PRACTICE MANAGEMENT TIP OF THE DAY
6 Steps to improve Your Practice Finances

Here are six specific areas you can control that will help enhance your practice finances, ensuring a healthier financial future.

3. Maintain bill collections

In addition to establishing accurate coding and billing practices, bill collecting is another critical area to be in control. Billing personnel must be well-trained in circumventing obstacles and hurdles presented by insurance companies. These obstacles include required pre-authorizations, paying less than your contract allows, and even flat-out ignoring claims. In addition, your billing staff must keep current with specific policies, coverage criteria, and billing requirements of many different insurance companies. By staying on top of the latest changes and regulations, they are better equipped to submit accurate claims and avoid potential denials or delays.

QUERIES (NON-CLINICAL)
Query: Maintaining Podiatry License After Retiring

I recently retired from podiatry and will be coming up for licensing renewal next year. Although I cannot see a scenario of returning to work in my profession, I feel compelled to keep my license active especially still being in my sixties. I would love to hear from other retired podiatrists about their opinion on this.

RESPONSES/COMMENTS (CLINICAL)
From: Robert Kornfeld, DPM

Dr. Udell's dismissal of prolotherapy as "quackery" is most unfortunate. Although I never used the dextrose solution, I have been using prolotherapy for over 20 years. I have done it on a very large number of patients over the years. When combined with a proper work-up and an unburdening of immune deficiencies, it works incredibly well. I was originally trained by an orthopedic surgeon, a physiatrist, and an anesthesiologist, who were not holistic doctors by any stretch of the imagination.

Dr. Udell, it is never a good idea to condemn a therapy when you have no experience with it. As far as quackwatch.com is concerned, it should hold absolutely no weight in the medical community unless a therapy has been proven to be harmful. I'm quite tired of the childish and uninformed bias against anything holistic or natural. I can prove otherwise after 36 years of practicing functional and regenerative medicine. My practice has thrived on patients who were not helped by traditional medicine. Does that mean traditional medicine should be considered quackery when it doesn't work and reported on quackwatch? And let's get real here. As far as "FDA-approval” is concerned, their "approved" medicines kill over 100,000 Americans every year and cause millions of side-effects. Many of their approved medicines and therapies have injured many patients. So their approval is by no means a guarantee of safety or efficacy.

You are more than welcome to stay in your traditional lane and remain closed-minded if you choose to. But those of you out there who want to have everything at your fingertips to help your patients would do well to learn functional and regenerative medicine.

Robert Kornfeld, DPM, NY, NY

RESPONSES/COMMENTS (NON-CLINICAL)
From: Dean Clement, DPM

I am also in solo private practice. I’ve used a Pod Ray orthoposer/digital capture device with A2D2 software that incorporates an X-Cel machine. I think the whole thing cost 20K, including the computer and monitor. We’ve used it since 2017. The only part I have had to replace was the trigger cord in the X-Cel component, and that was in the past year. And we upgraded the computer after 4-5 years. Image quality is good—not as good as more expensive systems, but you get what you pay for. I’m very happy with it.

RESPONSES/COMMENTS (PODIATRISTS IN THE NEWS) - PART 1
From: Ivar E. Roth, DPM, MPH

According to Jamil Hossain, DPM, "If traditional treatments for plantar fasciitis fail to provide relief, surgery may be necessary”. The article goes on to say if rest, ice, and medication are ineffective, surgery may be considered. I keep seeing these articles that are not emphasizing that podiatrists are experts in treating these problems non-surgically with great success. The tenor of this article completely misses the point that surgery is rare for these easy-to-solve common podiatric problems.

Emphasis needs to be made by podiatrists espousing their opinion that we are different from orthopedists in that surgery is often not necessary with proper biomechanical support and orthotics. We are the experts for non-surgical treatments for this problem. I believe that pushing the same dogma as our orthopedic colleagues is disingenuous to our profession. We need to say we are different and explain that to the public, not fall into the trap we treat problems like this the same way as an orthopedist would.

Ivar E. Roth, DPM, MPH, Newport Beach, CA

RESPONSES/COMMENTS (PODIATRISTS IN THE NEWS) - PART 2
From: Cosimo A. Riciardi, DPM

I found it ironic that: "Apple Cider Vinegar and Tea Tree oil Kills Fungus: NJ Podiatrist" followed by a recipe of 6 drops of oil in water and vinegar is presented the week following the closure of a poll asking "What is the biggest existential threat to podiatry?" I mean while our colleagues are discussing peer reviewed medical and surgical management, we have this gem.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Don Steinfeld, DPM

I answered last week’s question of what the greatest existential threat to podiatry is with ‘other’. My feeling, as an ‘85 graduate, is that a lack of unity among us is the key. No obstacle, including the insurance industry and the government, is insurmountable IF we act as one. Together we could wield a mighty sword.

We surely can all find reasons to complain about our leadership. But it’s our leadership. How terrible the lack of involvement of young practitioners is! Don’t miss the forest for the trees. Our profession has made spectacular progress. The APMA is our voice and our sword. My late father-in-law was a factor in optometry’s success in expanding their status and privileges versus opthalmologic opposition. This has benefited my wife who followed him in practice. How did he do it? Leadership. Organization.
