The Texas Health Education Application Service (TMDSAS) has added the UT Health Rio Grande Valley School of Podiatric Medicine (UTRGV) to the state's central application service for all public medical, dental, and veterinary schools, creating parity in the access for applicants to those graduate medical professional schools.

TMDSAS simplifies the application process for both the applicants and the participating schools. It provides one standardized application for first-year entering classes.

Dyane E. Tower, DPM, MPH, MS has been promoted to Senior Medical Director and Director of Clinical Affairs at the American Podiatric Medical Association (APMA). Dr. Tower earned her podiatric medical degree at the William M. Scholl College of Podiatric Medicine, a Master of Science degree at the Rosalind Franklin University of Medicine and Science, and a Master of Public Health degree at Dartmouth College.

NY Podiatrist Receives the NYSPMA Lifetime Achievement Award 
 
Michael DellaCorte, DPM received the New York State Podiatric Medical Association (NYSPMA) Lifetime Achievement award both virtually at the NYSPMA House of Delegates and also in person on June, 23, 2022. The in-person award was presented by Drs. Corrine Renne Landau, Carlos Silva, and Arthur Gudeon. In attendance were many close friends and colleagues and his wife Denise.

Dr. DellaCorte has been very heavily involved in podiatry and the NYSPMA for many decades. He served as the residency director for several programs. He was very involved with the American Board of Podiatric Medicine and has been on their board for many years. He was instrumental in helping increase the scope of practice in New York and has guided and helped many podiatrists over the years, teaching them to have pride in themselves, their work, and their profession.

Healing of Leg Fracture Earliest Sign of Civilization: Mead
 
A student once asked the anthropologist Margaret Mead what she considered to be the earliest sign of civilization in a culture." The student would expect the anthropologist to talk about hooks, clay bowls, or sharpening stones, but nope. Mead is reported to have said the first sign of civilization in an ancient culture is evidence of a person with a broken and healed femur. Mead explained that in the rest of the animal kingdom, if you break your leg, you die. You cannot run from danger, go to the river to drink water, or hunt to feed yourself. You become fresh meat to predators. No animal survives a broken leg long enough for the bone to heal.

A broken femur that healed is proof that someone took the time to stay with the one who fell, healed the wound, brought the person to safety, and cared for them until they recovered. "Helping somebody going through difficulties is the starting point of civilization," Mead explained. Civilization is a community help. Whether se actually said it is open to debate, but the point is clear.

Query: Complicated Toenail Surgery
 
I have a rheumatoid patient with a grossly deformed interphalangeal joint with significant osseous malformations leading to a chronic ingrown toenail on the hallux fibular border. I have ruled out any infection, osteomyelitis, or neoplasm. The bone is grossly hypertrophic and needs excision along with the nail structures on the fibular border. There also may need to be some plastic closure performed. What are the appropriate ICD10 and CPT codes reflective of this scenario?

Response: The example you provided, the ICD-10-CM codes to use include either one of these:
1) M89.371 Hypertrophy of bone, right ankle and foot
2) M89.372 Hypertrophy of bone, left ankle and foot
 
The CPT code for removing the bone of the distal phalanx is:
1) CPT 28124-Partial excision (craterization, saucerization, sequestrectomy, or diaphysectomy) bone (eg, osteomyelitis or bossing); phalanx of toe
2) CPT 28153-Resection, condyle(s), distal end of phalanx, each toe
3) CPT 28160-Hemiphalangectomy or interphalangeal joint excision, toe, proximal end of phalanx, each
 
The odds are you will use CPT 28124, but it depends on how much and where anatomically you remove the phalanx bone. As to the closure and the the nail... these are bundled; I feel it would be considered as secondary and would not be additionally coded.

RE: Insulin Costs Outpacing Financial Resources for 4 in 5 Patients

80% of diabetics or caretakers for someone with DM face credit card debt averaging $9,000 due solely to the cost of insulin. More than 60% have decreased the usage of insulin because of the cost, increasing multiple risk factors. The results are from the survey by CharityRx. The insurance industry, drug industry, and insurance regulators must look at this looming problem carefully. The risk involved with these patients is significant, potentially increasing morbidity tremendously. Secondarily, the resultant medical consequences can increase costs to the healthcare system.
 
This is just one of many aspects facing the elderly and anyone on a fixed income with any medical condition. The current rate of inflation, increased cost for food products, and decrease in the efficiency of the supply chain continue to make it more difficult for geriatric patients. Physicians are not given enough credit for the vast majority who care about their patients and independent of reimbursement concerns, put a hand out to help someone get the medical attention they need. RE: Enmity Between Podiatrists and Orthopedists

At one point, I was doing cases at a surgicenter in Corpus Christi. For various reasons, I started working at the hospital OR and no longer brought cases to that surgicenter. I attended a meeting in front of the directors at the surgicenter to vouch for another podiatrist in town who was hoping to get rearfoot privileges at the facility. After looking at my surgical logs from residency and private practice and seeing that a grand majority of it was ankle and Achilles work, an orthopod on the director’s committee stated, to my face, “I don’t care how many ankles you’ve done in your residency or private practice, NO podiatrist is qualified to do an ankle fracture.”
 
As a fair amount of this guy’s efforts at the foot and ankle would end up in my office for one reason or another, I thought it amazing that he was willing to throw such a stone (when he obviously lives in the proverbial “glass house”). That was, however, the enmity which existed between podiatry and orthopedics in the city of Corpus Christi.
