PODIATRISTS AND DIABETES

Podiatrist Meets with Florida Surgeon General

Dr. Chet Evans, chair of the Governor's Diabetes Advisory Council, met with Florida Surgeon General Dr. John Armstrong to discuss issues involving Florida citizens that have, or are affected by, diabetes. Evans said, " Diabetes is the 6th leading cause of death in Florida, costs the state in excess of 10 billion healthcare dollars annually, and one out of ten of Florida's citizens have received that diagnosis by their doctor. Another one in ten have pre-diabetes or are undiagnosed diabetics."

(L-R) Drs. Chet Evans and John Armstrong

The chair of the Diabetes Advisory Council meets yearly with the Surgeon General to propose recommendations that can benefit those who suffer from the burden of diabetes. Dr. Evans commented, "The Surgeon General's Healthy Weight Initiatives will help to significantly address the scourge of diabetes in Florida, because we know that obesity is the predominant risk factor leading to this disease. The Council looks forward to working with him in that regard."

PA Podiatrist Provides Nail Preservation Tips for Runners

One of the side-effects of running is the bruising, loss, or thickening of toenails. It isn't pretty. "It comes from repetitive trauma of the nail against the shoe," said Nick Taweel, doctor of podiatric medicine at the Rothman Institute at Jefferson. But it doesn't have to be this way for everyone, Taweel said. "I guess in a way it's part of the sport, but it certainly can be prevented," he said.

Dr. Nick Taweel

First, he said, make sure your shoes fit properly "so there's no pressure on the ends of your toes." Then tie your laces so that they're snug but not super-tight. "If the laces are loose or not tied properly, the foot can slide forward," he said.

INTERNATIONAL PODIATRISTS IN THE NEWS

Malta Podiatrist Provides Summer Nail Care Tips

Our feet will take most of the heat this summer as we trade shoes for sandals or even go barefoot. Podiatrist Yvonne Grattan explains that aside from making our toes look good, foot care is beneficial to our health. Let us start with the nails. "It may seem like cutting our nails is straightforward, but unfortunately, it is not. Incorrect cutting can cause serious problems. It is important to follow the natural line of growth (where the pink becomes white) and leave a millimeter or two of free white edge. Cutting the nails to the skin is never a good idea," says Grattan.

Podiatrist Yvonne Grattan

Nail plates should be a healthy pink and fairly smooth, even their texture. If not, get them checked by a podiatrist. "Changes can be due to various disorders and health issues, and foot ailments can be a sign of more serious medical problems," says Grattan.

QUERIES (MEDICAL-LEGAL)

Query: Locum Tenens After Death of Practitioner

How long can a clinic legally operate after the solo practitioner passed away? Can it be longer than the 2-month locum tenens?

Name Withheld

Editor's comment: PM News does not provide legal advice. The maximum time period after the death or disability of a practitioner is 60 days (Source: Medicare A/B Reference Manual, Chapter 12.4).

QUERIES (NON CLINCAL)

Query: HSA and Laser

Is the use of laser treatment for fungus nails a payable expense under a patient's HSA? I have seen several different opinions about whether or not this is considered a cosmetic procedure and thus, according to the IRS, a non-deducible medical expense and therefore a non-covered item.

QUERIES (DME)

Query: Effect of DME Competitive Bidding on Podiatrists

I would like to know how competitive bidding for DME products in large cities like Miami has affected podiatrists dispensing diabetic shoes, braces, wound products, etc.?

RESPONSES/COMMENTS (CLINICAL)

RE: Tibial Torsion and Femoral Anteversion
From: Joel Lang, DPM

After many years of treating children for rotational deformities and reading on the subject, I found my "truth" in my own observations. Have you noticed that it is very rare to find a 'neurologically intact' adult with significant rotational deformities of the lower limbs. Watch people walk down the street. How many rotational deformities do you see. Virtually none!

So, must we conclude that all the rotational deformities of childhood for these people have been treated effectively, or is something else at play? Most neurologically intact children will "unwind" at age 6 and, if not, will probably have to wait until age 12.

If the rotation is not functionally problematical, I suggest you leave it alone or treat with minimal inconvenience and expense. You can then take credit for that which would have self-corrected anyway. Remember, children fall down mainly because they are still learning to mobilize and sometimes, as I have told parents, "The child's mind writes checks that the body can't cash." Be patient, supportive, and watch the magic of maturation.

RESIDENCY SHORTAGE CRISIS - PART 1A

RE: Unmatched Residency Placements Currently Stand at 92 (Stuart J Wertheimer, DPM)
From: Paul Kruper, DPM

Dr. Wertheimer has a misguided viewpoint of responsibility. Whoever makes the decision is responsible for the outcome of the same decision. He blames the profession for not correcting the damage caused by CPME. That is like Dr. Wertheimer using an experimental instrument to perform surgery on a patient, then when the surgery goes bad, blaming the podiatry profession for not doing enough research on the instrument he chose to use.

The CPME chose the three-year residency instrument, and it was up to them to ensure that instrument would cause no harm. They failed and now students are being harmed. CPME, not the profession, is now  responsible for correcting the harm caused by their choices. The lesson for CPME is to do the homework and research to evaluate the consequences before making a decision. Now fix it.

RESIDENCY SHORTAGE CRISIS - PART 1B

RE: Unmatched Residency Placements Currently Stand at 92 (Stuart J Wertheimer, DPM)
From: Name Withheld

My experience with the CPME and residency expansion under the direction of Dr. Wertheimer is much different than what I just read. I was a student rotating at an incredibly busy and prestigious residency program. The program was applying for additional residency positions. It was time for the site visit when two individuals representing the CPME showed up. They sat at the table in the conference room and called for the director.

The director came in and they didn’t so much as stand up to greet him. He offered them a tour of the facility. To his shock, they said...
