Ankle Replacement by WI Podiatrist Gives Patient "His Life Back"
After more than 30 years with the Michels Corporation, Ron Sabel looked forward to enjoying his retirement, but a troublesome ankle threw a wrench into those plans. Five years ago, Michael DeGere, DPM, performed reconstructive surgery on Sabel's ankle with the understanding that it would only be a temporary fix. When the arthritis and pain in his ankle continued, DeGere recommended a total ankle joint replacement. Since that visit, DeGere assumed a new role at Agnesian HealthCare as vice president of Population Health Management, and so he referred Sabel to his partner, Alyssa Stephenson, DPM.

"My life changed the day I met Dr. Stephenson," Sable said. Stephenson said, "Mr. Sabel is a very active gentleman who loves to work outside and spend time with his grandkids. Due to his activity level, I felt a total ankle replacement with an implant was the best option." After the surgery was complete, being the do-it-yourselfer he is, Sabel did his rehabilitation exercises at home. "Dr. Stephenson gave me my life back," he said.

NV Podiatrist Helps Aussie Patient Through Facebook
Peter Bregman, DPM, a podiatrist specializing in peripheral nerve surgery of the lower leg, recently helped a patient in Australia without even being there! Dr. Bregman was asked by one of his patients to join a CECS chronic compartment syndrome group on Facebook, so he obliged and joined. Little did he know that he would learn a lot from these patients, and that he would help some of them as well.

Bregman posted a comment on the Facebook page that indicated that a patient who was having surgery was to make sure that the common fibular nerve was to be decompressed in addition to just doing a fasciotomy. Recently, he received an unexpected thank you. The patient who had the surgery in Australia thanked Dr. Bregman for making that suggestion because the patient is now pain-free after the surgery and attributed this to Bregman’s help.

NY Podiatrist Offers Some Home Remedies for Common Foot Problems
If you can't afford to visit a professional, Dr. Suzanne Levine, a NYC podiatrist, shared a few home remedies for common foot problems. For minor calluses, Dr. Levine suggests to try adding gel orthopedic inserts to your shoes to reduce friction. You can also soak your feet in warm water with Epsom salt for 20 minutes. "Use a pumice stone to work over any thick areas of skin and finish with a moisturizer," Dr. Levine said. If your callus becomes painful and inflamed, you'll need a doctor. But whatever you do, don't try your luck at a nail salon.

Blisters are similarly a result of bio-mechanics, like poor-fitting shoes. "Blisters represent your body's attempt to protect a sensitive area of the foot," Dr. Levine explained. "Your best bet for minor blisters is to leave them alone. Cover the offended area with antibiotic cream and a Band-Aid. Do not pierce your own blisters," Levine warned. "In many cases this could lead to infection, inflammation, and pain. If that happens, it's time to see a doctor," she continued."

AAFAS Holds 40th Anniversary Scientific Meeting at LSU
The Academy of Ambulatory Foot and Ankle Surgery hosted its 40th anniversary international seminar on minimally invasive foot surgery at LSU Medical School June 11-13. 64 podiatrists and orthopedists attended from North America, Latin America, and Europe. Lectures were given by renowned international speakers, including Al Brown, DPM, one of the founders of the AAFAS. The course consisted of a cadaver lab and intensive didactic instruction on minimally invasive forefoot, rearfoot, and ankle surgery.

Procedures included treatment of bone spurs, hammertoes, metatarsals, bunions, tendons, as well as the use of the HyProCure. Biomechanical considerations were also discussed. A special emphasis was placed on the avoidance and treatment of complications. The next meetings will be at TUSPM, Argentina, and LSU.

Legal Challenges Remain for Health Law
The Affordable Care Act survived another major test at the Supreme Court this week, all but assuring its place in the social safety net, but the legal challenges to the law are far from over. One of the more significant tests is a lawsuit filed by Speaker John A. Boehner and other House Republicans who contend that the Obama administration is spending billions of dollars without the necessary appropriations from Congress. The government is also trying to fend off dozens of lawsuits challenging rules that require health plans to offer women coverage for contraceptives without co-payments or deductibles.
The suit filed by Mr. Boehner, in the name of the House of Representatives, is not just a bookkeeping dispute. It involves fundamental questions of executive power and Congress’s power of the purse. At issue is a provision of the healthcare law that requires insurance companies to reduce co-payments, deductibles, and other out-of-pocket costs for certain low-income people. The federal government reimburses insurers for the “cost-sharing reductions.” In their lawsuit, House Republicans say the Obama administration needed, but never received, an appropriation to make these payments to insurance companies. As a result, they contend, the spending violates the Constitution, which says, “No money shall be drawn from the Treasury, but in consequence of appropriations made by law.”

I have a colleague whose standard of practice in destruction of symptomatic verrucae is to use topical applications of silver nitrate. Can such applications reasonably be considered "destructive"?

Dale Feinberg, DPM wrote a nostalgic message entitled “Gone With the Wind” about the demise of podiatry as we knew it, and compared the demise with the recent criticism of the confederate flag that we’ve all seen in the news. His thesis is that there was something virtuous in each institution and we are “letting” them wither away. Dr. Feinberg and I were roommates freshman year at CCPM in 1978, and shared a few important life experiences together. I think his feelings are sentimental and yes, I can be sentimental at times as well. But fundamentally, I have believed for 20 years that the private practice fee for service model that we all enjoyed during these years was doomed to be replaced by a system with much different payment dynamics.
Frankly, I hated billing insurance companies for services. I never felt empowered in that situation, and always felt like I was going to Dad for money, only to have him judge whether my “claim” was worthy to be paid. I felt I had done the correct service for the patient, but that’s not what got me paid. Submitting...
