PODIATRISTS IN THE NEWS - PART 1
NY Podiatrist Explains Plantar Fasciitis

Imagine the arch of your foot is the arch of a wooden bow. The plantar fascia is this taut string between the two ends of the bow — the ball of the foot and the heel. Every step a person takes, the arch of the foot collapses under their weight, and the plantar fascia stretches, says Bryan Markinson, DPM, a podiatrist at Mount Sinai in New York City.

Dr. Bryan Markinson

After thousands of daily steps over decades, the plantar fascia can sometimes say, “I’ve had enough,” Dr. Markinson said. “You can get plantar fasciitis from running off a bus,” Markinson said. “When you land on the front part of your foot, your heel is in the air and you’re asking the plantar fascia, for a split second, to bear the whole brunt of your body’s weight.”

PODIATRISTS IN THE NEWS - PART 2
TX & OH Podiatrists Discuss Bromhidrosis

Feet are designed to stink. They have more sweat glands than your armpits do. "Your feet are doomed to smell because apocrine glands, like the ones in your underarms, produce perspiration on the soles of your feet," Leslie Campbell, DPM, a podiatrist and member of the American Podiatric Medical Association, told Vice. With your feet being essentially a sweat generator, they also attract bacteria, and when bacteria gobble up the sweat, a repugnant stench is born.

(L-R) Drs. Leslie Campbell and Joy Rowland

During warmer months, you can trade your sneakers and flats to open-toed sandals to give your feet some room to breathe. That way, they won't have to be forced to stay in a cramped space where bacteria are sure to thrive. But if you insist on wearing closed shoes, see to it that the shoes are really clean by disinfecting them with a sanitizing solution. "Take the insole out of the shoe, lightly spray the insole, and let it dry for 24 hours," suggested podiatrist Joy Rowland, DPM of the Cleveland Clinic. "Then when you put the insole back in the shoe, you've treated the shoe."

PODIATRISTS AND FORENSIC MEDICINE
Like Fingerprints, Footprints Can Be Valuable Evidence: IN Podiatrist

Michael Nirenberg, DPM, a clinical and forensic podiatrist who is the president of the American Society of Forensic Podiatry, said that “in the broad population, footprints are distinctive and individualistic ... and you can see distinctive features in footprints. And you can compare two footprints to see if they have the same features.”

Dr. Michael Nirenberg

Still, they have not been scientifically proven to be unique, Dr. Nirinberg said. “So there’s a gap in our knowledge and we need to give the benefit of that to the accused,” he said. “I would not want someone convicted solely on a footprint.”

PODIATRISTS AND FOOTWEAR
NY Podiatrist Recommends Sneakers For Jobs That Require Standing

If you're a nurse, a waiter, a professional marathon runner, a nanny—just to name a few—you know the aches and pains that can occur when you're moving around 24-7. As Amnon Barnea, DPM, of Walk NYC Podiatry says, "Being on your feet all day causes significant strain on the 26 bones, 30 joints, and more than 100 muscles, tendons, and ligaments in each foot."

Dr. Amnon Barnea

From sneakers to loafers to heels to sandals, you can find a pair of shoes that are comfortable and supportive. Dr. Barnea says, "It is advised to wear shoes that provide both cushioning and stability. The one without the other will likely increase fatigue, pain, and a decrease in duration and functionality." When it comes to exactly what kind of shoes to buy, he adds, "Sneakers would be the preferred shoe to wear all day."

PRACTICE MANAGEMENT TIP OF THE DAY
Common Pitfalls in a Doctor's First Year in Practice

Congratulations for making it through medical school and residency, and even perhaps a fellowship, and now you are ready for your first or second job out of school. Here are some areas that trip up new doctors the most.

3. Always protect yourself and have an advocate fighting and reviewing things for you.

Never trust another lawyer, another doctor, or another partner without doing your own homework. Trust, but verify. This is so important. You don’t always have to tell your potential new partner or your employer that you are calling your lawyer for advice. Your communication with your lawyer is privileged and confidential, and they can be a sounding board for you. If you don’t have a good relationship with your lawyer, find a another one. But think about it when someone says, “This has been vetted by legal.” Who does that lawyer work for? If it’s not you, get your own opinion.

QUERIES (NON-CLINICAL)
Query: Fee for Prolotherapy?

Just wondering how much people are charging for these injections? By the way, after looking into it further, prolotherapy has been around in podiatry a long time - think about it - Canthrone induces an inflammatory response and it has been around a long time. I don’t know about dextrose?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Have We Forgotten Why Podiatry Exists?
From: Lawrence Rubin, DPM

Our beloved podiatry is in peril. We've forgotten why we are a profession in the first place. In my first day as a student at the then Illinois College of Podiatric Medicine (ICPM), I heard Henri DuVries say, "You are about to become a member of a wonderful profession, because you will give comfort and healing to a part of the body that all of us MDs are neglecting. Never forget that, or you will eventually have no purpose."

Lawrence Rubin, DPM, Las Vegas, NV

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: Transfer of Residency Program
From: Gary M. Lepow, DPM

The long-standing residency program at St. Joseph Medical Center in Houston has received approval by the Council on Podiatric Medical Education for a transfer of the residency program to the Village Health - Heights General Hospital in Houston, beginning July 1st, 2023. The program will be under the supervision of Sohail Rao, MD, MA, DPhil, MBAe (Chief Academic Officer and Designated Institutional Official - GME.) and Founder/Director of the Texas Institute of Graduate Medical Education and Research.

I will serve as the Podiatric Medical/Surgery Director. I invite any podiatric student to come visit the "transferred" program and/or apply for the student, one month rotation. The new transferred program will be interviewing at the January CRIP interview program, for 4 incoming first-year residents who will begin July 1, 2024. The program will highlight a residency clinic, limb salvage program (directed by Dr. Randal Lepow), and a three-year educational program promoting medical and surgical research.

I would like to thank the CPME for their decision allowing the program transfer and to all our Greater Houston podiatric faculty who have given their time and expertise to educate our residents in becoming excellent and compassionate healthcare providers.

Gary M. Lepow, DPM, Houston, TX

RESPONSES/COMMENTS (NON-CLINICAL) - PART 3
From: Robert Kornfeld, DPM

Dr. Belanger asserts that, "Not everyone has the ability to have a concierge practice or a holistic practice that does not participate in any insurance plans if someone practices in a relatively low income area." Dr. Belanger - that is simply your own belief. As adults, we have freedom of choice. No one is a victim of circumstance to their own prior decisions. I started my career in a relatively low income, blue collar town. And when it worked for me, it was great. But I had bigger aspirations than to sit around watching self-serving insurance companies whittle away at my income. I was very lucky because insurance companies and Medicare way back in the '90s began to target my practice and deny claims for anything they considered "holistic". So I developed a "hybrid" practice before I ever heard the term.

Yes Dr. Belanger, even in my town, people were willing to pay me directly for...

Editor's note: Dr. Kornfeld's extended-length letter can be read here.
