2 Podiatrists Appointed to the Podiatric Medical Board of California
 
Governor Gavin Newsom has announced the appointment of two podiatrists to the Podiatric Medical Board of California. Devon Glazer, DPM has been CEO and podiatrist at Artisan Foot and Ankle Specialist since 2009. Dr. Glazer is a member of the California Podiatric Medical Association, American Podiatric Medical Association, and the American College of Foot and Ankle Surgeons. Glazer earned a Doctor of Podiatric Medicine degree from the New York College of Podiatric Medicine.

Sumer Patel, DPM has been Chair of Foot and Ankle Surgery Chiefs at the Permanente Medical Group since 2016. He has been Assistant Physician in Chief at the Kaiser Permanente Santa Clara Medical Center since 2022 and Physician Operating Room Director there since 2013. He has been a foot and ankle surgeon at the Kaiser Permanente Santa Clara Medical Center since 2000. Dr. Patel is a member of the American College of Foot and Ankle Surgeons. Patel earned a Doctor of Podiatric Medicine degree from the California College of Podiatric Medicine.

Police Investigate Break-in of OH Podiatrist's Office 
 
Police are investigating a break-in at a local podiatrist’s office in which petty cash was taken. Police were called to the office of E. F. Cosentino, DPM in Girard, OH.

They arrived to find damage to a door handle, and a man at the practice reported some cash was missing. No other items appeared to be missing or moved, according to a police report.

Common Pitfalls in a Doctor's First Year in Practice
 
Congratulations for making it through medical school and residency, and even perhaps a fellowship, and now you are ready for your first or second job out of school. Here are some areas that trip up new doctors the most.
 
4. Know the importance of compliance and how to report problems.
 
You don’t want to be the office tattle, but what if you start to see something in your first job or two that raises red flags? How do you handle it? If you are working for a small group, make sure they have a compliance plan. Make sure there are some basic audits of the billing and coding in your practice. Hiring your own auditor is a good thing! So many doctors think they know how to bill and code but then end up messing it up, and that can really cost you. Remember that if your partners are messing things up, it can come back on you as a member of the practice. If your APPs are billing out under your national provider identifier (NPI) and not following the "incident to" rules, that comes back on you. So you don’t have a choice to bury your head in the sand. You need to know the basics of compliance.

On a trip to India, a common treatment for heel spur syndrome there was demonstrated to me by a local physician. A small, pointed hammer was heated and applied to the plantar heel, burning the skin.

Yes, Dr. Sullivan, prolotherapy has been around and used by podiatrists for a long time. My uncle started his podiatry practice in 1934 and my father joined him in 1943. They used prolotherapy using sodium moruate and/or alcohol, I believe. They were no longer using it when I joined them in 1982 but it was presented to me in a positive light. It seemed to me at the time that uncle was more focused on palliative care and orthotics and my Dad on office-based 'open' surgery. 
 
I never learned the technique but believe that it has a place in ligamentous and other soft tissue conditions.

Like Dr. Kornfeld has said, it is up to you to determine your future. You can become a tool of the insurance industry or decide to do something about it. The easiest route is to take the insurance but as you have seen, it probably is not the best route. It takes guts and fortitude to fight forward and take chances. With the insurance, patients will come to you as it is the easiest route. In direct or concierge care, you will have to seek out the patients. It is the exact opposite situation. 
 
While I entered practice in an affluent area, the medical community was very anti-podiatry, so it was not easy but with persistence over time, I developed a great following of cash patients. I have many working people who really should be going to insurance doctors but they want the services that I offer that others do not offer. We do take payments to make it easier and almost all patients are happy to honor their commitment and pay off the services they received from me on time, as I delivered the care they sought.

Over the years, I have observed Dr. Robert Kornfeld describe that he jettisoned himself free of insurance coverage and made a success out of it. He practices in New York. Apparently, it works for him. I know of MDs and DOs who established themselves in a so-called holistic practice. I just never could understand how a highly specialized practice, say separate from gastroenterology or cancer care, could encompass a holistic medical treatment style. He does not mention if he practices any foot surgery and how any patients would be willing to self-pay for that.
 
Perhaps some of them obtained partial reimbursement from their insurance. But most insurance panels have strict rules about how doctors must exist in their framework. They might require separate exclusion contracts in order to avoid...

RESPONSES/COMMENTS (MEDICARE ADVANTAGE)
RE: Medicare Advantage Plan Expenditures

As a follow-up to my recent letters regarding Medicare Advantage Plans and where all the money CMS pays them goes (and doesn't go), recent postings from several Medicare Advantage Plans advertised openings for a Director of Medicare Risk Adjustments. The salaries paid range from mid $150Ks to the mid $300Ks, and the CEOs of these companies have base salaries of no less than $8M, with some having 8 figure salaries.
 
In comparison, most medical directors of Medicare contractors are paid far less in the mid-$200K range, equivalent to what many MDs/DOs are making. The point is that it pays to go to business school, get an MBA, and 6 years post high school, you can go to work for a Medicare Advantage Plan (and that is perhaps where the Advantage name is correctly used). These individuals have the potential to be making enormous salaries with much less debt and far more working years, during which you can be funding retirements, your children's education, etc. So now do you wonder where the $ is?

RE: Spending Habits on Medical Procedures

I live on the edge of a very wealthy community. It is overloaded with restaurants, boutiques, and some interesting quasi-medical practices. One practice advertises I.V. nutritional infusions. Another office offers salt cave breathing. I happened to speak to the owner of the I.V. place. His comment to me was surprisingly different than what I assumed. This place does well, ”but my place in ___________ does fantastic.” This is a town that is relatively poor. 
 
This gives credence to Dr. Kornfeld’s philosophy. Social media now drives all this medical thinking, sophisticated and unsophisticated as well. My former office was in a lower middle class neighborhood and I practiced to that level. My dental neighbor was a high fee cosmetic dentist doing quite well. As I walk around, people are constantly plugged in, and that is the future.
