2 VT Podiatrists Graduate from SVHC Podiatry Program
Southwestern Vermont Health Care has announced the graduation of two residents in its podiatry residency program. At a small, masked-and-distanced gathering of health system leadership, Cody Anderson, DPM and Kurt Malkames, DPM were awarded certificates of completion. Dr. Anderson will originate a hospital practice in a rural community in South Carolina, and Dr. Malkames will join a private practice in Silver Springs, MD.

Six residents are currently in the program. They are third-year residents Shazad Buksh, DPM, and Breton Fox, DPM, and second-year residents Kyle Lewis, DPM, and Nathalie Santos, DPM, and first-year residents Daniel Hennessey, DPM, and Daryl Silva, DPM.

CT Podiatrist Discusses How Much Children Should be Running
"Many people are taking to the roads to run during these trying times, which is a good way to distance from anyone who might be infected with coronavirus and still get in exercise. So many parents are constantly asking if children should run? There has been a growing concern that running is dangerous for children," says Robert Weiss, DPM.

"The parent of a young runner should do what he or she can to keep running fun for the child. Try not to push. Let running be something the child chooses to do. Keep it in moderation, and allow the child a way out if he or she so chooses. The danger comes when the pressure to run is placed on him or her by enthusiastic parents, coaches, or peers. That is when the child will run even though it hurts," says Dr. Weiss.

Aussie Podiatrist Reports 'Influx' of Ugg Boot Foot and Slipper Injuries
A Riverina podiatrist says he has been inundated with cases of 'Ugg boot foot' and other slipper-related injuries as people remain hunkered down at home without appropriate footwear. Wagga Wagga's Active Foot Clinic's principal podiatrist Ben Hinton said he had seen a "massive influx" of patients.

"People being at home, in working from home situations... they've normally got really good shoes, or they wear orthotics for a condition ... all of a sudden they go 6 to 8 weeks without." His practice has seen up to 40 people a week over the past few months for "home related" injuries. Hinton said he had treated other injuries from people going "too far too soon" in their lockdown-inspired exercise regimes.

Family Sticks Together At Bianco Brothers Instruments
Identical twin brothers Vincent and Joe Bianco run the shop, once a bustling gathering place for family and neighbors. Amid the pandemic, the business has been cut back to a drop-off and pick-up service. “The tough part is making the change and not being able to deal with customers the way that you always did,” Joe’s son Peter said. “They all want to shake our hands or give us hugs and kisses,” Joe said. “We have to walk away from that and bump elbows or blow kisses through the window.”

In addition to sharpening knives for everyone from chefs to knife swallowers, Bianco Brothers disinfects and sharpens medical tools for healthcare workers. Above all, the Biancos rely on each other. “Family is how we’re going to survive this,” Vincent said. “My brother Joe is a rock. He’s my twin brother. He’s not just my brother. He’s part of me,” Joe said. “We’ve always been together, so we will always do everything together.”

Lacking in Estate Planning Documents: While many physicians deserve kudos for keeping and maintaining important documents around their practices, they aren’t always known for being as vigilant with their personal estate planning documents. Pertinent documents include your financial powers of attorney and medical directives while you are alive and your wishes for your estate after you pass. In the absence of these documents, your family could be exposed to making tough medical decisions on your behalf, a lengthy probate process, and costly attorney fees—all of which could be significantly injurious to their financial well-being.

RE: Where Did Biomechanics, the Foundation of our Profession, Go?

Over the past few months, I have been spending many hours on webinar after webinar and two significant observations have become crystal clear to me. 1) It’s incredible how bright and surgically talented the young presenters are with their operative skills and 2) The foundation that separated our profession from orthopedics has been overlooked. From lectures on bunion surgery discussing hypemobility of the 1st met-cuneiform joint to hammertoe surgical reconstruction, there is ZERO discussion on the biomechanical etiology of these conditions.
Gait analysis and applied biomechanics are what has and will make our profession catapult forward. I often ask about the biomechanics on these webinars, but the question, if chosen is barely answered. With ABPOPPM (now ABPM) and ACFAOM (now ACPM) dropping the word “orthopedics”, are we losing the skill to determine the difference between open chain kinetics and closed chain kinetics pathology. If so, how can correct surgical procedures be explained? As a past president of ACFAOM (1999-2001) I am very worried with the direction our profession is going. Am I alone?

We are all struggling to open our practices and provide the safest possible environments. Some doctors and dentists are requiring that patients' temperatures be taken before they are seen. Others require that patients attest that they have no symptoms of the coronavirus. Needless to say, all offices require social distancing in the waiting rooms and the use of masks by patients and all staff.
Now there is a big elephant in the room. Irrespective of your political beliefs, what do you say to a patient when he or she says to you, "why are you being so strict when the President of the United States just conducted a political rally where there were thousands present and there was no social distancing and the majority of attendees were not wearing masks? Combine that with the protest marches in many cities, applauded by many local government officials, where there too was no social distancing and many participants did not use masks.

Essentially all the noteworthy, professional, and highly-trained immunologists would strongly disagree with the statement from Dr. Kirlin’s comment, “The effectiveness of double[cotton] masking may be as effective as the vaccine that we’re waiting on. We could be achieving the same thing a vaccine would if we all just wore our masks.” What is his source of information? Is this his opinion, and if so, what statistics or science is that based upon?

I first met Robert Sheinberg 40 years ago at NYCPM where he excelled as a student. We have practiced in the same Florida community for the past 25 years. He was a brilliant, well respected, and very talented physician. He was extremely philanthropic as he and his wife helped establish the local "Y". His passing happened way too soon and he will be missed by his family, friends, patients, and community. Very sad.

The PM News poll shows that 1/3 of those responding still grind fungal nails without a dust extractor. Considering the risk that aerosol toxic nail dust poses to your health and that of your patients, I cannot understand why some continue to use this technique.
Better options include medically treating dystrophic fungal nails: Having patients use a nail softener or topical antifungal on their nails to permit sharp debridement, nail surgery, and oral antifungals. I urge those of you who continue to grind nails to consider changing to a safe, medically indicated treatment method.
