UK Podiatrist Discusses Cracked Heels
 
Whether painful or not, cracked heels can be unsightly, especially if you are planning to wear sandals. Podiatrist Dina Gohil says, “They appear as the result of a loss in moisture in the heels and as we walk, the pressure on the heel pad causes the callus on your heels to crack. “Some of the most common causes are obesity, ageing, pregnancy, diabetes, hypothyroidism, fungal infection, vitamin deficiency, and biomechanical imbalance like flat feet.”

You might benefit from taking a multivitamin supplement, as vitamin C, E, and B deficiencies have been associated with cracked heels. Dina recommends staying hydrated and using a pumice stone to gently file away dry skin. She says, “For chronic cracked heels that are painful to touch, make sure to see a podiatrist to help safely and effectively restore your heels.”

6 Steps to improve Your Practice Finances
 
Here are six specific areas you can control that will help enhance your practice finances, ensuring a healthier financial future.
 
5. Structure consistent income
 
In the long run, establishing structured and consistent income is a more practical and manageable compensation plan, rather than relying on periodic large bonuses. While some physicians set low salaries to be conservative and then receive large bonuses or "true-ups," this system makes it very challenging to maintain regular personal savings or investments. Not to mention those pesky surprise expenses that always seem to rear their heads right about the time bonuses are paid, interfering with savings intentions. A better strategy is to set your salary at 70-80% of anticipated production so you can automate saving toward personal goals. Then utilize smaller bonuses toward one-time expenses and paying down debt.

In a post-operative period, can application of a new cast be billed? If so, is a modifier needed?

Response: Application of a cast during the global period, which I assume you are alluding to, is a covered service and not included in the global fee payment. You can bill for the cast application and for the materials. No modifier is needed as it remains outside the global period.
 
CPT 29405 Application of short leg cast (below knee to toes) is likely the most common code utilized for a below-the-knee cast. Materials vary from Medicare to local carriers.
 
Q4038 for fiberglass supplies and Q4037 for plaster supplies.
 
Q4039 and Q4040 for pediatric plaster and fiberglass, respectively.

Dr. Ribotsky implores me to "publish my outcomes". But not having lived in my shoes, he does not understand. I will explain. There are 2 parts to the story. 1) I adopted a more holistic paradigm in 1987 after a chronic illness (which could not be helped by any conventional doctors for a few years) was healed and resolved within 3 months of seeing a holistic internist. I experienced the power of natural healing firsthand. I dove into learning everything I could and began experiencing its power on my patients.
 
I wanted to share this with the profession so I spent a number of years lecturing about the things I do to assist my patients to heal. Although I was invited to speak a number of times at large podiatric conventions, my efforts were not being well received by the podiatric community. So I started trying...

Glen, thank you for enlightening us on your successes with prolotherapy. Perhaps you can publish your technique as well as findings. If you can document that prolotherapy works, the treatment will no longer be in the realm of alternative healthcare. If the article is well written, there are many journals in our profession that would love to publish it. As for charges, I based this on what a certain non-podiatric physician in my geographic area was charging and it was well over a thousand dollars for each treatment. Some of these patients were quite upset after spending 10K, out-of-pocket, for a series of dextrose injections.

I’ve received a few emails and calls about a note I wrote last week about my thoughts on the ACPM Bylaws ballot sent to members. I was not advocating a position on the two choices on the ballot. What I was saying is that EACH ITEM should be voted upon individually.   
 
Proposed bylaw changes:
1) no longer require APMA membership for renewal
2) broaden the category of those who are eligible for fellowship beyond just ABPM diplomates.
