Marathon Preparation Requires Two Years: IL Podiatrist

The weather is warm and the race is on to get in shape for one of the many charity runs or marathons in your area. But if you haven't been training on a regular basis, you've got to walk before you can run, experts say. "The attrition rate is fantastic because people go out too fast and get injured. Once people get injured, they drop out," says Stephen Weinberg, doctor of podiatric medicine at Weil Foot and Ankle Institute, Chicago.

In fact, you should be running regularly for at least two years before you tackle a marathon (26.2 miles), says Dr. Weinberg, who is a marathon runner and has served as medical director for the Chicago Marathon for 21 years.

MA Podiatrist in Top Quarter of Finishers at Boston Marathon

Dr. Robert Martin, a Manteno-based podiatrist, could almost host a small marathon with all of the patients from the area that he has served since joining Prairie Rock Foot & Ankle Clinic. He knows how important healthy feet are to a runner, not to mention their value for everyday pedestrians. But, Martin also knows how important good foot health is to a runner because he’s a runner himself. In fact, last month, he ran in the Boston Marathon.

Dr. Martin finished in the top 25 percent of runners in the race with a time of 3:41:24. He had hoped to do a little better, but was still quite thrilled and honored to have made the run at all. “It was a difficult day because it was so hot,” he said. “I was shooting for 30 minutes faster.” All the same, Dr. Martin’s time was still worth kicking up his heels over. The Boston Marathon was only his third marathon. Not only that, in the two years since he started running again, he lost 80 pounds in the process.

NY Podiatrist Finds Contentment in Rural Setting

Dr. Monty Peerhbai was working for a company out of Chicago and took care of patients in northern Indiana. He was looking for a place to open his business, and some patients suggested Monticello, NY. “I wanted to see some smaller towns,” the foot doctor explained. He took one look at Monticello and decided it was where he wanted his podiatry office. “I liked the lakes, and I liked the people,” Peerhbai said.

The 48-year-old doctor has not always been a podiatrist. About 10 years ago, he was a college professor. He chose podiatry because of his affliction ­— he once had polio. “That would be a good fit,” he explained. Peerhbai is originally from Pakistan. At age 1, he contracted polio, a disease still prevalent in that country today. The disease affected his right leg, and that’s why he now walks with a noticeable limp.

Special Olympics Georgia has designated Fit Feet for preventative foot care. This enables participants to compete in the best condition possible. The Atlanta foot doctors of Ankle and Foot Centers, Ketan Patel, Joseph D. Giovinco, and Kevin Pearson, volunteered their services to participants and coaches.

The physicians and their team of staff members, who also volunteered, conducted full evaluations and consultations. Dr. Ketan Patel has been volunteering annually at the Special Olympics Summer Games for the past several years.

On June 15 and 16, Howard Dananberg, DPM lectured at the 10th Anniversary Biomechanics Seminar from the Podiatry College at the University of Barcelona in Spain.

QUERY (CLINICAL)

Query: Skimboarder's Toe

A 38 year old female came to see me 12/2011. Three months prior, she had hurt her foot while playing volleyball. She described a hyperextension of the 1st MPJ. There was soreness both plantar and dorsal, with palpable tenderness primarily at the dorsolateral aspect of the 1st MPJ, with no tenderness upon ROM of the joint. X-rays were negative for osseous pathology. Mild overlying edema was present. Anti-inflammatory measures (oral medication, heat, local steroid injection) were administered. The patient did not return for appointed follow-up, but was seen again recently.

The same symptoms and signs remained present, with no improvement reported from the previous measures. An MRI  was ordered, and the conclusion was: "Sprain accompanied by probable tear to the lateral aspect of extensor expansion and lateral collateral ligamentous complex." The findings are compatible with so-called "skimboarder's" toe. I have searched the literature, and although the injury is adequately described, there seems to be no standard treatment. Have others had experience with this condition?

Query: E-Prescribing

I am just getting started with e-prescribing and wanted to know a few details. 1) If you prescribe a medication over the phone to a Medicare patient and document it in a patient's chart, would that qualify for one of the ten Rxs needed to avoid the Medicare 1.5% penalty? 2) Could you submit that type of encounter on a Medicare form to get credit for the ten scripts needed? 3) Would you still get credit towards your ten needed prescriptions if you e-prescribe a medication to a patient and they don't pick it up at the pharmacy?

RE: Non-Surgical Approach to Plantar Fibromas (Chuck Ross, DPM)

I inject the fibomas directly with straight (no added local) Kenalog-40 with a 20 gauge needle. I've cut down on excisional surgery by 90%. Do a proximal field block prior to injecting the steroid. I use about 0.25-0.5 cc for each mass. They typically shrink over the course of 4 weeks. They can come back, but it usually takes 3-4 years.

I have had good success fabricating biomechanical orthotics with an accommodation to off-load the site of the fibroma. In many of my patients, the lesion has all but disappeared with this therapy.

RE: Chronic Calcaneal Apophysitis (Jeffrey Kass, DPM)

The immediate concern is that there is a consolidation of the calcaneal apophysis in a 12 year old male when such a process is expected in a male at 15-17 years of age. Such a process may indicate an occult Salter Harris Fracture (Type V) or an infection, as eliciting factors that created such a premature closure of the growth plate. I am concerned by the pain elicited out of proportion with any attempt at weight-bearing and regional osteopenia as compared to the contra-lateral view in the rearfoot region.
 
My recommendation is that a neurology consult be obtained to determine underlying complex regional pain syndrome. Pain management, along with aggressive treatment of CRS/causalgia, is imperative so that the condition does not become chronically disabling.

I was pleasantly surprised by the contribution of Ms. Bartos to the case in question from a "non-podiatrist." It was well-written, concise, and overall offered many treatment options regarding the condition. I would not be particularly concerned what any MD or DO would think of her post as a point of contribution. In my opinion, the failure to hear or be receptive of ideas/inputs from all fields of health professionals indicates a "closed mindedness" that would concern me. The only apology, which I would offer, is that for a colleague in question who apparently does not exhibit, in my opinion, an inter-disciplinary respect for others.

Under the HIPAA regulations, you should have a valid Business Associate Agreement with Athena.

Once you have severed relations with Athena, and have received all the information, and can confirm that it is in usable format, you are required to request that they delete all e-PHI (electronic protected health information) that they possess that belongs to your practice. They are required to delete this information or send you a detailed response explaining why they are unable to delete this information.

If they are unable to delete the information, they are required to treat the information with the same care and security as you in order to protect patient privacy under the HIPAA regulations. Keep a copy of all correspondence with Athena on file as part of your HIPAA Security Manual.

RE: Barefoot Running More Efficient (Robert Steinberg, DPM)

The point that Dr. Steinberg initially made was, "When practiced judiciously, minimalist shoewear can help strengthen foot muscles." Just to make the point clear, only one unpublished study presented in 2005 at the ISB Congress, which was financially supported by Nike and studied a Nike shoe, showed an increase in muscle strength after wearing these shoes. Other than this one study on a relatively thick soft-soled running shoe, and financially supported by the same company that makes the shoe being studied, there is absolutely no evidence that "minimalist shoes" increase muscle strength.

Many barefoot/minimalist shoe advocates are now saying that wearing traditional running shoes "weaken feet," even though there are no studies showing this to be true. In fact, at the American College of Sports Medicine Annual Meeting a few weeks ago in San Francisco, I debated one of the nation's leading barefoot running researchers on "Barefoot vs Shod Running." One of the slides shown by this authority on barefoot running showed a patient in a neck brace with the comment being made that traditional running shoes and orthotics weaken feet just like a neck brace weakens the neck. Even though there is no research evidence to support these claims, they are often being repeated as fact by the minimalist shoe advocates, who do not seem to be hindered in making unsupported health claims about the wonderful health benefits of their favorite shoes on a regular basis. Sad, but true.
