NC Podiatrist Adds Caveats to "Benefits of Calluses" Study
Before you take a pumice stone to your foot calluses just because they're unsightly, you might want to consider the idea that they are actually nature's shoes. That's one of the messages from a new study suggesting that in certain ways, walking on callused feet can be better for you than the modern luxury of cushioned shoes. Researchers found that calluses offer the foot protection while you're walking around, without compromising tactile sensitivity -- or the ability to feel the ground. That's in contrast to cushioned shoes, which provide a thick layer of protection, but do interfere with the sense of connection to the ground.

That comes with some big caveats, though: People with certain medical conditions, such as diabetes, should neither go barefoot nor let calluses build up, said Jane Andersen, DPM. She's a podiatrist and chair of the communications committee for the American Podiatric Medical Association. People with nerve damage or poor blood circulation to the feet -- from diabetes or other medical conditions -- should see a foot doctor regularly and, if needed, have calluses trimmed, Andersen said. Calluses can lead to ulcers in those cases. People with nerve-damaged feet also need to wear shoes, she said. That reduced sensation means they may not notice any cuts or other injuries they'd get while walking barefoot. Beyond that, Andersen noted, barefoot humans of the past were not running around on hot asphalt and other modern surfaces.

CA Podiatrist Is a Big Fan of Wearing Socks
John Chisholm, DPM is a podiatrist in Chula Vista, California, and president of the California Podiatric Medical Association. Chisholm, like most foot doctors, is a big fan of socks. He says that socks serve two important functions: they reduce friction between the shoe and the skin, which prevents blisters and abrasions; and they provide a physical barrier between the foot and the microbial petri dish that is a sweaty shoe.

"If you were to take a scraping off a well-worn leather shoe, you'd find it's a zoo of micro-organisms that can cause disease in the human foot," says Chisholm, noting that the number one threat is the athlete's foot fungus. "It likes places that are dark, warm, and don't have a lot of air circulation, like the inside of a shoe." Chisholm notes that the athlete's foot fungus is not only the cause of the classic raw, scaly patches between toes, but also most toenail fungal outbreaks.

Wickenburg Community Hospital Welcomes UT Podiatrist
Wickenburg Community Hospital (WCH) recently welcomed Tanner Moore, DPM, podiatric surgeon, who is scheduled to start practicing at the WCH clinics in July. Moore specializes in all aspects of foot and ankle care, including surgical reconstruction, diabetic management, sports medicine, and wound care.

SCPMA/Podiatry Institute Hold 2019 Annual Conference
The South Carolina Podiatric Medical Association (SCPMA) and Podiatry Institute held its annual scientific conference in Hilton Head, SC on June 20-23 at the Hilton Head Marriott Resort and Spa. On the heels of the recently passed legislation endorsing podiatrists to perform ankle surgery, along with the leadership of the SCPMA President William Harris IV, DPM and Educational Conference Chairman Michelle Butterworth, DPM, the conference achieved record setting numbers.

The "Footprints in the Sand Conference: Challenging Cases and Current Treatment Options in Podiatric Medicine and Surgery" reached a record setting attendance of 221 attendees from 16 states, topping the previous 1995 attendance of 182 attendees. The educational enlightenment was complemented by the 6th Annual Golf Tournament and a dinner cruise on the Spirit of Harbour Town around the island. Dr. Harris stated, "This was by far the best conference that our association has ever hosted. The interactive discussion panels with the distinguished faculty and the hands-on workshops made it a one of a kind experience!"

ON THE INTERNATIONAL LECTURE CIRCUIT |
NJ Podiatrists Lecture at Serbian Medical Conference
Leo Davidson, DPM and Simon Becker, DPM traveled to Belgrade Serbia last week to participate in their 10th annual Serbian Diaspora Medical Conference "Medicine Worldwide and in Serbia." This was by a personal invitation of The Royal Highness Crown Prince Alexander and Crown Princess Katherine of Serbia who sponsor this conference.

During the 3 days in Belgrade, Drs. Becker and Davidson gave the attending doctors a glimpse into the advances in podiatric medicine and surgery in the U.S. Dr. Davidson lectured on LisFranc fractures, while Dr. Becker lectured on plantar plate tears. Their lectures were attended by Serbian orthopedic surgeons, pain management specialists and physical therapists.

6 Practice Expansion Considerations - Part 6
Physicians often dream of expanding their practice footprint. Whether such expansion means multiple locations in the same state or locations across the country, there are steps you can take to grow your practice while also be compliant with state and federal laws. Here are six things to consider before setting up shop elsewhere.
6. Tame your ambitions. The idea of practice expansion is exciting, but don’t be in such a hurry to expand your practice. A solid practice with good expansion potential will only get stronger as time goes on. Doctors often want to sign a contract right away or they have a partner who wants to do a deal, and they push the expansion forward without proper legal review. When it comes to healthcare, it’s worthwhile to take your time, do things right, and avoid mistakes. It can be costly to miss an important healthcare law in your expansion efforts, causing undue hard work and hurting your practice’s overall reputation.

Query: Affected by the Post-op Global Period of Another Doctor
I was consulted to see a hospital in-patient who was within the 90-day global period for lower extremity surgery performed by another podiatrist that I am not affiliated with. Medicare denied the claims for my in-patient visits due to “Separately billed services/tests have been bundled as they are considered components of that same procedure. Separate payment is not allowed.” Is there a modifier that I should append to the claims?

Response: The global period is tied to the provider NPI that performed the surgery. You should not be caught up in that. The fact that you are in the same profession does not apply either. I would call your carrier directly and see what is going on. For you to bill the -55 modifier would require that the surgeon had billed the -54 modifier. The other modifiers do not apply as those are tied to returning to the OR by the surgeon or unrelated services by the operating surgeon. Something is not correct. If this is somehow a new policy, your state Medicare rep needs to be alerted.

Dr. Bob graduated from the Temple University School of Podiatric Medicine. He first practiced podiatry in Roanoke, Virginia, but left after 5 years to set up a practice in Indianapolis.

It’s no wonder that there is so much confusion about cold sterilization vs. autoclave sterilization, especially when the EPA and CDC are at extreme odds about what liquid agents effectively kill viruses, fungi, or bacteria pertaining to basic instrument disinfection via cold sterilization. The CDC identifies major conflicts in what is approved by the EPA as “registered or approved” hospital grade QAC disinfectants vs. true liquid sterilants vs. intermediate grade liquids QAC disinfectants to the point no one should consider cold sterilization for any related clinical practice for instrument disinfection.
The EPA quit verifying their own “registered or approved” liquid disinfection agents in 1982 due to funding limitations. Anything EPA “approved or registered” for instrument disinfection is...
