MO Podiatrist Performs New Bunion Procedure
 
A podiatrist with the Jefferson City Medical Group is the first doctor in Missouri to use a "game-changing" procedure to correct bunions. Dr. J.P. McAleer has been using the lapiplastly correction procedure for about a year and is working to bring the procedure to other podiatrists throughout the state and country. Traditional bunion surgery has a 70 percent rate of re-occurrence, and McAleer said out of the 100 plus patients the group has treated, none of them have had re-occurring bunions.

McAleer has been traveling throughout Missouri and to other states to teach other doctors how to perform this surgery. He is one of the first doctors in the United States to adopt lapiplasty. He said one of the benefits of the procedure is that it is reproducible, whereas traditional bunion correction varies case-to-case.

He most recently worked at Tséhootsooí Medical Center in Fort Defiance, AZ.

Aussie Podiatrist Opens Ingrown Toenail Clinic
 
Brindabella Podiatry in Tuggeranong has officially opened a specialist clinic to treat ingrown toenails, a common and painful problem for many Canberrans. Brindabella Podiatry owner Nicole Hart said the clinic regularly saw as many as 15 patients a day with ingrown toenails, one patient only seven months old.

The new, stand-alone surgery suite will allow the podiatrists to treat patients with the ailment in a separate surgical suite. The treatment options are self-management, non-surgical intervention, or surgery with local anesthetics. She said that the overwhelming majority of ingrown toenails could be treated successfully and new advances in treatment meant no discomfort during or after the procedure.

IL Podiatrist Recommends Best Summer Footwear
 
"Flip-flops are my least favorite type of shoes worn in the summer," said Sarah Dickey, DPM, president-elect of the Illinois Podiatric Medical Association and a podiatrist at Loyola University Health System. "These provide zero structural support or protection for your feet." FitFlops and Birkenstock-type shoes are good alternatives, Dr. Dickey said, because they have sturdy soles that don't allow bending.

Dickey recommends choosing sandals that are breathable, have arch support, a rigid or sturdy sole, and straps that secure the sandal to your foot in at least two spots. Sandals with top-of-the-foot and heel straps help stabilize the foot on the foot bed of the shoe. This eliminates excessive motion, which helps prevent overuse injuries of the foot and ankle, she said.

PRACTICE MANAGEMENT TIP OF THE DAY
Three Keys to Efficiently Collect from Patients - Part 1
 
1. Make it Clear. - You’ll have a much easier time collecting what patients owe you if you make your payment policies crystal clear from the beginning. If you collect co-pays and other fees at the time of service (and it’s highly recommended that you do), don’t wait until checkout to explain this to patients. If you don’t make new appointments until all outstanding fees are paid, inform patients of this as well. State clearly when you will turn over payments due to collection agencies, and other similar policies.
 
“This information is a part of our new patient info pack,” says Alina Collins, office manager at First Coast Family Medicine in Jacksonville, FL. “Patients sign an agreement at the very beginning, so they know what we expect.” Most patients want to pay their medical bills; it’s easier for them to do so if they know right from the start what is expected of them.

Query: Multiple HTs with digital amputation coding
 
Would the proper way to code right 2nd digit amputation with right 3rd and 4th digit arthroplasty be: CPT 28820 – T6 , CPT 28285 – 59 -T7, PT 28285 – 59 – T8 (List highest RVU first and then List the lesser RVUs with 59 modifier)?

Response: It depends on who the payer is. If it is a non-Medicare payer, you have to check to see if they have their own coding edits. If they do, you should follow them. If they do not, then I suggest coding exactly the way you listed in your post. CPT 28820 has a higher work RVU than CPT 28285.
 
However, it is very different if Medicare is the payer because Medicare has their own CCI edits. Every time you have multiple procedures on a Medicare patient, you must check the CCI edits to ensure proper coding. This is an excellent example, because there is no Medicare CCI edit between CPT 28820 and CPT 28285. Furthermore, there is also no CCI edit between CPT 28285 and CPT 28285 (itself!) That means if this is a Medicare patient, you do not need any 59 (or X) modifiers. The -T modifiers are appropriate regardless of who the payer is.

RE: Using an Orange Light to Better Visualize Lesions

Skin cancer on the feet can result in death. In some patients, a parallel ridge pattern on the dermatoglyphic areas of the feet indicates a high probability of skin cancer. Sometimes, examination under a polarized or non-polarized white light just might not provide a clear diagnosis.
 
I discovered that an orange light may be helpful in distinguishing between the parallel furrow and parallel ridge patterns. I believe other colors might also give similar, if not improved, ability to give a more accurate diagnosis.

I was in Costco on Saturday, June 16, 2018, and realized they could save tons of money by getting rid of pharmacists and optometrists! Anyone can read a prescription, count pills, and make people better, right? Digital devices can scan the eye and come pretty close to correcting vision and improving eyesight, right? Of course, they are not going to do that, but they do invite into their stores, people with no licences to advise people that they can make their feet feel better by standing on a mat and have the bottom of the feet mapped. Then produce devices that may cause injury to the foot, ankle, knee, and low back.
 
So, what if it has a 90-day guarantee?  The damage from devices like these might not show up for months. If you want to try something quick and easy, and inexpensive for foot pain, pick up a pair of rather stiff insoles at a sporting goods store. If they don't work, you're only out $35-$40 bucks, not the $130 Costco charges for their ridiculous insoles. If you have foot or ankle pain, you deserve to be seen by a licensed expert, a doctor of podiatric medicine and surgery.
