Dr. Mann said, "Research suggests that there may be impaired thiamine (vitamin B1) metabolism associated with many neurodegenerative diseases, including ALS. Diabetic and alcoholic polyneuropathies, both neurodegenerative diseases affecting the feet and legs, often respond to benfotiamine. Work on benfotiamine and Alzheimer's disease looks very promising. Hopefully, further study of this phenomenon will yield therapeutic solutions and help people suffering from these terrible diseases."

ON THE LECTURE CIRCUIT
VA Podiatrist Presents Running Shoe Workshop
 
Ben Pearl, DPM recently presented  a running shoe workshop to the Shirlington Run Group in Arlington, Virginia. Dr. Pearl, a member of the group, teamed up with a running shoe company to stage a virtual and live event. The recorded event reviewed the concepts of stack height and drop, and how they influence the performance and ride of the shoe. Pearl showed runners how to properly fit a shoe and demo running shoes with various stack heights and heel drops.

Pearl talked about how wider toe box and lower heel drop shoes have helped him with his neuroma and turf toe. He suggested runners consider varying the heel heights of their shoes slightly and mix up the terrain to lower the incidence of overuse injuries from the same repetitive motion.

An important last step is building and maintaining a positive and healthy workplace culture. Recognize that you are not just a medical provider but also a leader and the person who sets the tone and controls the culture. Be intentional about developing yourself as a leader. Read books, attend leadership conferences, and even invest the time and money to work with a qualified coach. Finally – be selective about who joins your team. Hire people who will do more than just complete the task at hand. You want strong individuals who will personify the culture and brand you've created, and who want to present that culture to your patients and colleagues.

RE: The Passing of Steven Joel Levitz, DPM
 
Dr. Steven Levitz, Professor of Orthopedic Sciences at The New York College of Podiatric Medicine (NYCPM), died June 20, 2023 at the age of 72 as a result of a fall. A graduate of NYCPM (1979), he completed his podiatric residency (1980) and a fellowship in orthopedics and pediatrics (1981) at NYCPM. He was the senior-most faculty member of the school.

I know little about prolotherapy, nor do I have any expertise in holistic medicine. With that preamble, I find the demands on Dr. Kornfeld to produce literature defending his use of prolotherapy both interesting and somewhat disingenuous. For many years, Dr. Ribotsky, as an example, advocated the use of fillers for the treatment of various foot lesions. Where was the literature to support that? Anyone familiar with the 2023 International Working Group on the diabetic foot guidelines will note that the majority of wound care products advocated by “thought leaders in the diabetic foot” within our profession, have little, if any, studies to support their use. Yet my colleagues continue utilizing these modalities.
 
I am sure that Dr. Kornfeld would not utilize therapies that he did not feel were in the best interest of his patient and did not provide satisfactory outcomes. He would be a fool to...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Breakthrough with Wellcare with the -25 and -59 modifiers

We recently had a breakthrough with Wellcare (formerly HealthNet) regarding reimbursement issues with the -25 and -59 modifiers. OPMA has been working with Wellcare representatives on denials of payment pertaining to the use of modifiers -25 and -59. Their Medicare Advantage plan has been denying various combinations of office visit and procedures for years. We have worked with them to implement a customization for podiatry which will allow providers to bill per correct coding and not require them to submit medical records. The claims must be appropriately modified and have a different diagnosis on “trigger” lines to support modifier use. This will eliminate submission of medical records if these criteria are met and will remove the validation of time (the 6-month follow-up window) as it never should have been applied to these claims.  This customization went into effect on June 17.
 
We are also working with them on the YS denial code which appears to be a denial but is really a request for medical records to support paying the claim. This is a Centene-wide systems configuration issue and if they do ultimately agree that it is deceptive, we have been told that it could take a long time to fix.  However, denial codes involving the YS code should be drastically reduced due to the above customization, hopefully resulting in less administrative paperwork and quicker payment. We hope what we have achieved in Oregon will empower those in other states to get similar issues resolved.

RE: Two Podiatric Certifying Boards is Enough

Our profession needs only two recognized boards, the ABPS and ABPM. Peruse through our profession’s many journals and count the number of boards we have. We think that more is better. My son is an internal medicine doctor and is board certified in one board only, with no need for him to have more board certifications on his CV.
 
Also, our profession needs to advertise in the orthopedic surgery and medicine journals to explain what our profession does and the training our residents complete. My nephew is an orthopedic surgeon specializing in hand surgery. He was also chief of the Ortho Dept. at his hospital, and he did not have a good understanding of how podiatric residents and physicians actually are trained.

Cigna Health Care recently announced a total limit of four injections for each CPT code 20600 Arthrocentesis, aspiration and/or injection; small joint or bursa (e.g., fingers, toes); without ultrasound guidance) or 20605 (Arthrocentesis, aspiration and/or injection; intermediate joint or bursa (e.g., temporomandibular, acromioclavicular, wrist, elbow or ankle, olecranon bursa); (without ultrasound guidance).
 
This misguided policy by Cigna Health Services should be the last straw for providers to seriously consider stopping cooperation with any insurance company with misguided policies. How in the world will a hand surgeon know that a DPM or other MD/DO provided an injection to the foot/ankle or...
