NYCPM Instructor Publishes Article About Gait Analysis in Forensic Magazine
 
Bryan B. Kagan, DPM, Adjunct Instructor in the Department of Community Medicine at the New York College of Podiatric Medicine (NYCPM), has co-authored an article “Considerations of Gait at Crime Scenes” that has been published in the current (June/July) issue of Forensic Magazine, and also appears on the magazine’s website.

Forensic gait analysis is the identification of gait features and patterns of subjects seen on closed circuit television (CCTV) and from footprints forming a gait pattern left at the scene of a crime. Dr. Kagan teaches forensic podiatry at NYCPM, which is the first school in the nation to offer an introductory course in the new discipline of forensic podiatry.

Calluses Can Alter a Runner's Gait and Lead to Injury: IL Podiatrist

Think of those tough spots as badges of honor! "Callus formation is a natural response to friction and pressure," says podiatrist Dr. Megan Leahy of the Illinois Bone and Joint Institute. "Because runners obviously take more steps than non-runners, calluses may occur more readily."

Calluses can be harmless, and a thin layer can even offer some protection against blisters, the elements, and typical wear and tear, says Leahy—but if they become too thick or develop a deeper core (known as nucleation), they can become quite bothersome. "If the calluses are painful, this may alter your mechanics and lead to injury."

PODOA Back From Second Mission from Africa
 
Podiatry Overseas Association’s (PODOA) second mission trip to Togo, Africa was another resounding success. PODOA continued their partnership with K’palime Hospital. In addition to donating medical supplies to the community hospital, PODOA’s mission team performed a variety of surgical procedures over a 7-day span with Dr. Ayouba, the Chief of Surgery in K’palime, Togo.
 
Dr. Mario Cala educates Togolese physicians on the concepts and theories of external fixation.
 
Podiatry Overseas also held a 2-day lecture and workshop on external fixation for doctors from the University Hospital of Lomé. The lecture & workshop were both led by the Chief Medical Officer of Podiatry Overseas, Mario Cala, DPM. PODOA is in the early stages of planning their return to Togo for the next medical mission trip in December 2014.

QUERIES (CLINICAL)
Query: Removal of Excessively Large Accessory Navicular
 
A 13 year old female presented with a chief complaint of a painful, protruding accessory navicular on the right foot. It became painful after a minor sprain. Conservative treatment using a CAM walker boot for 4 months has reduced pain from a 10 to an annoying 3. However, any long-term pressure from shoes causes pain. The MRI indicates that virtually all of the posterior tibial tendon is connected to the accessory bone. The patient and parents want the protrusion and/or the accessory itself removed. 
 
Excessively Large Accessory Navicular
 
Due to the size of the accessory, and the MRI report, my fear is that if I completely remove the accessory bone, the entire tendon will be detached, and she is going to have a 1 cm gap between the navicular base and tendon. I am considering just shaving off the part that is visibly protruding from the medial side of the foot rather than removing the entire body. Any suggestions for tendon re-attachment devices for this situation or other experiences would be appreciated.

Query: Returning Eligible Professionals (EPs) to the EHR Incentive Program 
 
"If you successfully met Meaningful Use in 2013, you will be exempt from the payment adjustment and do not need to submit a Hardship Exception Application for Payment Year 2015."
  
The above statement is copied and pasted from the CMS website. Does this mean if we don't meet Meaningful Use Stage 2 for 2014, then we will not be penalized, even without a Hardship Exemption? And would I assume there is no incentive money? And will we still be expected to meet Stage 2 requirements for the whole year in 2015?

In regard to the surveys and being paid for them being a scam, it appears that there is another potential "scam" out there. I was approached to participate in a clinical study for a pain cream. I initially agreed, but when I got the paperwork, I found that the patients were given a $50 "gift card" for participating, but their insurance would be filed for the medication. It also said that they would not have to pay more than $25 out-of-pocket. So each time a prescription was filled, the "investigating" company would be paid $1,000 or more and the Dr. $100. Great way to make a living. I think this proposition was sent all over the country, so beware.

Anti-kickback laws at both the federal and state level make it illegal for a prescriber to benefit financially from referring a patient to a specific pharmacy or other medical service. The federal laws and many states include specific provisions to create a “Safe Harbor” exemption to the anti-kickback laws. Physician Specialty Compounding, LLC (PSC) has worked with a number of law firms to document the steps for creating the “Safe Harbor” required for a prescriber-owned pharmacy.  After the signing of a mutual Non-Disclosure Agreement, PSC is happy to share our lawyers’ legal analysis.  PSC strongly recommends that you consult with your attorney to protect your interests and to verify full compliance with all federal and state laws.

The legalities concerning compounding pharmacies and the ways for them to "distribute" money to the prescribing physician are multiple and have been discussed here before. In my opinion, gray area or not, they do not pass the "stink test." Dr. Kass summed it up by saying the "incentive dollars (in any form) are nothing more than a kickback/scam." If that isn't clear enough, and you want to proceed, first read about today's indictment in L.A.

In my experienced opinion, spanning 45 years of podiatric surgical practice, including multiple board certifications, there is no question that cosmetic foot surgery is well within the international standards of surgical care! 
 
I'm appalled that my colleagues will even debate the efficacy of plastic and reconstructive foot surgery when thousands of surgeons worldwide perform these procedures routinely to help their patients.

It seems to me that a podiatrist would want to keep the credibility of statements attributed to him as accurate as possible as we all try to strive for maintaining our hard-earned equality and professional position. His quote may have been taken out of context,  “In the past, we’ve had to use surgery to release the tendon in the toe and sometimes actually remove pieces of bone. The Toegrip® implant is a much more conservative way to relieve the pain, which also means a faster recovery."
 
From the company's surgical technique instructions: "The proximal and middle phalanx articular surfaces are resected."
