OH Podiatrist Wants VA Accountability for Long Wait Times
After the unearthing of secret wait lists for Veterans Affairs hospitals across the country, U.S. Rep. Brad Wenstrup, R-Cincinnati, met with Ross County veterans Friday to discuss the benefits of allowing veterans access to private practice physicians and VA doctors. “You really have to scratch your head because you can’t believe this kind of thing happens,” Wenstrup said about the lack of medical service to veterans.

“In a private practice, where their reputation is on the line, they are going to follow up with you,” Wenstrup said, speaking to veterans gathered at Veterans of Foreign Wars Post 108. A native of Cincinnati, Wenstrup worked as a podiatric surgeon for 26 years in private practice before being elected to the U.S. House of Representatives and becoming a member of the Veterans Affairs Committee. He also is a lieutenant colonel for the U.S. Army Reserve and is serving on a joint committee evaluating VA reforms. “This culture is broken, and it doesn’t work,” he said. “I don’t see any reason why someone can’t be a VA provider and not have their own practice.”

GA Podiatrist Discusses PAD on Local Top Docs Radio Show
Mohammed Sharif, DPM of Village Podiatry Centers, recently joined fellow doctors from Advanced Vascular Resources of Atlanta and Hyperbaric Physicians of Georgia in an appearance on Top Docs Radio. Together, the doctors educated listeners on the multi-specialty team approach essential in achieving the best outcomes for patients with limb-threatening wounds.

Dr. Sharif is a board certified podiatric surgeon who routinely performs vascular studies to determine if peripheral artery disease (PAD) is present in his patients who appear to be at risk. Common risk factors include smoking, diabetes, obesity, high blood pressure, and family history. If you notice leg numbness, a change in color of your legs, painful cramping in your hip, thigh, or calf muscles after activity, then you may need to see a foot and ankle specialist such as a DPM as well as a certified wound specialist.

CA Podiatrist Faces Illegal Prescriptions Charges for Second Time
A San Leandro podiatrist who was convicted of selling illegal prescriptions in 2008 has been charged with two new counts of the same offense, authorities said Friday. Alameda County District Attorney Nancy O'Malley said in a statement that Dr. Tan Nguyen, 62, had written more than 5,000 prescriptions -- mainly for addictive narcotic painkillers hydrocodone and oxycodone, the muscle relaxer carisoprodol, and the anti-anxiety medication diazepam -- in the last year.
On June 17, an FBI agent posing as a patient called Nguyen's office; he picked up the phone himself, according to a probable cause statement. "What kind of pills are you expecting to get?" he asked the agent, according to the statement. "If you want pills, all I can give you is Norco (a narcotic painkiller) ... no oxycodone." When the agent arrived at the office two days later, Nguyen made a copy of her identification and told her, "I can only give you 100 Norco," according to the statement.

OH Podiatrist Introduces Graphical Foot Pain Diagnostic Tool
Myfootshop.com recently introduced an interactive graphical tool to their website that is used to help customers diagnose foot and ankle problems. The tool employs images of the foot and ankle that are activated with 'hot zones'. Each hot zone is associated with conditions of the foot and ankle. Click on a specific zone to research the foot and ankle problems found in that particular part of the foot. Each image comes with a graphical overlay that details anatomical landmarks of the foot and ankle.

"The new graphical foot tool is a great way to narrow down your diagnosis," said Jeffrey Oster, DPM, medical director of Myfootshop.com. "Our goal is to guide our customers into the right diagnosis and the right product, the right way. Query: Records Release to Third-Parties
Once records are provided to the patient as per a request, then if the patient authorizes another party to have those records, can you demand additional payment from that third party?

Editor's note: PM News does not provide legal advice. Under HIPAA and most state laws, patients are entitled to a copy of their records. New York and other states allow a reasonable fee to be charged, but if the patient refuses to pay the fee, you must still provide the records. You can then sue the patient separately in small claims court. The policy on third-parties is less clear, but obviously the third-party could simply direct the patient to make the request to have the records sent directly to the patient, and you would have to comply.

I recommend a standard Kidner procedure with PTT re-attachment via any available tendon anchor. I prefer the Arthrex absorbable one.
Remove the prominent navicular along the medial column to prevent any prominence medial. Anchor the tendon with a Mite or Arthrex bone anchor. Have no fear, the anchors work great, are easy to use, and a 13 year old will heal like a champ. Once you have incised down to navicular, you will find that the tibialis posterior tendon fiber extends to every tarsus bone except the talus. Thus, you cut parallel to the tendon to bone and reflect superior and inferior. Remove the prominent bone, place a bone anchor in the residual navicular. The suture that is attached to the bone anchor is used to re-attach the tendon and tissue you reflected.

Everyone knows the old saying "if it sounds too good to be true, it probably is." Given the limited details that Dr. Rubin has shared, my initial impression is that the "Pharmacy-Based Study" sounds like a recipe for disaster. At a bare minimum, any clinical trial involving human subjects must first be approved by an Institutional Review Board (IRB) in order to comply with FDA regulations. Even a clinical trial involving a commercially available medication or device must have an IRB-approved written protocol with an informed consent form, a clinical trial agreement (i.e. a contract between you and the sponsor), an investigator's manual, case report forms, and a litany of supporting documentation that is beyond the scope of this email.
The IRB will require you to file periodic written reports regarding details of the progress of the clinical trial. Without these things, you will be figuratively burned at the stake if you are audited by the FDA. Clinical research may sound like 'easy money', but having conducted many clinical trials (one of which was audited by the FDA a few years ago), I can tell you there is nothing easy about it.
