PODIATRISTS IN THE NEWS
Leave Scalpel Removal of Calluses to a Professional: CA Podiatrist

Calluses on your feet can be thick, hard, and even painful. Depending on the severity of your calluses, you may need to head into the podiatrist's office for proper treatment. But, if you're able to manage them at home, you need to be careful about what tools you use. Diane Koshimune, DPM, a podiatrist in San Jose, California, says you want to keep scalpels and pedicure knifes out of your DIY pedicure kit.

Dr. Diane Koshimune

"More and more people come in stating that they have used a scalpel on themselves," says Dr. Koshimune. "Of course, some people are quite flexible and have a steady hand where they might be safe in using a scalpel blade on themselves. But due to limitations in the visibility of certain parts of the foot—for example, the outer border of the foot, the pinky side—using things like a scalpel to reduce or remove calluses can cause injury to the skin and subcutaneous tissue layers."

PODIATRISTS AND PODCASTS
VA Podiatrist Discusses Biomechanics and Orthotics with UK Professor

On his podcast Fit Foot U, Ben Pearl, DPM recently interviewed Nachiappan Chockalingam, PhD, a world renowned expert in biomechanics and a professor at Staffordshire University. Dr. Chockalingam was part of an initiative by the World Heatlh Organization to examine the cost-effectiveness of orthotics in treating medical problems. The following video is a segment of the interview which reviews the importance of repeatable measurements and calibration of devices in biomechanics research.

Click on photo to view short video

Dr. Pearl asked Dr. Chockalingam if he had any opinion of the effectiveness of the type of orthotics that are widely used by chiropractors. Chockalingam responded that orthotics are made to solve a problem for a particular patient, and studies to examine the effects on patients for different problems are a challenge. He referenced one particular study in which 5 top podiatrists and 5 top pedorthists made different orthotics for an athlete, but they all helped to some degree.

PRACTICE MANAGEMENT TIP OF THE DAY
Tips to Get a Raise from Your Payers

Negotiating a successful payer contract increase is possible for those willing to craft a thoughtful, data-driven argument for why they deserve more money.

8. Negotiate carve-outs for high-utilization services.

For example, your practice may be able to negotiate higher rates specifically for office visits to maximum revenue impact, Andrew Harding, co-founder and vice president of customer success at Rivet says.

HEALTH INSURANCE NEWS
Anthem is Now Elevance Health

The company formerly known as Anthem commemorated its official rebranding to Elevance Health on June 28 by ringing the opening bell at the New York Stock Exchange and beginning to trade under the new ticker symbol "ELV". The former Anthem website now reflects the name change, which is a combination of the words elevate and advance to represent the company's commitment to "elevating the importance of whole health and advancing health beyond healthcare for consumers."

When it first announced the rebrand in March, the payer said Blue Cross Blue Shield health plan names would not change, though it planned to narrow the number of brands under its umbrella. The company owns BCBS plans in 14 states. On June 15, the company launched two new subsidiaries under the Elevance name: Carelon and Wellpoint. Carelon, a healthcare services brand, will consolidate the company's existing portfolio of capabilities and services businesses under a single name. The Wellpoint health plan brand will unify the company's Medicare, Medicaid, and commercial health plans in select markets.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Graduate Successes
From: Danae Lowell, DPM

I would like to take a minute to congratulate all our new podiatry college graduates. I am always excited to hear from them about where they are going, what kind of practice they’re going into and, if they choose to share, the compensation they’re receiving. After years of college and residency, they finally get to reap the rewards of their hard work and dedication to our field. Without providing a name to protect their privacy, one of our graduates is joining a multi-disciplinary practice owned by a podiatrist.

The new graduate is making just shy of $200,000 plus a bonus structure and has no on-call requirement in an area of the country where the cost of living is moderate. This is one of many I’m aware of and I’d be interested to hear about more of these success stories from others. If you have an example such as this, please share. I love the continued growth and practice potential this profession has to offer our young colleagues.

RESPONSES/COMMENTS (NON-CLINICAL) PART 2
RE: Beware of Latest Licensing Scam
From: Michael Schneider, DPM

There is a scam going on in which you get a call from your state licensing asking if you are aware of a complaint against you. When you say no, the caller wants to connect you with an investigator. I hung up and checked the phone number that it came from, and it was the actual telephone number of DORA (Colorado). I called DORA and was informed that it is a scam and the scammers have been able to make it look like the call is coming from your state agency.

RESPONSES/COMMENTS (NON-CLINICAL) PART 3
From: Steven Finer, DPM, John Lanthier, DPM

During my residency, 40 plus years ago, the two orthopedists were podiatry-friendly. We scrubbed into all their cases and did all the heavy lifting for them. They took us to the clinic where we injected, examined, and cast patients. Along came a new group from a large teaching hospital. The head doctor said, “feet are for walking in so orthopedists can fix their bodies.” Two months later, he was singing our praises and his hospital established a high power podiatry residency.

Steven Finer, DPM, Philadelphia, PA

When I was first applying to have surgical privileges at the main hospital in town, a spinal orthopedist reviewed my surgical logs as we sat in a conference room. He was deciding on whether to approve my application. He said, “You better not f*** up any pediatric patients.” I didn’t know where this was coming from as I was just new in town and on staff. I was too concerned about getting privileges so I didn’t retort. However, after seeing his surgical spinal failures come through my office in the coming years and subsequently learning about the poor success rates of spinal surgery, I could have told him the same about his surgeries.

RESPONSES/COMMENTS (PODIATRY MANAGEMENT ARTICLES)
From: Vincent T. DiPaolo, DPM

I really enjoyed the article on heel pain in the June/July 2022 issue, but please note these two conflicting statements: Dr. D. Richie: “…in the literature, steroid injections are always cited as the most effective treatment of plantar heel pain.” Dr. L. Weil, Jr: “There are virtually no studies validating the benefits of cortisone injections for heel pain.” Personally, I find them very helpful in most cases. Any comments?
