospitals will quickly find us very valued and welcomed team members once they are aware of our capabilities and realize that not all of us want to make our living selling patients unneeded crap out of our provider-owned retail stores. I personally feel there is a very strong future for podiatry in the hospital era of medicine.

In one study, patients preferred their physicians to be in stereotypical physician attire: white coat, name tag, and stethoscope over a shirt and tie for male physicians, and a dress for female physicians. Older patients and private insurance patients were less accepting of blue jeans. (Level of Evidence: 4)
In another study, 68% of patients surveyed felt that professional attire inspired the most confidence...

RE: Wrong Way Corrigan?
We've all seen patients wearing orthotics incorrectly: left in right shoe and right in left shoe. This patient was wearing her orthotics, shown in the photo, incorrectly for 3+ years. She obtained them from another provider. (You can see her feet imprinted on the top covers of the orthotics.) She came to see me for her chronic plantar fasciitis.

When I demonstrated the proper use and placement of her orthotics, she tried on her shoes and was impressed at how well they felt and noticed the support in her MLA. I advised her that if she wore them properly, as much as possible, when walking and standing, her condition would improve and may resolve.
