NJPMS Votes to Formally Affiliate with OPEIU Guild 45
 
The Office and Professional Employees International Union (OPEIU), AFL-CIO, today announced the formal affiliation of the New Jersey Podiatric Medical Society (NJPMS), who will become members of OPEIU's First National Guild of Healthcare Providers of the Lower Extremity, Guild 45. The NJPMS just completed a one-year trial affiliation with OPEIU.

The affiliation will bring 650 podiatrists into OPEIU Guild 45, which represents more than 4,600 podiatrists in various states. NJPMS is a state component of the American Podiatric Medical Association (APMA). "OPEIU Guild 45 is extremely pleased to welcome the New Jersey Podiatric Medical Society," said OPEIU Guild 45 President John Mattiacci, DPM. "This affiliation makes us stronger and better able to meet the specific challenges facing today's podiatrists.  We look forward to working together to improve standards of practice for doctors of podiatry."

Research in the the Journal of American Medical Association shows that exercise can reduce the incidence of diabetes and heart disease, but interestingly, does not result in a decrease in obesity."

Home remedies, no matter who swears they work, are more often bunk than bona fide fixes. But some actually do have science on their side and, in fact, perform even better than their traditional medical counterparts. MSN dug into the research, talked with the experts, and found 18 treatments that passed muster. Prepare to look at duct tape in a whole new way.

Docs theorize that the combination of the airtight environment and something in the adhesive is toxic to warts (but harmless to you), says Patrick DeHeer, DPM, a podiatrist and board-certified foot surgeon based in Indianapolis. In fact, 85% of warts were eliminated with the tape, compared to just 60% who had them frozen off, according to research published in the Archives of Pediatrics & Adolescent Medicine. Replace the tape every 6 days for 2 months, or until the wart disappears.

National Medicare Fraud Takedown Results in Charges Against 243 Individuals 
 
Attorney General Loretta E. Lynch and Department of Health and Human Services (HHS) Secretary Sylvia Mathews Burwell announced today a nationwide sweep led by the Medicare Fraud Strike Force in 17 districts, resulting in charges against 243 individuals, including 46 doctors, nurses, and other licensed medical professionals, for their alleged participation in Medicare fraud schemes involving approximately $712 million in false billings. In addition, the Centers for Medicare & Medicaid Services (CMS) also suspended a number of providers using its suspension authority as provided in the Affordable Care Act. This coordinated takedown is the largest in Strike Force history, both in terms of the number of defendants charged and loss amount.

I performed an Austin bunionectomy, Akin osteotomy, and tailor's bunionectomy (reverse Austin), all on the right foot.

Response: The Austin-Akin combo would be billed as a double osteotomy CPT 28299. In the professional edition of the CPT book, there are other examples of what would be considered "double" osteotomy. One of the illustrations associated with CPT 28299 clearly demonstrates an Austin/Akin. As far as the fifth metatarsal, that would be coded CPT 28308.

RE: Breach at Online Password Saving Service

On June 15, LanPass an online password storage tool reported that LastPass account email addresses, password reminders, server per user salts (a SQL uniqueidentifier - currently stored in the user table - generated automatically at insert time using default), and authentication hashes were compromised. Their report goes on to say, "We are confident that our encryption measures are sufficient to protect the vast majority of users." When it comes to HIPAA security being confident that the vast majority of users is not sufficient. When events of this type happen, it does put users at risk.
 
LanPass needs to be applauded for their fast response to this event, and the transparency in which they handled this event, but the event does underscore the increased risks we face as doctors who store sensitive patient information.
 
I recommend that you do not use third-party services to store passwords, have your browsers set to not store passwords, and have a strong password policy. Now is a good time to review your current password policy and determine if you are using any third-party services to store your usernames and passwords.

Whether or not independent practice is dead or alive is not the issue. The issue going forward is simple. Salaried docs are nothing more than employees subject to the needs and whims of management. Who, in their right mind, would put themselves in the position of acquiring enormous debt in order to be a podiatrist who can be fired at any time? What I predict we are going to see in the not too distant future are sub-par students entering medical and podiatric medical schools because the smart, high achievers will be too smart to put themselves in such a vulnerable position.

RE: DC Podiatrist Comments on Effects of High Heat and Humidity

Dr. Firestone brings up a very timely and important topic about loss of body fluids during times of high humidity and elevated outdoor temperatures. Although this can be stressful to young athletic patients, loss of body fluid during elevated outdoor temperatures can be life-threatening to elderly patients. We as podiatrists should be aware of signs of severe dehydration since we all treat geriatric patients. According to the Mayo Clinic, some of these symptoms include: shriveled and dry skin that does not bounce back when pinched, low blood pressure, rapid heart beat, fever, and if left untreated, delirium or unconsciousness.
 
In our practice, we warn our elderly patients who present on very hot days to drink more fluids and stay in air-conditioned environments. We also apprise them of some of the signs of dehydration. We also provide water for them to drink in our waiting room.
