Limb Length Discrepancies Common in Athletes: CT Podiatrist
 
"Limb length discrepancies of the lower extremity often cause disabling problems of the hips and lower back," says podiatrist Dr. Robert Weiss. "It is also known as the short leg syndrome. This problem may affect as much as 75-80 percent of the population, resulting in pain in the lower back, pelvis, or hip. Pain may also radiate down the thigh (sciatica nerve)," says Weiss.

"The majority of symptoms are usually found on the side of the longer limb because it is in contact with the ground longer and thus absorbs a greater amount of pressure and stress. The short limb also develops various symptoms such as shin splints, which are usually a result of the short leg overstraining while walking or running. A functional limb length discrepancy will respond well to biomechanical orthotic therapy.

OH Podiatrist Comments on Trendy Chacos 
 
A brand of outdoor sandals called Chacos (“CHAH-kohz”) — reflects a trend toward comfortable, if not high-fashion, footwear. With thick rubber soles reminiscent of Crocs — the shoe of choice in the mid-2000s — and crisscrossed straps in more than 40 colors and patterns, Chacos were originally designed for adventure sports such as hiking and kayaking.

Dr. Said Atway, a podiatrist at the Wexner Medical Center at OSU, has seen a decrease in common foot problems, including pinched nerves and collapsed arches. As consumers become more aware of the potential harm of flip-flops and flat sandals, he said, they switch to footwear that feels good. Even so, Atway said, no summer shoe compares to athletic shoes or boots in terms of cushioning. Still, because summer inevitably means sandals, “You’re better off with something that’s going to give you some kind of support.”

APMA Named a Top Workplace by The Washington Post
 
The American Podiatric Medical Association (APMA) today announced it has been selected for the second year in a row as one of The Washington Post’s Top Workplaces. The Top Workplaces are determined based solely on results of an anonymous feedback survey completed by employees. APMA was one of the first 150 organizations awarded the distinction on the 2014 inaugural list, and was selected as number 40 from the more than 300 companies evaluated in the Washington, DC, area for the 2015 list.

ICD-10 UPDATE
AMA, State Medical Societies Call on CMS to Mitigate ICD-10 'Disaster'
 
The countdown is on toward the Oct. 1 start date for the nationwide conversion to the ICD-10 diagnostic and procedure codes, so the American Medical Association and four of the nation's largest state medical societies are reiterating their long-held opposition to the switch.
 
The latest blasts aimed at ICD-10 come in a resolution by the AMA and a letter to acting CMS Administrator Andy Slavitt from the medical societies in Texas, California, Florida, and New York, calling the pending conversion a “looming disaster.” The AMA statement repeated its long-held policy stance against the ICD-10 conversion, but if that's not feasible, for the CMS to adopt “mitigation strategies.”

Common HIPAA Misconceptions - Part 3

3. Requiring Unnecessary Business Associate Agreements
 
HIPAA requires practices to have written agreements, often referred to as business associate agreements (BAAs), with other entities that receive or work with their PHI. Essentially, the agreements state that the business associates will appropriately safeguard the PHI they receive or create on behalf of the practice.
 
Still, some practices take unnecessary precautions when it comes to BAAs, says Robert Tennant, senior policy adviser of government affairs for the Medical Group Management Association. "A lot of practices are very concerned about people like janitorial services [and] plant maintenance folks, and they have them sign business associate agreements, but those folks are not business associates for the most part," says Tennant. "You may want to have them sign confidentiality agreements basically saying, 'If you do come across any information of a medical nature, protected health information, you are not permitted to look at it, copy it, keep it ...,' But, you do not need to sign a business associate agreement with anybody other than those folks that you actually give PHI to for a specific reason, like if you've got a law office or accounting office or a shredding company that is coming in to pick up PHI to destroy it."

Query: Foot Care and Nail Avulsion Coding
 
I am wondering if I can bill for reduction of toenails >5 and callus trim 2-4 lesions ("at risk" or routine foot care) along with a two nail avulsions (with a local anesthetic) on the same day. If so, then how would I code this, and what modifiers would I use? If not, then still can someone show me how to code this? Can I use an E/M code if I bill the nail avulsions separately?

Response: There is no reason that you can not bill this combination. Not sure what you mean by using an E/M if billing the nail avulsions separately. If all the services are done on the same day, then all should be submitted on one claim form. If the E/M is significant and separately identifiable, it should be billed as well. As always, documentation is key.

The nails would be CPT 11721-59. The calluses CPT 11056, and the nail avulsions CPT 11730 for the first, and CPT 11732 for the second nail.

There is no formal brick and mortar school for podiatric medical assistants. The American Society of Podiatric Medical Assistants (ASPMA) fills this gap by offering study materials and certification exams for assistants. Exams are available in both Administration and Clinical assisting and are available in conjunction with many podiatry conferences throughout the country as well as online. Membership is required and assistants must be employed by a doctor member of APMA. CEUs are required annually and ASPMA offers the opportunity to obtain these credits.

I agree so much with what Drs. Hultman and Rosenblatt have stated in the article and post in response. However, I would make one suggestion. Just like a good attorney, who never asks a question in a courtroom  unless he already knows the answer, I recommend the following. The APMA should work with the colleges of podiatric medicine and have groups of students, or even whole classes take the licensure exams in that particular state. Let's make sure that our education is truly giving our students the ability to pass the exam before we open this up to official scrutiny. I have been involved in residency training for my entire career of 26 years, and I am absolutely certain that our graduates can be competitive in almost all aspects of medicine, but I would like to be absolutely certain that they can compete in ALL aspects of medicine.
