FL Podiatrist Evidence-Based Treatments of DFUs in Veterans
Some of the risks of diabetic foot ulcers (DFUs) are well-known, including infection and, in the most severe cases, amputation. VA researchers now have identified additional risks—primarily falls—heightening the urgency to implement new tools to manage and treat DFUs. Yet, that has always not been easy. “The treatment of diabetic foot ulcers is complex, and the literature indicates a wide variety of evidence-based methods to improve wound healing and prevent adverse events,” explained Latricia Allen, DPM, MPH of the James A. Haley VAMC in Tampa.

The issue is how best to treat veterans’ diabetic foot ulcers to avoid unfavorable outcomes. Dr. Allen noted options might include holistic management of the patient, including blood glucose control, improved nutrition, or management of co-morbidities. Other options could be evidence-based wound management, off-loading, or prophylactic foot surgery, adding that advanced wound care technologies (e.g., negative wound pressure therapy, growth factors, acellular tissue matrices, bio-engineered allogenic cellular therapies, hyperbaric oxygen therapies) would be appropriate for some patients.

If you go to any running or walking store and look at several different pairs of shoes, you’ll see that many of them have excessive cushioning and support. While this pillow-type padding can feel pretty amazing when you walk in these types of shoes, board-certified podiatrist and foot surgeon Dr. Bruce Pinker says they can prevent you from using certain muscle groups that can actually strengthen your body.

"A majority of people assume that foot pain is not that serious and can be ignored," says Peter Wishnie, DPM. "There are other people who have no idea that there exists preventative as well as curative care for the feet. The sad thing about it is that poorly managed foot pain could lead to surgery. However, this doesn't have to be the case because proper foot care can eliminate the need for surgery."

"There are many ways that injuries in the feet and ankles present themselves, and they come about due to different reasons. Engaging in sports is one of the main reasons. Another origin of foot problems is diabetes. Whatever injury a person has should be handled with care to prevent it from becoming worse," says Dr. Wishie.

UK Podiatrist Discusses PTTD
"One of the most common causes of adult-acquired flat feet is posterior tibial tendon dysfunction (PTTD)," explains Rebecca Gomez, a podiatrist and clinical podiatry lead at the Royal Marsden Hospital in London. "This is where one of the ligaments on the inside of the ankle that helps support the arch loses function." This can lead to instability, deformity of the foot, and degenerative changes in the surrounding joints in the form of osteoarthritis.

PTTD can be the result of overuse, or a sprained ankle which didn’t heal properly, but risk factors also include diabetes (as high blood sugar levels can damage tendons), hypertension (which reduces blood flow to the ligaments that keep the arch in place), and obesity (as the load the feet are supporting is increased).

Three Keys to Efficiently Collect from Patients - Part 2
2. Make it easy - “Anything that makes it simple and easy to pay, it a great investment,” says Laurie Morgan, a San Francisco-based senior consultant and partner with medical consulting firm Capko & Morgan. “For example, using tablets [computers] at check-in can show patients what they currently owe and give them an easy way to sign up for a payment arrangement if they need to.” Not only does this make it more likely that you’ll get paid, it shows respect for your patients in that it offers a private and less embarrassing way for them to make payment arrangements.
Some practices have had good luck keeping a credit card on file for patients. You can arrange to have a set amount charged to the card each month, especially for expensive procedures. “Patients don’t want to end up owing $2,000 at the end. This way you can charge a portion each month and offer a relatively painless way for patients to pay.”

Query: Posterior Calcaneal Spur
I have a healthy, active 73 year old female patient with chronic Achilles pain due to posterior spurring as seen on the x-ray below. All conservative care has been exhausted with the exception of custom orthotics; shoe changes with accommodation, cortisone, needling, soft tissue laser, and PT have been somewhat helpful, but the posterior spur remains the most symptomatic.

I really don’t think the orthotic will be the difference maker at this point. Before she was referred to me for soft tissue laser, her previous DPM recommended surgical removal of the spur, but required removal of the Achilles to resect it. My question is: are there any new approaches or procedures that do not require TAL resection and the associated lengthy recovery?

While I commend the new innovation of bunion surgery, I would like to know the source of the comment that traditional bunion surgery fails 70%! Having been in practice for 33 years, I have yet to see that many bunion procedures fail at that rate. If bunion surgery had that high of a failure rate, very few, if any, would be performed. Malpractice would be outrageous and hospitals, insurance plans, etc. would not allow the procedure to be performed.

I read and have been reading PM News since its inception. Many times over the years, I have disagreed with comments and assertions made by our colleagues either through an interview for a media outlet, or straight up for this platform. I generally hold my tongue and don’t comment, but this time I have to.
In the piece regarding the lapiplasty procedure, Dr. McAleer asserts that “traditional bunion surgery has a 70% recurrence rate.” I find that comment to be a lot of things including egregious, damaging, and laughable, but mostly, just wrong. Yes, of course there is an inherent recurrence rate with bunion surgery, but with today’s level of training and education, and the skill of the today’s podiatrist, the rate is more like 10%.
The lapiplasty is a good tool (that IS what it is, an expensive piece of equipment used to accomplish the Lapidus procedure), but it is NOT responsible for the reduction of recurrence; make no mistake about that. A Lapidus procedure would accomplish the same thing. But that is not the point. To simply disseminate a false piece of information is misleading and smacks of trying to increase sales of the equipment for the company that manufactures it.

In reading Dr. Zuckerman's comment on Shockwave Therapy, I disagree with his statement: "Comparing high-energy FDA-approved ESWT with the EPAT is false and misleading. They are entirely different modalities with different uses and outcomes."
EPAT produces a sound wave; it is commonly known as Radial Extracorporeal Shockwave Therapy. The old terminology would be low energy, whereas the machine is similar to the Dornier EPOS. The new terminology is Radial ESWT and Focused ESWT. They both produce sound waves. There is also no difference in the literature as to which technology works best, both produce similar results which are in general better than any other technology for soft tissue injuries.
