High Heels Create Potential Health Issues for Young Girls: CA Podiatrist

What do little girls need? To be taller. How do we make that happen? Put them in high heels. Michael Kors now makes 3-inch heels for little girls and has tons of mommy bloggers up in arms about it. Apparently, we can blame Suri Cruise for the trend. She was spotted wearing gold sandals with heels several years ago and don't all little girls want to look like Suri?

Dr. Joshua Kaye, Podiatrist, says this new trend is a really bad idea. “When it comes to young girls wearing heels -- they have not yet reached skeletal stability at the age of six and if they chose to wear heels, they increase their risk of fractures and ankles sprains, which in turn could lead to other potential health issues,” says Kaye.

Barefoot and Minimalist Shoe Runners Need to Start Slowly: UT Podiatrist

Barefoot running is, for many, not technically done without shoes. Rather, runners slip on shoes that look like gloves for feet, with little tubed slots for each toe. A 2010 Harvard Study and the book Born to Run both claimed that barefoot, or minimalist, running decreases pounding on the body and can help runners increase the length of their runs. Dr. Brandt Gibson, a podiatrist and runner in American Fork, however, is cautioning people to take it slowly.

"It can take up to six months to a year to run fully in those shoes," Gibson said. "You are increasing your chances of injury if you are wearing a barefoot running shoe and you don't work into it." He says to first try the shoes for five minutes during a typical run, a couple of runs a week, and then increase the time 10 percent at a time, eventually working up to the full length of the run. Gibson says that the caution is, in part, because many runners will end up completely changing the way they run, essentially retraining. If a runner is a natural heel-strike runner, the method will switch the runner to a mid-foot or fore-foot strike.

CA Podiatrist's Company Signs Distribution Agreement for Sanitizing Products

Rehabmart.com, an online e-commerce company that sells rehabilitation and medical supplies, has joined into a distribution agreement with Woodward Laboratories to offer their patented Handclens hand sanitizing and cleansing products to a wider consumer market. Founded in 1992 by Kenneth Gerenraich, DPM, with his original formulation to treat toe and fingernail fungus, and soon joined by Dr. Daniel Chodosh, a scientist and pioneer in transdermal delivery systems, the company gained immediate success and professional endorsement with its initial product, Myocide NS.

Fake San Jose Podiatrist Convicted of 19 Felonies

A San Jose man who owned a chain of laser treatment centers for toenail fungus turned out to be footloose about actually being a doctor and was convicted Friday of 19 felony counts of practicing medicine without a license. Cary Silberman, 53, ran spas in San Jose, San Francisco, San Ramon, Beverly Hills, and Manhattan called Shiny Toes, an operation that was tarnished last year when Silberman was arrested after an investigation by the Medical Board of California.

Lisa Schon, Santa Clara County prosecutor, said he was fraudulently presenting himself as a licensed doctor, diagnosing patients with toenail fungus, and treating the condition with lasers. Silberman faces up to 15 years in prison at his sentencing next month.

Most Medicare Docs Used EHR in 2011

A majority of physicians participating in Medicare used an electronic health-record system last year, according to a recent government survey. HHS' Office of the Inspector General randomly sampled 2,000 of Medicare's physicians in 2011 and found that 57% of them were using an EHR at their primary practice location.

The figure mirrors that from a 2011 annual report by the Centers for Disease Control and Prevention, which found that 57% of all office-based physicians were using an EHR system at their primary practice location. In contrast, the Medicare Payment Advisory Commission reported in April that only 25% of independent physicians had registered with the federal EHR incentive program, and 6% had qualified for payments.

Query: Another Set of Foot Orthotics

We have a patient who was last seen 2 years ago, and got custom foot orthotics that are working out quite well for him. He just realized that his policy will renew on July 1st and wants to get new orthotics before then. Does he have to come in for evaluation before getting new ones?

The patient does not want to come in for an appointment prior to getting his new ones. He just wants them reordered, and then he will come in for dispensing. He has BlueCross BlueShield of Maryland. Please advise.

Response: Two years is a long time. No self-respecting optometrist would dispense a new pair of glasses without first examining the patient.
While the patient may believe he's functioning well, what happens if you have a new device fabricated from the old digital image, his foot has changed, and the patient sustains an injury from the new device?

Whether you choose to charge the patient for that visit is one issue. I would never dispense a new device to anyone I have not seen in two years without first doing an examination.

Having dealt with a similar problem, before you decompress the joint any further, look at the sesamoids and make sure that you have enough "room" for further decompression. You don't want to place the distal aspect of the sesamoids within the joint space. Also, consider replacing the implant with a smaller size. The one that you inserted initially may have been too big.

RE: NY Assembly Passes Expansion of Scope of Practice Law

New York has a new scope of practice bill. The leadership has stated in an e-mail to the members that they were offered a bill by the Assembly leader with advisement to accept it now and have it pass now. The NYSPMA leadership then went on and explained that they were assured by the bill's sponsors that it could eventually be "tweaked" pending discussions with stakeholders.

The good news is that some level of DPM training and certification will be allowed to take advantage of the new scope, which for the most part will include the coveted osseous ankle. Those meeting the requirements for osseous ankle surgery will also be the ONLY ones who can biopsy a skin lesion or scrape a wart off the distal leg. It is this insanity that leads members to...

I remember the 1982 emergency meeting like it was yesterday. I was a young non-residency trained podiatrist in Santa Monica, CA who had just bought an old C&C practice that was heavily tilted to Medicaid. There was a Southern California meeting and a Northern California meeting for all 2,500 California DPMs. The gist was that our future was in balance and, if each podiatrist did not immediately contribute $1000 to the PAC, Medicaid in California would go down the drain.

As it turned out, only the recent graduates and the old timers heeded the call to action with their checkbooks, and the PAC used the funds to pass legislation that only impacted 20 podiatrists in the state. I came out on the short end of the stick. Some things never change with the passage of time. Some people call that politics.

Biomechanics is alive because of the life of Dick Bogdan. Dr. Richard Bogdan had a deep love for the study and understanding of the mechanics of gait. His acumen in this field was so broad that he would often make our heads spin in three different planes of motion.
 
He was an early pioneer in the field of simple machine mechanics by being the only podiatrist to write a biomechanical review for the spring orthotic device currently being developed for the U.S. Department of Defense and Army's Medical Research and Materials Command.
 
He will be missed by his former students, residents, and freinds of whom I am blessed to be one of each. "A Hui Hou" Richard.
