NY Podiatrist Describes How Walking Barefoot Alters Your Biomechanics
"Walking barefoot on hard surfaces allows our foot to collapse, which can lead to a tremendous amount of stress not only to the foot, but to the rest of the body," explains Miguel Cunha, DPM, a leading podiatrist in Manhattan and founder of Gotham Footcare. This can affect your gait (your stride when you walk or run). Dr. Cunha explains that we naturally pronate (move our feet inward to absorb shock and distribute weight) during the gait cycle.

But, "when we walk barefoot, we pronate for a longer period of time, which then alters the biomechanics and distribution of pressure and weight across the foot," Dr. Cunha says. "This imbalance may increase the progression of underlying foot deformities such as bunions and hammertoes and lead to painful conditions associated with excessive pronation such as arch and heel pain, shin splints, posterior tibial tendonitis, and Achilles tendonitis." In extreme cases, this can "translate upward, affecting other parts of the body such as our knees and back."

Wear Patterns on Shoes are Indicator of Foot Health: NC Podiatrist
If you suffer from sore calves, or unexplained pain in your feet, knees, or lower back, go grab a pair of your older shoes and flip them over. “Looking at the patient is definitive, but you can make a thumbnail sketch by looking at that,” said Richard Lind, DPM, a board-certified podiatric surgeon with Carmel Foot Specialists in Charlotte. Dr. Lind said when it comes to feet, everybody has a type. Looking at your shoe, if the outside edge is worn, you likely have a high arch and put all of the pressure on the outside of your foot.

“That high-arch foot is a much more rigid foot so they are a very hard heel striker and forefoot planter, so they’re going to be almost bowlegged, and they’re going to have some lateral knee strain,” Dr. Lind said. If your shoe is worn down on the inside edge, doctors say that’s because your arch collapses during walking, forcing your ankle to roll inwards.

5 Guaranteed Ways to Get Sued - Part 3
3. Lackluster patient communication - Patients want to know what’s going on with their health, but physicians make communication mistakes that muddy the waters. Rather than being clear and consistent, doctors tend to use medical jargon or offer confusing descriptions of health conditions and the planned treatment strategies. “Many patients, particularly those with less education or lower economic classes, are too embarrassed to ask what the doctor means or what they’re talking about,” says Samuel N. Klewans, JD, a partner at Grad, Logan and Klewans, a law firm in Falls Church, Va. He has experienced this lack of translation himself. “It has happened to me with doctors, and I said, ‘Tell me in English, something I understand.’” If a physician seems to be speaking a different language, patients may not even know what questions to ask.
Patients who feel their provider has been thorough, that everything was explained to them along the way, are less likely to pursue a claim if there’s an unexpected or unwanted outcome. “But if the physician is not communicative, he’s going to get sued,” Klewans says. “As a caregiver, it’s the doctor’s job to help patients feel comfortable.” Patients’ families are also a frequent part of the equation, something physicians don’t always consider. When patients and their loved ones don’t have a good understanding of the care and planned procedures, a lawsuit could be on the horizon.

I understand that the query was asking for surgical advice, but I would like to offer a few conservative care treatment suggestions that I used successfully in my practice that may not have been tried for symptomatic posterior insertional retro-calcaneal exostosis:
1. D/C all Achilles tendon stretching or any exercise that heel drops below the level of the forefoot during the symptomatic stage. It has been my experience that PTs often will give patients aggressive stretching programs for almost any type of foot pathology.
2. Start on modified Alfredson exercises. Do not allow the heel drop below the level of the forefoot.
3. Make sure the posterior heel is off-loaded when the patient is watching TV or reading. Alert the patient not to rest the posterior heel on a coffee table or foot stool.
4. Modify a night time splitting so the heel is off-loaded in bed. Most of my patients noticed significant improvement in their heel pain doing the above (sometimes with the help of a NSAID) within 3-4 weeks.

I am surprised that ESWT hasn’t been mentioned as a treatment for Achilles tendinosis with or without posterior calcification. Typically, the pain is caused by degenerative fibers of the insertional Achilles tendon. John Furia, MD published a peer-reviewed article in the American Journal of Sports Medicine titled, "High-Energy Shock Therapy as a Treatment For Insertional Achilles Tendonpathy." ESWT is non-invasive, office-based, with none of the disability and complications typically known with surgical approaches to this difficult clinical problem.

It is with great shock and sadness that I read of Gerald Kuwada, DPM's passing. I had known him for many years as being one of the most "multi-dimensional" people I have ever had the privilege to meet. He had an enormous range of interests and skills, many FAR beyond his profession.
It was always a joy to speak with him because you never really knew what HE knew until you had a chance to delve into his depth. When speaking with him, I found it far better to simply not speak and to listen instead. He had a "centered" outlook and view of life, with a calm voice that was always able to put even the most negative facts into a "wider perspective". We are all poorer for his passing. I express my profound condolences to his family.

RE: The Passing of Gerald Kuwada, DPM
It has been nothing less than a fortunate honor to have known Gerry as my resident in the mid-70s, and as a colleague and friend serving on various committees. Gerry was a warm, energetic, intelligent, competent person who possessed true loving kindness to everyone. I was very lucky to have him as a close friend, sharing podiatric issues as well as social and many enjoyable comical moments. I missed him by not seeing him often for the past few years, Gerry was a true unsung hero as a podiatrist and human being. My sympathy to his family and may G-D protect and bless his soul. As painful as it is to know he is gone, I am able to smile knowing the fun and laughter that he and our close knit friends shared.

The initial results of the survey being conducted on PM News indicate that the majority of podiatrists are not giving tetanus injections. The questions that should be asked is "why not?"
Let me preface this by saying that in our practice, we used to give tetanus vaccinations to patients presenting with puncture wounds along with a history of not having had one in ten years. We stopped doing this when the cost of storing the vaccine, which has a short shelf life, became overwhelming. That being said, I often wonder if we are professionally obligated to bite the stiff economic bullet and make this available for patients with puncture wounds. The window of opportunity for vaccinating a patient who sustains an infected puncture wound and contracting tetanus is very short, and if we miss that opportunity and the patient does indeed develop the disease, are we liable?
What we do now in our practice is that we send the patients to an internist or to urgent care for the shot, but what if those sources are not available on the day the patient presents with the puncture wound? Could I be held responsible for not giving the injection?
