FL Podiatrist Saves Limbs of Patient with Necrotizing Fasciitis

Chester Cocozza has a compromised immune system from heart, vascular and circulatory diseases, which is likely a factor to explain why he developed necrotizing fasciitis, a flesh-eating infection; but he is on the mend after aggressive treatment, said Dr. Philip Organ, director of the Wound Care Center at Physicians Regional Medical Center at Collier Boulevard. "He is probably two or three or four months away from completely healing," Organ said. "He no longer has an infection. His wound is shrinking in size."

Dr. Philip Organ

What really helped Cocozza turn the corner was getting on pain management therapy and getting his other medical issues under control, Organ said. That enabled Organ to take Cocozza to the operating room to remove dead and infected tissue. The surgery was on Christmas Day. "By the time I got him to the operating room, (the infection) was up the side of his calf," Organ said. Besides having the patient undergo daily hyperbaric oxygen therapy, Organ has used negative pressure wound therapy to help drain the wound and frequent debridements, or cleaning, the wound around the edges.

INTERNATIONAL PODIATRISTS IN THE NEWS
Broken Bones Unlikely to Predict Weather: Canadian Podiatrist

Is there any truth to the belief that bones that have been broken can help you predict the weather? "Back in podiatric medical school in San Francisco," says Dr. Lloyd Nesbitt of Toronto, "a local rheumatologist taught us that grandmothers would often be able to tell that it was going to rain as their bunions (enlarged big-toe joints) would be sore." This is because, during the fall in atmospheric pressure that indicates impending rain, the joints can expand.

Dr. Lloyd Nesbitt

However, he doubts that bones that have been broken would react in a similar fashion. "In fact, often when a foot bone has healed from a break ... it heals in a thickened or stronger state than it was originally. Unless the fracture involved an adjacent joint, then a broken bone would be unlikely to enable you to predict the weather."

No Research for Improved Performance or Less Injuries From Minimalist Shoes: MN Podiatrist

“There is no research to show that you are less likely to get injured or that your performance will improve (from wearing minimalist shoes),” said Dr. Paul Langer, a podiatrist at Twin Cities Orthopedics and an avid runner. “But when you run barefoot, or in minimalist shoes, you do change your movement pattern. For some people that can be beneficial.”  For example, the knee might not work as hard, but the feet and ankles work harder. Langer has seen stress fractures in the feet or Achilles’ tendon injuries.

Dr. Paul Langer

Langer, who uses minimalist shoes to augment his own training, advises people to transition into the style slowly. “I look at it as a training tool,” he said. “Some people can incorporate it into their training and it can rejuvenate their running.”

STATE PODIATRY NEWS
NY Assembly Passes Expansion of Scope of Practice Law

After a long and concerted effort, the New York State Podiatric Medical Association achieved a political victory when the New York State Assembly approved Bill A3475 expanding the scope of practice to the ankle for podiatrists who have completed a CPME-approved residency or are board certified by ABPS or other certifying organization approved by  the Education Commissioner.

In previous years, this Bill has been passed by the NY State Senate, only to fail passage in the Assembly. The bill now needs to be signed by the governor to become enacted.

Query: Neutralizing Powder for Cidex

Does anyone know the name of the powder that neutralizes Cidex? Where is it sold? How much does it cost? We were told that after neutralizing the Cidex, it can be poured down the drain.

I have had success in this situation by leaving the arthrosurface metatarsal cap in and performing a Keller procedure.

Vincent Gramuglia, DPM, Bronx, NY, a2onpar3@optonline.net

I've seen several cases of Arthrosurface removal. Of course, arthrodesis is probably the best option with the most predictable results. However, as your patient does not want arthrodesis, an alternative is to remove the implant and perform a Keller with folded GraftJacket interposition. A double-stemmed implant would be a challenge because of the way the first metatarsal head was reamed for the Arthrosurface.

I've had luck using transdermal Verapamil 15% gel from PDLabs- it's a non-invasive treatment also used for Peyronie's disease, but I'm not treating that with a ten-foot pole.

I'm afraid you haven't done your homework. The well prepared resident will have started all of this about one full year before graduation: getting on insurances, finding a location, approaching banks for a loan...etc. If you are taking out a loan, you should consider that you won't get any money from any insurance carriers for about a full year, and should factor that into your loan amount.

There is money out there, but you have to be ready. You can't just walk into a bank and ask for it. The lending brokers are looking for a prepared lendee. This includes population density studies, demographic information, professional density studies...etc. It is possible to...

RESPONSES / COMMENTS (NEWS STORIES)
RE: NY Assembly Passes Expansion of Scope of Practice Law
From: Jeffrey Kass, DPM

A New York "modified" Podiatry Scope Bill was passed by both the NYS Assembly and Senate yesterday. Is this good news or bad news? This is for the readership to decide. I encourage everyone to read it. Personally, this was a huge disappointment to me. It does absolutely nothing for 95% (the overwhelming majority) of the current practicing podiatrists in the state of New York. It will serve to help the 15-20 rearfoot board-certified ABPS podiatrists who are currently in New York. It will also potentially help those graduating from programs in the future. I find it tragic that the bill does not support the overwhelming majority of the podiatric physicians. Once again, we chose podiatric surgery over podiatric medicine. Once again, we divide ourselves. What did we just accomplish? Was the majority of the profession served?
