PODIATRISTS IN THE NEWS
MI Podiatrist Offers Summer Foot Health Tips

Wearing sandals, flip-flops, or going barefoot this summer? Don’t forget to give your feet a little TLC. “During this time of year, we see many foot injuries or problems due to non-supportive footwear and barefoot walking,” said Dr. Jodie Sengstock, past president of the Michigan Podiatric Medical Association. “Pool areas and public showers are home to infections such as planter warts and athlete’s foot, while the sun’s powerful rays can cause painful sun burns.”

The Michigan Podiatric Medical Association advises limiting walking barefoot because it exposes feet to sunburn, as well as plantar warts, athlete's foot, ringworm, and other infections, and also increases risk of injury to your feet. PODIATRIST RESIDENTS IN THE NEWS

NY Podiatry Residents Win First Place Prize in Writing Contest

Dr. Daniel Pollack, chief resident, along with two first-year residents, Drs. Scott Carlis, and Shaylyn McTeague were awarded first place honors in a writing contest that involved numerous submissions from each of the residency programs at Wyckoff Heights Medical Center in Brooklyn. This included entries from internal medicine, general surgery, obstetrics and gynecology, pediatrics, dentistry, and dental anesthesia residents. The contest is for original writing and research.

(L-R) Drs. Scott Carlis, Daniel Pollack, and Shaylyn McTeague

The title of their winning article was “Peroneus Quartus Autograft: A Novel Approach in the Repair of a Split Peroneus Brevis Tendon Tear.” This is the second year in a row that the podiatry department at Wyckoff Heights Medical Center has taken home the first prize. Last year, podiatry wound fellow Dr. Brittany Winfield received first place honors and podiatry resident Dr. Tahir Khan took second place.

INTERNATIONAL PODIATRISTS AND THE LAW
NZ Podiatrist Fined Over Unlicensed Practice

A podiatrist who practiced without certification for more than six years says she did it so residents in the town wouldn't have to make costly visits out of the district. Nina Williams worked as a podiatrist without holding a current practicing certificate between April 2007 and May 2013, a New Zealand Health Practitioners Disciplinary Tribunal has found. From May 2006, Williams worked part-time at the Rangitikei Health Centre in Marton but did not apply for a new practicing certificate when her certificate lapsed on March 31, 2007. She was a registered podiatrist, however. Williams told the tribunal she had recommenced practice after being approached by her regular clients in public, but worked for no more than one day a week and did not advertise her services. Williams was censured by the tribunal, fined $2,500, and ordered to pay $2,500 towards the cost of the tribunal.

RESPONSES/COMMENTS (CLINICAL)
From: Terry Mueller, DPM

The partially fused syndesmosis/synostosis of the accessory navicular is easily shelled out from the plantar calcaneo-navicular ligament/tibialis posterior tendon, leaving this and the other 8 insertion points of the tendon intact. This not only obviates the need for "anchors" (and avoiding pressure necrosis of that which you're trying to repair) but allows for reconstructive repair, to restore the altered leverage of the primary insertion point of the TP tendon to the medial aspect of the body of the navicular. I have utilized 3-0 PDS sutures for this since I first became interested in TPD in the early '80s with very satisfactory results.

If the APMA still has the TPD video in their library for members, videos/discussion of the actual technique of repair can be reviewed. Or, see my chapter "Tibialis Posterior Dysfunction" in the book Current Therapy in Podiatric Surgery (C.V. Mosby), or contact me for a copy.

RESPONSES/COMMENTS (MEDICAL-LEGAL) - PART 1
From: Brian Kiel, DPM

In response to Dr. Rubin and as a follow-up to my initial letter, no legitimate study charges patients or their insurance company for the product being studied. In a legitimate study, all product is supplied and the patient may or may not be paid. These faux-studies merely produce thousands of dollars for the compounding company. Not only that, what do you really know about them, where they are located and what sort of quality control is utilized? I think we should all stay far away from this type of program (scam).

RESPONSES/COMMENTS (NON-CLINICAL)
From: David J. Freedman, DPM

Please note the proposed LCD was pulled on June 27, 2014 by Novitas through the CAC process and leadership of both myself in the JL jurisdiction, and Dr. Paul Kinberg of the JH jurisdiction. We were able to convince Novitas that this was a bad LCD. We had several calls that culminated in a final meeting on June 26th with the carrier medical directors. The decision to cancel implementation of the hyperbaric future LCD was worked out. They will maintain the current LCD in place until further notice.

RESPONSES/COMMENTS (CODINGLINE)
From: Simon Young, DPM

My feeling is that we should not be treating psoriatic lesions. More importantly, we need to be on the lookout for psoriatic DJD, nail changes, joint changes, and other systemic symptoms. We are extremely valuable in the early diagnosis and referral to a dermatologist or rheumatologist with psoriatic expertise of this frequently missed malady.

Humira (TNF blocker) is frequently prescribed to eradicate the psoriatic lesions.

RESPONSES/COMMENTS (NEWS STORIES)
From: J.C. Mahaffey, MS, Joseph G. Smith, DPM

The American College of Foot and Ankle Surgeons (ACFAS) first issued a position statement on this topic in 2004. Our latest position statement on cosmetic surgery may be found at www.acfas.org/positions.

J.C. Mahaffey, MS, CAE, Executive Director, ACFAS, Mahaffey@acfas.org

My philosophy has always been, “If it doesn’t hurt, don’t fix it.” Having witnessed numerous bad outcomes from failed cosmetic procedures that took pain-free feet and ankles and turned them into problems, I have no difficulty talking people out of unnecessary cosmetic surgeries for unsightly feet.
