Gradually Increasing Activity Lessens the Chance of Overuse Injuries: VA Podiatrist
 
According to Katie Evans, DPM, "A long hike in the spring or early summer after a fairly sedentary season can easily lead to foot pain. Increasing walking or jogging distance greater than 50 percent of your norm can lead to foot pain. The general rule to follow for increasing an activity such as jogging is no more than 10 percent per week. Ten percent doesn’t seem like much but it will keep us out of trouble."

"We’ve seen so many people worsen a particular foot condition by continuing a workout routine in spite of foot pain. Continuing a jogging routine or exercise class while experiencing Achilles tendinitis or plantar fasciitis can make the condition worse. At the least, the chance of improving isn’t great while being this active. The same applies to your entire lower extremity. If something starts to hurt, back off for at least a week to see if it passes," says Dr. Evans.

PRACTICE MANAGEMENT TIP OF THE DAY
Common Pitfalls in a Doctor's First Year in Practice
 
Congratulations for making it through medical school and residency, and even perhaps a fellowship, and now you are ready for your first or second job out of school. Here are some areas that trip up new doctors the most.
 
1. Contracts - Negotiate them, find out what you are worth, and know how to slow things down and advocate for yourself well. Understand the expectations. Is the compensation and bonus structure easy to understand? Are you supposed to be on call nights and weekends? See patients 34 hours per week in the office? Have certain productivity expectations? Do you have sufficient paid time off (PTO) and continuing medical education (CME) reimbursement? Fully understand what’s expected and see if it’s reasonable or something you need to negotiate.
 
And find a back door OUT. This will likely not be the job for your entire life, so find a way that you can exit this agreement with a certain notice period – like 90 days, without any strings attached. Then walk through in your mind what would happen if you leave. Is there a tail policy you have to pay? A penalty? A non-compete clause? Think through all these before you sign.

Query: Treating A Ganglion Cyst
 
A new patient comes in to the clinic with a right foot pain and soft tissue mass. I felt a soft tissue mass not unlike a ganglion cyst. An MRI had been performed and it was not conclusive as to the nature of the mass. We had a long conversation about options, including excisional biopsy. The patient elected to have the mass aspirated which I performed in the clinic at the same visit. It produced a gel-like substance characteristic of a ganglion cyst. I would like to bill an E/M code and CPT 20612 for the procedure. Any thoughts on billing both for the procedure and the E/M in this scenario?

Response: A patient presents either as a new patient or with a new problem. You evaluated the patient/problem and discussed etiology of the problem and treatment options and devised a treatment plan. You evaluated and managed the problem-hence an E/M is warranted. The level of service is based on your documentation (problems reviewed, data reviewed, and medical complexity), not just on the diagnosis itself.
 
One argument that sometimes insurance companies put forth is that on some referrals, the diagnosis has already been made and hence no E/M is allowed when a procedure is also performed on the same day. That may be true in some instances, but in your case, the diagnosis is not clear even with an MRI. Make sure you document that as well. I would bill CPT 9920X-25 which allows you to be paid for both the E/M and the procedure CPT 20612.

I would like to relate my personal, anecdotal experience with "therapeutic injections of dextrose" aka prolotherapy. A few years ago, I developed debilitating shoulder pain and was told by an orthopedist that this would require major surgery to obtain any relief, and the sooner the better. I heard about a physiatrist in my area who specializes in prolotherapy. Two injections to my shoulder and the pain was 100% resolved. 
 
We could only wish to have a practice like that! The doctor had one employee, low overhead, and didn't accept any insurance, i.e. no insurance paperwork. As I recall, it was about $375 per injection. When I went into the clinic, there was always someone at the front desk checking out with their credit card and as I was giving my credit card after the injections, there was another patient being brought back for their injection. I have referred many patients to the physiatrist for various problems, who thanked me afterwards, as he was able to help them too.

Sorry to hear the sad news of the passing of Dr. Steve Levitz. My condolences to his family and friends. He was a humble, genuine person. I was a student of his at NYCPM in the mid ‘80s. He was a good attending to vent to and you always walked away with a smile. He had a practical approach to biomechanics that I found helpful.
 
On his own time, he evaluated a few classmates and me. He taught us how to make a foot orthosis in the College lab. My devices helped me complete the ‘85 NYC Marathon and I used them for 10 years or so  I saw him about 20 years ago and reminded him about those orthotics. He smiled and nodded his head. Rest in peace, Steve.
