CT Podiatrist Discusses Hallux Rigidus
 
“When it comes to arthritis, it’s important not to tough out symptoms or bear the pain,” says Danielle Butto, DPM, a foot and ankle surgeon and Fellow member of the American College of Foot and Ankle Surgeons. “Earlier treatment is not just about alleviating symptoms sooner. In many cases, we can even slow the progression of the symptoms, and use less invasive procedures to treat the condition than we would otherwise.” One common area where osteoarthritis occurs is the big toe. The big toe makes it possible for you to walk and run upright, absorbing forces equal to nearly twice your body weight when walking. With all it endures, it’s no surprise that overuse can erode cartilage, causing serious pain and even physical deformities.

Nevertheless, many people confuse hallux rigidus with bunions. Early signs of hallux rigidus include pain and stiffness during use, or during cold, damp weather, difficulty with activities like running and squatting, and swelling and inflammation around the joint. Additional symptoms may develop over time, including pain during rest, bone spurs, limping, and dull pain in the hip, knee, or lower back due to changes in gait. If you notice any of these symptoms, see a foot and ankle surgeon. Conservative treatments like shoe modifications, orthotic devices, and physical therapy, may prevent or postpone the need for surgery.

Some factors can lead to the development of diseases and complications. “Being overweight can interfere with walking. Wearing the wrong type of footwear, especially uncomfortable shoes, lack of hydration, exposure of the feet to sun without protection, and spending long periods standing are some of the main factors,” she points out.

6 Steps to improve Your Practice Finances
 
Here are six specific areas you can control that will help enhance your practice finances, ensuring a healthier financial future.
 
4. Properly manage cash flow
 
One significant error we often see is allowing idle cash to build up in a practice. Avoid this issue by regularly working with your practice consultant to determine appropriate reserve levels and accurately forecast expenses so that excess cash flow can be distributed to the owners at regular intervals. While 401K plans with profit sharing can certainly help accelerate retirement savings and tax deductions, don't get caught off-guard by high year-end profit-sharing calculations. Be sure to coordinate with your plan advisor to estimate the total amount needed to contribute for the practice, and escrow money each month in anticipation.

RE: The Passing of Carl J Di Franco, DPM
 
Carl J. Di Franco, DPM, age 88, passed away with his wife and children by his side on May 25, 2023. He was drafted into the U.S. Army and served in San Antonio from 1958–1960. After his military service, he attended the Ohio College of Podiatric Medicine, graduating in 1967.

After a successful internship in 1968, he started his practice in 1969. He ran a small but successful private practice for 35 years, retiring in 2004. He was an amazing doctor and confidante to his patients. Not only did he take the time to treat their feet, he listened to their stories and troubles.

The doctor who started the discussion was asking advice for treating interdigital neuromas and the proper dosage of the % medium used. Here is my experience with prolotherapy:
 
I have been using prolotherapy for over 10 years, stabilizing ankles, repairing torn plantar fascia, tendon and plantar plate tears, and even neuromas. When given in the right dosage, the right way to inject and absolutely no anti-Inflammatories such as Advil or ibuprofen or aspirin and say in the ankle, when it is set properly, it is very effective. I suggest that you keep your license active in so-called retirement. At age 60+, as long as you are in good health, there are opportunities that pop up. It’s also fun to go to podiatry meetings to meet and greet old friends. It keeps you from going stale and keeps you mentally active. It’s amazing as to what can be in front of you. Why waste that possible opportunity?

I gave up my NC license 4 years after retiring in 2018, reluctantly I might add, but I think it was the right decision. There are some good reasons to keep your license including: 
- opportunities to help colleagues who need short-term assistance with their practices because of illness, or injury.
- boredom, especially for those who don't have other compelling interests to keep them busy. You might want to do a little work on the side after a brief period of retirement.
- possible community medical service in local free clinics or foreign mission work. Lots of possibilities here and most countries do require an active license.
- continued connection with your colleagues and friends while attending CME meetings.
 
There are also a few reasons to consider giving up your license:

I sold my office in 2018. I then continued to see patients 9 hours a week at nursing homes and a few house calls. Then Covid hit and I quickly pulled the plug. I contacted the PA board and went Active Retired. Naturally, you still pay the licensing fee. You can still treat family members and prescribe. I also cancelled my malpractice insurance which had a tail. It took about 2 years for some of the insurance companies to stop sending info. This year, I shredded a number of retained records, keeping the required years. You must stay busy at whatever level you can. Family and health are the priorities.

“Suum  cuique” - To each his own. As I live in South Florida, home to many retired physicians, many of whom were my patients for many years, I’ve asked this question often. Almost all specialties keep their licenses active. Personally, it has been 8 years since the Coast Guard accident that placed me on permanent disability, I’ve given up my board certifications (even before they expired) and my DEA license. I’m keeping my Florida license (taking CME, etc.). Who knows the future? And if you do, please share your Powerball numbers.

The X-Cel x-ray machine, (specifically designed for podiatry for use with film and chemistry) has been the podiatry standard and workhorse for DPMs for many, many years with numerous machines manufactured in the '80s still in use today.
 
Amazingly, these machines can be modified with today’s technology, (DR-ready) by replacing the 8-in. high Orthoposer base with a low 3½ in. one, ADA handicap-accessible base (unlike the AD2D system’s very high 8-in. base).
 
Old or new, together with today’s wide range of hospital-grade DR imaging plates and modest pricing makes the X-Cel still the podiatry standard.

Most companies sell one product that they, of course, will say is the best. We sell multiple systems from multiple manufacturers. When considering x-ray solutions, there are really three products. 1. The x-ray machine that generates the x-ray. 2. The panel or system that develops the x-ray. 3. The software that processes and enables you to view and store the image.
 
Every practice is different with various levels of technology, needs, desires, and capabilities. When you try to integrate a new system into your practice, there are many things to consider.
