Employees Returning to the Office Find Shoes Not Fitting: FL Podiatrist
 
After months or even years of working from home in slippers or bare feet, some people are finding they can no longer fit in their previous work shoes as they return to the office. This is because they have spent so much time at their at-home desks in slippers or barefoot, much different than narrow work shoes. Gainesville podiatrist, Jere Scola, DPM said the transition from a sedentary lifestyle, to now working 8 hours on their feet can cause heel and arch pain as well as swelling. This results in the shoe not fitting.

“When they’re sitting more, now their tendons are not getting worked as hard. They are having their feet up more so their Achilles tendon will shorten. It is like when someone has surgery and they broke their leg. Now they are in bed for a little. They try to get out of bed and have to relearn how to walk because they are not used to the low bearing forces on their body,” said Dr. Scola. He has seen increased injury because the swelling is changing the mechanics of how people walk; causing knee, hip, or lower back pain.

VA Podiatrist Interviews Dr. Simon Bartold on Running Shoe Selection

Ben Pearl, DPM recently interviewed Dr. Simon Bartold about running shoe selection and re-examining the current fit to vertical model on his podcast Fit Foot U. Dr. Bartold opined that there is an over-emphasis on the frontal plane and the sagittal plane is actually more important in a running evaluation. Selecting running shoes based on a frontal plane treadmill test is a flawed model that can lead to injuries. Also problematic is the idea of selecting a shoe model that was last year’s as the features and fit may change on an updated version.

Trends in 3-D shoe printing were discussed. Although 3-D printing has exploded, there has not been as much integration with shoe printing to date. There is a company that used pooled metric data to design a women’s running shoe. Bartold believes that this only begins to scratch the surface of what is possible when mining metric data.

4. Differentiate Yourself
 
When you ask for a payment rate increase, show the payer what value you provide compared with your local peers, Andrew Harding, co-founder and vice president of customer success at Rivet says. For example, is yours the only practice that offers extended business hours during the week or telehealth appointments on the weekends? Are you the only podiatric physician in a 5-mile radius? Harding says it’s easy to obtain this information by going to a payer’s website or an online booking service such as Zocdoc and performing a search using your ZIP code to see how many physicians practice nearby. Physicians may have market dominance within a few-mile radius and not realize it.

Query: L3000 Denials
 
We have been submitting L3000 to DME for many years and this year they are all being denied. These are for diabetic patients, they are stating you need a brace or shoe on file. Could you help me please? Do we need a specific modifier? I have looked into the HCPCS and found that this is a valid code.

Response: L3000 is a valid code. That said, this code is used for various “-itis” that affect the feet. Many insurance carriers will pay for these orthotic devices when part of a shoe with a brace criteria. If a patient with diabetes has plantar fasciitis, then this is the code to use and coverage depends on the carrier’s orthotic policy.
 
However, if the patient is at-risk and you are ordering diabetic shoes/insoles to protect the foot due to neuropathy, history of ulceration, etc., then the various A codes are the proper codes to use for shoes and insoles. Why these were paid in the past is unclear unless there was a policy change. It wasn’t clear from your post which insurance carrier you were referring to.

RE: Frustration with Off-Loading

After 40 plus years in clinical practice, I am amazed when I see progressive, acquired tissue injuries (heel decubitus). Though preventable, it also seems as if the frequency of these lesions has increased. Attempting to instruct the nurses to properly off-load the heel with a pillow placed below the calf proves to work for a very short period of time. When doing rounds, I often see the heel directly on the pillow, and oftentimes the pillow thrown on the floor. I have tried some “devices” with limited success. Am I the only one frustrated?

I have a suggestion for Dr. Udell, who reports he is contracted with an insurance company for services, and orthotics are a covered service, only at $53 a pair. I am not a lawyer, and do not consider this to be legal advice. There are laws against discrimination, and they are based on race, religion, etc. But there is no law against discrimination based on fee schedule. The contract you signed does not require you to perform every service on the fee schedule, otherwise you would have to watch a YouTube, I assume, on doing a triple arthrodesis. Just write a prescription for the orthotics in this case.

I have had the same problem, and as I am in NY as well, they are likely the same plans. For two of them, I now don't do the orthotics in-house. I give them an Rx for it to be done at an outside vendor. If your favorite pizzeria lost two dollars on every slice with pepperoni with no recourse, they would stop offering pepperoni.

Dr. Feinberg,  your comment about not being recognized as an equal may have been true in your day, and it may still be true in certain areas of the country. However, the younger generation of orthopedic surgeons who did their residencies with us, shoulder to shoulder, see that we are equal. Podiatry schools that were stand-alone for decades with no affiliation with DO schools, may have a problem with DOs and MDs not seeing them as equal. In Des Moines and other schools, where for decades, the first two years of medical school are the same for DPMs as DOs, we all had the same teachers, classes, and exams - all sitting together (except we didn't do OB or Psych).  These DO colleagues regard us as an equals because we did the same MEDICAL training in school and even did better than them academically in many instances. 
 
Dr. Feinberg, respectfully, I am afraid that you are living in the past, 40 years in the past, with your neurologist colleague who was right about us largely in the past but very wrong about us today. Stand-alone podiatry schools have to go, and they must truly integrate into an MD or DO medical school like Iowa.

It's sometimes hard to be truthful about our problems without being accused of being anti-podiatry, and in the case of Dr. DiResta's post, partially responsible for the decreased interest in podiatry among college students. In my experience with a few thousand students and a few hundred residents, and the vast majority of my colleagues, I am quite sure that not a single one ever questioned my passion and love for podiatry.
 
Dr. DiResta states emphatically that he has no problem with podiatrists saying they went to medical school. Okay, but people who did go to medical school, and regulate and certify medical schools, do have a problem with it. When that problem is solved, by "filling in the gaps" as he says, who does Dr. DiResta think will be saying...

As much as I admire Dr. Brenner, I must comment on the statement that DPMs and estheticians are performing "medical pedicures." Know this: Estheticians are not licensed to touch the feet or perform pedicures in any of the tri-states and are not trained to do so. Reality is that they are not licensed to touch the feet in any state, though in a few, they can "add moisture." They cannot wield instruments, files, polish, etc.
 
Pedicures are a regulated skill in every state in the U.S. and estheticians are not allowed to perform them. And when performed by a podiatrist, they are called "routine foot care" for that reason. Additionally, training estheticians to do "medical pedicures" by shadowing a podiatrist is not adequate for performing this service, nor is it legal or safe. One infected nick or cut by an esthetician performing a "medical pedicure" will not be supported by your insurance company in the case of a lawsuit. Check it out. An esthetician's legal standard operating procedure (SOP) involves the face and ends at the decollette except for the "adding moisture" thing in some states. BTW, I am an internationally certified and licensed esthetician and have trained estheticians for 20+ years. I know their SOP.
