TN Podiatrist Discusses Surgery for Plantar Fasciitis
 
According to Jamil Hossain, DPM, "If traditional treatments for plantar fasciitis fail to provide relief, surgery may be necessary to alleviate the inflammation of the plantar fascia, a thick band of tissue that connects the heel bone to the toes and top of the foot. The surgical procedure involves partially cutting the fascia to release tension and allow the inflamed area to stretch out properly, while any scar tissue around the ligament may also be removed. While surgery may not always provide a complete resolution of symptoms, it can reduce pain and repair structural damage caused by years of overuse or chronic misalignment due to poor posture or improper footwear."

"It’s essential to discuss all possible options with a doctor before deciding on a surgery for plantar fasciitis. The condition typically caused by overuse and repetitive strain of the tendon and ligaments, resulting in inflammation and sharp, stabbing pain in the heel. If conservative treatments such as rest, ice, and medication are ineffective, plantar fascia release surgery may be considered. This procedure involves cutting the tissue connected to the heel bone to release tension along the tendon and increase ankle mobility," says Dr. Hossain.

Apple Cider Vinegar + Tea Tree Oil Kills Toenail Fungus: NJ Podiatrist
 
Apple cider vinegar contains acetic acid that can kill existing spores. In fact, subjects in a study published in the International Journal of Medical Device and Adjuvant Treatments who applied the acid to their nails daily saw improvements in fungus growth within 14 days, and 93% of participants cleared the infection within 24 weeks.

To boost results, New Jersey-based podiatrist Dana Canuso, DPM, recommends adding a few drops of tea tree oil, which “inhibits the growth of new fungus, while apple cider vinegar fights fungus that’s already present.” To do: Add 6 drops of tea tree oil to 4 cups of warm water and 1 cup of apple cider vinegar. Soak feet for 15 minutes, then rinse and pat dry.

Query: Lots of Toe Fractures
 
I have a patient that suffered fractures of the 1, 2, 3 and 4th toes of the right foot. We chose to treat these closed fractures with conservative care. I did not have to reduce or re-aline the fractures, but we did splint the 1st and 2nd toes together and the 3rd and 4th toes together. I was wondering what the best option for coding this scenario.

Response: There has been controversy over whether it is fracture care or an E/M with splinting. The most accurate approach, in my opinion, is this:
 
CPT 28490 Closed treatment of fracture great toe, phalanx or phalanges; without manipulation x 1
 
CPT 28510 Closed treatment of fracture, phalanx or phalanges, other than great toe; without manipulation, each x 3
 
These codes have a 90-day global so keep that in mind as you continue to monitor. You noted no manipulation or re-alignment was needed.

QUERIES (NON-CLINICAL)

Query: Resource for Best Value in X-Ray Machines

I am looking for some assistance while shopping for a new x-ray machine for my office. I am currently in solo private practice, and I've had a poor experience with almost every in-office x-ray machine that I've come across. The image quality is not as clear as hospital/outpatient images, and the various components of the machines break down frequently. In my experience, they break every 6 months or so. The most common complaint I hear when I ask around is the cord on the digital plates going bad. My main concern is the large expense of a new machine, with the poor reliability. I would have no problem with spending $30K on a new machine that would last 20 years with minimal repairs. Is there a "Toyota" version (fair price and excellent reliability) of a podiatry x-ray?

I was hoping someone can steer me in the right direction. Are there any resources to compare the different x-ray machines/vendors for the best value in terms of price and reliability? Or perhaps this would be a good topic for a survey, blog, or article from someone with some expertise on the subject.

Injections of dextrose is one of the components used in an alternative medical therapy called prolotherapy. It is not FDA-approved nor has there been significant research showing it to be effective. RE: Issue with ACPM Bylaws Consensus Ballot

I have a problem with ACPM’s ballot to change their bylaws.
 
Requested change 1) no longer require APMA membership for renewal
Requested change 2) broaden the category of those who are eligible for fellowship beyond just ABPM diplomates.
 
Approximately two weeks ago, ACPM sent the members a ballot for bylaw changes. What they requested was a consensus vote on two significant changes to the bylaws. Having served two terms as president of this organization, albeit 20 years ago, I believe having a consensus ballot is inappropriate. I sent letters to the board and president to which no action was taken to invalidate this election to date. Therefore, I’m requesting to see if others have a problem with the two changes being requested; approved, or rejected together as opposed to being voted on individually.
