PODIATRISTS IN THE NEWS |
NY Podiatrist Discusses Prevention of Cracked Heels
Our heels are a bit like tomatoes. Put too much pressure on them and bam, they expand sideways and split. Heels are more likely to develop cracks if we spend a lot of time standing, wear backless shoes that don’t support the heel, or let the skin get very dry and callused. “If you have too much friction or sliding on skin that is thick, eventually the layers will split,” says Jacqueline Sutera, DPM, a New York City podiatrist. “The problem gets worse as we get older because there’s loss of the fat pad on the bottom of the foot, and the body replaces it with calluses.”
|
Dr. Jacqueline Sutera |
Ultimately, the best way to avoid cracked heels is to prevent them, and that means weekly upkeep. “We have to tweeze our eyebrows and shave our legs, so pumicing and moisturizing is just one more thing to add to the routine,” Sutera says. If your cracks are very deep, bleeding, or seem infected—you’ll see redness and swelling; they might feel warm, and it will probably hurt. See a podiatrist.

APMA IN THE NEWS |
APMA Can Help You Find the Best Shoes for Your Feet
Looking for a pair of comfortable shoes? it can be a dizzying experience. To help identify styles that promote foot health and wellness, the American Podiatric Medical Association, a leading resource for foot and ankle health information, is one place to start. The non-profit medical association offers consumers guidance through its Seal of Acceptance program, created to inform both podiatric physicians and consumers about products whose quality, safety, and effectiveness allow for normal foot function and to promote good foot health.
|
Kathy Balderson |
Shoe brands interested in obtaining the seal must go through a formal review process that includes a written submission to its member podiatrists on issues including product composition, evidence of quality control, and research documentation. Companies must also provide wear-test samples to a committee of 10 podiatrists for their first-hand evaluation of the product, according to Kathy Balderson, APMA’s director of programs and continuing education. Acceptance allows brands to use the seal in their marketing materials. “It’s not an endorsement but a recognition that a product promotes good foot health,” said Balderson about the strict guidelines.

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Guaranteed Ways to Get Sued - Part 4
4. Being a jerk - Perhaps the number one way physicians can guarantee they’ll be served with a lawsuit is extraordinarily simple: Be a jerk. Michael Mustokoff, a Philadelphia-based partner at law firm Duane Morris, says patients have expectations for how providers behave based on their previous experiences and what they see on television. These idealistic personages can include Marcus Welby, Trapper John, and Greg House.
What patients don’t expect is to be treated unkindly because their physicians are stressed by the rigors of running a business, or because they must sacrifice time providing care to deal with non-clinical issues, such as reimbursements and licensure headaches. “When the most careful, astute, and skilled physician makes a mistake—and they all do, no one is infallible—how the patient regards the effort is every bit as important as their result,” Mustokoff says. If the physician is a jerk in those circumstances, the mistake is amplified. Patients who feel wronged may see a lawsuit as a fitting redress.

RESPONSES/COMMENTS (OBITUARIES) - PART 1 |
RE: The Passing of Lawrence G. Lefler, DPM
It is with sadness that I inform you that I received a call a short time ago from Dr. Lawrence G. Lefler's daughter informing me that he passed away Saturday. Larry was President of APMA many years ago and then was Speaker of the House of Delegates. He was a true gentleman and gave so much so that our profession could grow and prosper.
|
Dr. Lawrence Lefler |
We became friends in 1959. We spoke and communicated weekly for all those years. I am sure that the AMA files contain a detailed record of his activities for APMA. He demonstrated great integrity and was a mensch. He helped organize the first federally funded training program on aging at the University of Nebraska with the late John Carson. Rest in Peace, my dear friend.

RESPONSES/COMMENTS (OBITUARIES) - PART 2 |
RE: The Passing of Gerald Kuwada, DPM
It is indeed a profound sadness to read about the passing of Gerry. I knew him well as a student at CCPM in San Francisco (class of 1976). I was a year ahead in the class of 1975. We were part of a very small core of diverse students of that era at CCPM. We had a bond of friendship that didn’t require constant contact or communication. The first time he met my two children, he made a lasting impression on them with his sincere kindness. As colleagues, we served together as members on the CPME; team members of residency site evaluations; and most importantly as part of the test construction team for the ABPOPPM (AKA ABPM). Gerry was a guy who loved life. I will miss his infectious laughter, kindness, and our life-long friendship. RIP Gerry.

CODINGLINE CORNER |
Query: CPT 17110 billing
Do we need Lt or Rt modifier with CPT 17110? What modifier should we use if a patient has bilateral plantar warts?
Dipan Patel, DPM, Lambertville, NJ
Response: The code CPT 17110 is based on the total number of lesions “up to14 lesions”, regardless of where these lesions are located. Obviously, the same hold true for CPT 17111 “15 or more lesions.”
No anatomic location modifier(s) is/are required for either of these codes.

RESPONSES/COMMENTS (CLINICAL) |
From: Ira Baum, DPM, Cindy Resnick, DPM
I refer you to the dissection technique of Luke D Cicchinelli, DPM: Another Approach to the Posterior Heel Surgery. Podiatry Update 2002. His approach, if done meticulously, many times spares the majority of the AT insertion. It may give you the option of simply reinforcing the AT insertion, if necessary.
Ira Baum, DPM Miami, FL
I have successfully used plantar fascial night splints for the treatment of chronic Achilles tendonitis. It effectively stretches the Achilles and its usage often precludes the necessity for surgical intervention.
Cindy Resnick, DPM, Staten Island, NY
