MD Podiatrist Discusses Buying (and Replacing) Sport-Specific Footwear
 
Running, basketball, volleyball, hiking, and tennis are some of the activities for which it's essential to have a sport-specific shoe, especially if you're a more serious athlete who wants to maximize your performance, said Damian Roussel, DPM, a podiatrist and foot and ankle surgeon with The Centers for Advanced Orthopaedics in Frederick, Maryland. A good-quality running shoe is essential. Look for a light, flexible style that will absorb the repetitive impact of the foot hitting the ground, says Dr. Roussel.

Sometimes, shoes look and feel fine, even when they are worn out. So a way to keep tabs on them is mileage and time. Running and walking shoes should be replaced after 300 to 500 miles of use (485 to 805 kilometers), Roussel said, and basketball shoes after 45 to 50 hours of play. "For aerobic dance, or tennis, replace shoes when they show signs of unevenness on a flat surface, or when they display noticeable creasing," he added.

Walking is the most popular form of exercise with more than 145 million Americans calling walks a part of their regular routine, according to the Centers for Disease Control and Prevention (CDC). Because each person's foot and body are unique, a shoe that's good for you might not be the same for someone else. Here are the factors to consider when you're purchasing a good pair of walking shoes: Unique to each person, your arches need to feel supported, especially as you increase your mileage. "Arch support allows the big toe joint more mobility and to support proper alignment," says Dawn Figlo, DPM, member of the American Board of Podiatric Surgery, and founder of The Organic Foot in New York City.

Look for a wide toebox. Comfort is personal, especially when it comes to the width and shape of your foot. If you have wide toes, a narrow fit probably won't work for you. "A wider toe box will prevent overcrowding of the toes and decrease the influence for bunions, hammertoes, and neuromas," says Dr. Figlo.

Dr. Ribotsky wrote that the repair charges at a garage were $285/hr. He asked, “I’m not sure how many DPMs are able to increase their fees to keep up with what things are costing today?” 
 
I remember 45 years ago, the orthotic fee was $400. Today, the charge is $500. Using the Smartasset inflation calculator, the 1977 $400 fee would be $937 this year at 2.97% average inflation rate. Just to stay a little above of the average inflation rate, $950 would be reasonable. I’m wondering how many DPMs are charging $950 for orthotics?

The biggest hindrance I find in podiatry is every DPM seems to have a different value they place on what they do. There isn’t really an industry standard in confidence. Few DPMs charge comparable prices to MDs or DOs. Patients see the irregularity in pricing and they also see how the doctor values their service by what is charged. 
 
I’ve experienced a lot of different doctors’ offices in NYC doing locum tenens work. Too many podiatrists here undervalue what they do. I can tell by how patients treat me doing locum tenens work how well the DPM is respected at that office. Cost of service is perceived to be linked with quality received. My thought: our profession needs a way to lift itself up as a whole and support each other; rather than do things like financially take advantage of newly graduated residents and young physicians.

In reading some of these comments, you might wonder why any potential applicant would want to be a podiatrist. Really? Why do some of our most esteemed colleagues belittle podiatry school training and perpetuate the myth that there is no resemblance of parity with our MD/DO colleagues? Why? When podiatrists post medical treatments in this blog that some perceive as beyond their scope, they immediately call foul! Why? And knowing our residency programs today, how can they comment that somehow our residencies would never be acknowledged by ACGME? If you don't want parity, you won't ever get it.
 
I love being a podiatrist and I am forever advocating for our profession and for parity, especially with hospital privileges and third-party reimbursement. Knowing the large variation in educational training programs between...

RE: “Who wants to be a podiatrist?”

I think this topic is a good reminder that our profession has very passionately-held opinions about the need for real parity, and the best way to achieve it. It is also becoming clear that our perspective on this issue is heavily influenced by the type of education and training that each of us received, and how recent that education and training was obtained. 
 
To paraphrase the great Jon Hultman, "today's graduates of podiatric medical school are either (1) over-trained or (2) under-licensed". The only systematic study of the comparison between medical, osteopathic, and podiatric medical schools that I know of was the California Physicians and Surgeon's Taskforce. Its findings were that at the end of 4 years of podiatric medical school and 3 years of residency training, the education and training of the podiatric graduate was comparable to that of a medical or osteopathic graduate. It is foolish, and useless to base my opinion on how my education at CCPM in 1986 and my PSR-12 residency compares to those of an MD or DO. Today's graduates have a broad-based, comprehensive educational experience. I didn't. 
 
Another pertinent quote, "it's the license, not the degree, that determines what you can do as a doctor." As long as we practice with a limited license, we will not be able to practice to the extent of our education and training, and thus will not have parity. All efforts towards parity should be focused on changing the license of a DPM to an unlimited, plenary license in our respective states. Wishing for an MD or DO degree to magically appear is...foolish.
