TN Podiatrist Discusses Higher Incidence of Foot Problems in Summer
Podiatrist David Franklin, DPM from East Tennessee Medical Group says he sees many more foot and ankle injuries in warmer months. “This is, of course, is mostly due to the change in footwear,” Franklin said. “What’s important to remember is that we need to wear the appropriate shoes for every activity we’re doing this summer. Playing any active sports in flip-flops or sandals definitely should be avoided as it can be a direct cause of sprains, fractures, and stress-related injuries,” he explained.

“Skin infections also occur more frequently during the summer,” Dr. Franklin said. “These usually are secondary to a trauma, but also can be the result of a skin fungus. Heel pain also is quite common during warmer months, again a result of the footwear we choose, but I also see issues such as ingrown toenails and plantar warts on a daily basis during the summer. Also, feet tend to get ignored when we’re using sunscreen, so remember to apply some to your feet, as well, to avoid sunburns.”

But, sometimes tunnel vision conquers will-power and physicians will regrettably borrow from their 401 (k). Not only does borrowing against your retirement account postpone savings goals (which are already more difficult for physicians), but brings a potential tax-burden and possible penalty fees to the table.

Are there published guidelines one should follow as practices re-open? For example, chairs separated by 6 feet in waiting room, having patient wait in car until called, one family member to accompany patient, Plexiglas sneeze shields on office counter tops, co-pay by credit card only, handwipes available, mask plus plastic face shield, etc.?
Are ultraviolet light room sanitizers an effective way to prep an exam room? Recommended brand, model #, cost?

RE: High-frequency Spinal Cord Stimulation Eases Painful Diabetic Neuropathy

Painful diabetic neuropathy that doesn’t resolve with standard treatment may have relief of pain and improved sensation using a 10-kHz spinal cord stimulation device (Nevro Corp.) as reported in a large randomized controlled study. Within 3 months after starting treatment, 79% of patients had substantial pain relief compared with 5% of patients managed with conventional medical treatment in the largest-ever randomized, controlled trial of spinal cord stimulation for managing painful diabetic neuropathy. The median age of patients was about 61 years and roughly two-thirds were male. All subjects had lower extremity pain.
Natalie H. Strand, MD, assistant professor of pain medicine at Mayo Clinic, Scottsdale, AZ., said that while the findings of this randomized study may require corroboration, they do suggest that this neuromodulation device may provide another option for frontline diabetes providers when patients have persistent pain despite appropriately medication management. Study results were presented as a late-breaking poster presentation at the 2020 virtual annual scientific sessions of the American Diabetes Association. (Family Practice News, June 14, 2020)

In addition to revising the full forgiveness application, the SBA also published a new “EZ” version of the forgiveness application applying to borrowers who:

-
Did not reduce the salaries or wages of their employees by more than 25%, and did not reduce the number or hours of their employees; OR
-
Experienced reductions in business activity as a result of health directives related to COVID-19 and did not reduce the salaries or wages of their employees by more than 25%.
