Tips to Get a Raise from Your Payers
 
Negotiating a successful payer contract increase is possible for those willing to craft a thoughtful, data-driven argument for why they deserve more money.
 
7. Perform a payer analysis.
 
How does one payer’s payment rates compare with others? Is that payer paying below-market rates? If so, use deidentified information (either the average or rate by rate) to build a case for higher rates, Andrew Harding, co-founder and vice president of customer success at Rivet  says.
 
Talk with other physicians in your local area to get a sense of their contracted rates, says Boyd Stewart, vice president of payer/provider collaboration at KLAS Enterprises. KLAS is working with various practice management vendors to obtain blinded data it can use to publish benchmarks for payment rates, denial rates, and more.

There is no doubt that there are some orthopedic surgeons, especially older practitioners who did not train with podiatrists, who might cast aspersions on DPMs doing ankle or rearfoot surgery. By and large, the younger orthopedists who trained together with podiatrists would be less likely to say such disparaging things about our colleagues. That is unless they felt that they were losing money to the DPM "down the block." 
 
Money has always and will always be the source of enmity between the two professions. There are other examples from other medical specialties where enmity and bad mouthing is present. Some colon surgeons will bad mouth gastroenterologists because both professions do colonoscopies and get paid well for those procedures. Neurologists are often at odds with physiatrists because they both bill for neurological testing. The list goes on and on.

Dr. Jacobs’ tales reminded me of when was the first residency trained podiatrist in town. Luckily, the bylaws of the osteopathic hospital were written that allowed my application to be approved. In the first month, I was doing a consult and I was stopped in the hallway by the head of orthopedics and told, “You better not take up the beds of my patients!” I replied I've been in practice one month, I have no patients to put in-house. Luckily, the internists, family practice (they could manage their patients in-house then...no hospitalists) and physical medicine docs were happy to use my services; my practice grew. The allopathic hospital in town was off limits to DOs and DPMs. 
 
After 5 years of doing my best, I could (for my patients) finally apply. They changed the bylaws and allowed DPMs on staff. It may have been due to their family medicine and internal medicine residents rotating with me for the prior two years to learn about foot and ankle pathologies. I received a mug prior to the bylaws changes at the residents’ graduation that said 'THANK YOU FOR TEACHING OUR DOCTORS," but was still not allowed on staff then. I know things are better and improving. Three-year residencies have helped a lot, but we as a profession must continue pushing for parity! We all know that due to costs and human nature, we will eventually have one national system.

Insulin costs are just the tip of the iceberg. Drug prices are out of control. I got involved in brain cancer research and run a brain cancer foundation. For these patients, the co-pay for the first month of chemotherapy if they have Medicare alone is about $1,000. About half of these patients cannot afford it and go without it. This leads to an earlier death and a much smaller chance at long-term survival.
 
When my dad was diagnosed with a malignant brain tumor and he was told his cost for the chemo was going to be about $5,000 a month (this is before Medicare started paying for it), he refused it. When I found out he refused it, I bought it for him but it was too late. He died just 3 months later. Statistically, the average glioblastoma patient in the USA lives 8-9 months. However, if you only count patients who get access to treatments, the average is...
