AT THE COLLGES
TX Podiatry School Prepares to Open in August
 
Texas finally has a school of podiatric medicine and it happens to be in Harlingen, housed within UT Health RGV, the clinical arm of the University of Texas Rio Grande Valley School of Medicine. With the blessing of the Council on Podiatric Medical Education, UTRGV becomes the first university in the state and one of just 10 in the entire country to teach and graduate Doctors of Podiatric Medicine (DPM). Classes will begin in early August of this year.

Javier Cavazos, DPM, assistant dean of clinical education for the new school, said students and faculty will serve the community through UT Health RGV, the Area Health Education Center (AHEC) program, Coastal Bend Veterans Affairs, and Federally Qualified Health Centers, 16 of which are scattered across the Valley. “Diseases endemic to our region, like diabetes and cardiovascular disease, are a research focus for our faculty and students,” he said. “We will collaborate with the UTRGV School of Medicine and the South Texas Diabetes and Obesity Institute to provide evidence-based practice and medical research to our community.”

Negotiating a successful payer contract increase is possible for those willing to craft a thoughtful, data-driven argument for why they deserve more money.
 
3. Provide cost and quality data.
 
Even if you’re not part of an ACO, CIN, or other type of value-based contract that requires you to capture certain quality and cost metrics, you’re probably participating in Medicare’s Merit-based Incentive Payment System, which does require some of this data collection. What do Medicare data say about the care you provide, and how might you use those data to your advantage when negotiating with commercial payers? Use those results as leverage.

RE: The Passing of Ken Vogen, DPM
 
It is with great sadness that I inform our podiatry family that Ken Vogen, DPM passed away March 4, 2022. He died in his sleep from a heart attack at the age of 69. Ken graduated CCPM class of 1981. He swam and played water polo in high school and college. He worked as a Huntington Beach, California lifeguard for years, saving many lives.

He worked for the West Los Angeles VA for over 10 years. He finished his podiatry career in Laguna Woods where he and I worked together for 12 years. He was loved by his patients. He is survived by his wife Stephanie and his four adult children. Rest In Peace my friend!

After 39 years of practicing podiatric medicine and surgery, I retired on March 31, 2022. That was my way of ending my burnout. I sold my practice to my associate Dr. Amr El Khashab. He has been my associate for the past 5 years. I know that there is more to life than podiatry. After going to college at 16, going through 8 years of college and podiatry school and 2 years as a Northlake resident in Chicago Illinois, I felt it was time to smell the roses. To all my colleagues, I wish you all the best and don’t wait too long. At 67, I don’t know how much more time I have left. I’m not sick but you never know. Keep in touch.

Perhaps Dr. Peslar can advise me and any others with a problem we are having with getting paid for orthotics. I am contracted with many insurance companies in my geographic area and am bound to accept what they pay for podiatric services. The problem is that they tell their clients that they are fully covered for custom foot orthotics. What they don't tell them is that the doctor (in this case me) will get paid 26 dollars and fifty cents for an orthotic, which is far less than the lab fees.
 
I've had patients willing to pay upfront for good custom sports orthotics only to be told by the insurance company, months later, that I owe the patient a refund and need to accept the twenty six dollars and fifty cents. Some of the coding gurus in our profession told me that I should drop out of the participation, but this would be cutting my nose to spite my face. I would lose compensation from all other services. Has anyone else encountered this and what have you done?

I don’t think the poor sentiment regarding medical practice is limited to podiatry. I think many of our MD and DO colleagues feel the same about recommending medicine as a career to their children. There were a lot less parents hooding children at my son's graduation this year than there were when my brother-in-law graduated medical school 25 years ago.

Dr. Chisholm succinctly outlines the problem with “podiatry” as a profession. We are the only graduates of residency training that graduate able to do less than when we were residents. This is dictated only by our limited licensure not by our degree. Changing the degree to an MD or DO will ultimately destroy the profession, as fewer and fewer graduates would take residencies in podiatric medicine and surgery. The profession would wither on the vine over time and disappear. Clearly, the best path forward is to expand training, testing, and licensure with the DPM degree. I am proud of my profession and I take great pride in the work I (and my colleagues) do every day.

I've been following with great interest the different opinions as to why our applicant pool is so low compared to the MD and DO schools and I'm surprised that no one has mentioned one obvious factor: "routine" foot care. I would imagine that most mainstream podiatric practices still involve doing a fair amount of regularly scheduled RFC, and I doubt that most young people interested in a career in medicine want to spend a substantial portion of their day trimming or debriding toenails.
 
Is this part of practice necessary? Absolutely yes! The need is there, especially in the geriatric and at-risk population. But why should a DPM, with 4 years of undergraduate education, 4 years of podiatric medical education, and 3 years of comprehensive medical and surgical training be required to be...

I grew up on a cattle ranch in California and our family veterinarian reminded me that animals were not covered by health insurance. I was a pre-vet major in college. In 1965, there were 13 veterinary schools and 10 optometry schools nationwide. Today, there are over 20 optometry schools and over 30 veterinary schools. I am sure there are a number of optometrists and veterinarians who would love to be a podiatrist if they could be given advanced standing in podiatry school. Just a thought.
