Having More Endovascular Specialists Would Save Limbs: CA Podiatrist

Giving patients options before resorting to amputations has become standard in the past 10 years. Treating peripheral vascular disease, which restricts blood flow to feet and legs, by opening arteries is more common as new tools and techniques have been developed. But it takes trained specialists to do the procedures, and some doctors say that's where Fresno County is lacking.

"We don't have enough endovascular specialists here to open up the lower extremities," said Dr. Jay Mukker, a Fresno podiatrist. "It's a huge, huge problem." Having more of the specialists would save a "tremendous amount of legs and feet," he said.

CA Podiatrist Speaks at UK Biomechanics Conference

An international biomechanics conference was held in Manchester, UK on June 22-23, 2012. The conference included lectures on the latest research on barefoot running, sports injuries, foot orthoses, and running shoe technology; and included practical workshops on in-shoe pressure analysis and gait examination techniques.

Brad Wenstrup: That is something that I would work for change because it just doesn’t track. We’re in with lab technicians. Even the people who are in the medical service corps – the leadership in the medical service corps have said to me, “You guys don’t really belong in here. You should be in the medical corps.” I see the day coming when we will be in the medical corps because the discrimination in pay is unbelievable. What I got paid for my job compared to the other surgeons was ridiculous.

Consider revising the original procedure.  Remove the cap of the implant and decompress the joint 1-2 mm (this is possible with this implant design); then relace the cap. Be sure to release all soft tissue adhesions and re-inspect the sesamoids for mobility.

RE: The Passing of Richard Bogdan, DPM

I was saddened to learn that Richard Bogdan, DPM, a native of Linden, New Jersey, and a friend, colleague, educator, author, researcher, and sports medicine pioneer passed away on April 24, 2012 at age 68 in Perth, Western Australia, after a valiant fight against Mantel Cell Lymphoma. Richard led an illustrious and exciting life as an international podiatric medical educator with a specialty in orthopedics and sports medicine.

He graduated from CCPM in1972 and went on to complete a two-year residency at the California Podiatry Hospital at CCPM, receiving a Masters in Science diploma in 1974. In reference to Dr. Raducanu's statement: “That being said, I'm also a believer that the solo practice is going extinct precisely for the reasons you've stated.” Didn’t Mark Twain say, "The reports of my death are greatly exaggerated"?

Belief in the extinction of solo practice is a little extreme. True, it is very hard to open cold with your debt and lack of available financing. But don't give up if owning your own practice is what you want. The delivery of medical care and medical re-imbursement is changing, absolutely-positively yes; however, a well designed business model of a solo practice will thrive if properly done in most any re-imbursement environment. Not that it will be easy. It is not for everyone. I concur that the majority of podiatrists should practice in a large group.

However, even in managed care or a national “single payer” system, there will STILL be at least two tiers of delivery of care and opportunities for smart and talented well trained docs in solo or small group practices.

The demand for services will ALWAYS be there. Finding a niche, and determining the delivery of that supply of doctors and their care of patients will be the key to very successful small practices of the future. Look for it, it is not an entitlement. Just because you are smart, have worked hard in your training, and earned your degree, doesn't mean success will be handed to you.

Check with your area's compounding pharmacies. We were able to get 2% lidocaine plain for around $8.00/50 ml bottle. We also were able to get dexamethasone and sclerosing injections compounded when they were not available through other suppliers.

RE: NY Assembly Passes Expansion of Scope of Practice Law (Jeffrey Kass, DPM)

I don't understand how Dr. Kass can look at our scope expansion bill and not see it as a tremendous victory for podiatry in New York State. Make no mistake about it, podiatry would slowly die without the expansion. Maybe I am biased because I am one of the "15-20" this will "help", but that is a very short-sighted view and one that thankfully our association did not share. Maybe there are only "15-20" in this state because the best trained podiatrists DO NOT COME HERE? Have you looked at the dwindling license applications?  Have you tried hiring a 3-year trained associate yet to join in your practice? 

Oh and by the way, all residency programs will be 3-year minimum soon. Who will buy your practice when you retire? This scope bill ensures podiatry's survival in this state. Yes, there are still things that need to get done for our profession. No one has lost sight of that. But don't minimize what we have done here and the tremendous impact this will have for EVERY podiatrist in this state.  Kudos to our association and lobbyists for their years of work and dedication on this. Kudos to everyone who has supported this effort.

RE: NY Assembly Passes Expansion of Scope of Practice Law (Jeffrey Kass, DPM)

I understand Dr. Kass' disappointment, but I disagree with his conclusions. No law can satisfy everybody. Board certification should have always been a goal in anticipation of such a landmark accomplishment for our profession in NYS. True, a board certified practitioner is not necessarily better than someone who is not, but a certified practitioner has the necessary diploma of achievement and has stated a quantified accomplishment which can be validated and shown to legislators and societies.

In reality, professional progress must supercede everything and be allowed to proceed. We must understand the circumstances of the leadership and negotiation process. We have finally become medically validated by the Reuter studies, nationally and internationally. Our value is irrefutable. We save limbs and lives.
 
I am certain that there will be learning opportunities provided by NYSPMA, APMA, ASPS, etc. to allow practitioners to comply with the law and acquire new skills. Celebrate this accomplishment and give thanks to the people involved. We "old timers" must give way to the newer generation and provide opportunities for them and us to thrive.  On the whole, old and new practitioners should see more income, and this will allow for future success of our profession in NYS.
