Knock-Off Crocs are an Improvement for Inmates: VA Podiatrist

The Virginia Beach Correctional Center, in November 2010, started selling a knock-off version of Crocs - in neon orange to match inmates' jumpsuits - and they've become one of the most popular items purchased through the jail's canteen, officials said. In the last year, inmates bought 215 pairs at $15.99 apiece, said Capt. Rocky Holcomb, commanding officer of corrections.

Dr. Arnold Beresh, a Hampton podiatrist who has treated local inmates, said the clogs appear to be an improvement over the other footwear available in jails, though the knockoffs don't always provide enough heel and ankle support and tend to be warm, which can cause athlete's foot.

When Randy Munoz began working with the Latino Diabetes Assn., it was an idea on paper. The plan was to go into some of the county's poorest communities and provide free classes in Spanish on yoga, healthy cooking, preventing diabetes and how to live with it.

KFx Medical Gets FDA 510 Clearance for Smaller AppianFX Tissue Fixation Anchor

KFx Medical Corp. announced it has received 510K clearance from the Food and Drug Administration (FDA) for its 5mm AppianFx Tissue Fixation Anchor. KFx said that the AppianFx line of implants re-attach tissue to bone in shoulder, knee, foot and ankle procedures, which combined, exceed well over one million annual surgical procedures. Product offerings include those that directly place and secure tissue into bone, both with and without the use of sutures.

Hospital-Owned Practices to Receive Lower Medicare Rates Under New Rule

Physicians and practice managers are not ready to comply with a July 1 deadline for a new Medicare billing policy that would pay lower rates for certain office visits and other services in facility-owned physician practices, associations representing doctors and administrators have told government officials.
 
Physician offices wholly owned or operated by hospitals will be required starting on that date to change their coding practices under a regulation from the Centers for Medicare & Medicaid Services. The policy requires a hospital and its entities providing outpatient services to coordinate billing efforts when patients are admitted to the hospital or soon afterward.

This is an interesting dilemma we all face from time to time. While I can not see the base on the film and I can not know the condition of the cartilage, nor can I appreciate the quality/quantity of the ROM, I am none-the-less predisposed to offer an opinion.
1. If the cartilage is poor, just do the MJP arthrodesis ... The IMA will reduce enough. (Dayton, et al. JFAS 2002 September-October; 41(5):316-9.
2. If the cartilage is intact, then one could do an Austin with fibular sesamoidectomy (Gerbert, et al. JFAS Vol 36 issue 5 September 1997, Ravenell, et al. JFAS 2011 May-June ;50(3):287-92).
3. The dislocated 2nd MPJ is also an issue.
4. In any event ... MPJ fusion with closing base wedge is not a great idea!

You don't want to 'out-smart yourself', but if you think it's a true case of Sever's disease, start aggressive PT, NSAIDs, orthotics for any biomechanical pathology. Looking at the lateral radiograph of the left heel, the growth plate actually has more sclerosis than the right. Secondly, re-examine your other x-ray views (AP, obliques). There is something that looks pathologic around the talus and STJ.

X-rays show more closure of the epipheseal plate, right side, and a hint of decreased bone density. RSD is possible - Pain ALL the time, way out of proportion to injury. Linear scleraderm deep scarring, fibrosis, lots of pain. Both of these have essentially a clinical diagnosis. Scleraderma may have tissue pathology in the skin if a scar forms. I hope it's just re-injury apophysitis!

This makes me so sad to hear. I wish that you had had a better experience. I can tell you that you are not alone, BUT there are good people out there who will look out for you. After a couple of false starts myself, I am thrilled to say I am in a wonderful situation, practice wise, where everyone looks out for everyone else, the egos are put aside for the betterment of the patient and the practice. It's a wonderful breath of fresh air for me and a situation I feel lucky to have landed myself in after almost 10 years of struggle.

Please find a place for yourself that you will be happy in. Life is too short to be miserable. You're right. You've worked too hard to be treated that way. If you need any guidance or just someone to talk to, please feel free to e-mail me anytime.

Dr. Sugar requested a substitute for the snap fixation pins.  I like the Tornier Nexfix Compression Pins: (tornier-us.com/upper/hand/hantra001/index.php). This product has been marketed to hand surgeons, but is ideal for all small bone work. This pin has a variable pitch thread with better compression than standard threaded K-wires. They need to be cut off at the bone surface with a pin cutter.  The pin cutter supplied by the maufacturer of the product allows one to cut the pins almost flush with the surface.

I have read all of the recent threads concerning our post-graduate training. It really is not that complicated. No one can have too much training. Just because your residency ends, it does not mean that your training ends. In fact, it is really only starting. As the expertise of our profession grows, it is hard to acquire all the knowledge needed to be the best foot and ankle doc in just 3 years. There are fellowships, workshops, lectures, and more for all of us, at all levels of our careers. Also, not everyone has to have the same skills set. One just needs to be the best in the area one wishes to function in, be it trauma reconstruction, wound care, or biomechanics. Knowledge is power.

RE: Barefoot Running More Efficient (Kevin Kirby, DPM, Robert Steinberg, DPM)

Drs. Kirby and Dr. Steinberg come to the defense of traditional structured running shoes as if I am condemning them. I am not addressing standard running footwear, nor am I addressing the effects of orthotics. Rather, I am citing literature, as well as pointing out a growing trend that supports that minimalist shoes can be safely worn for running for many people. Indeed, as I have posted, some literature supports the benefits of wearing minimalist shoes, both in terms of decreased total oxygen consumed during running and an increase in the cross-sectional area of the intrinsic foot muscles after running in minimalist shoes for a period to time.

I understand Dr. Steinberg's concern that he sees runners who develop musculoskeletal issues after running in minimalist shoes, but some runners also develop issues running in structured shoes. And podiatry patients are pre-selected for biomechanical issues, which is why they present to the office so they may not be an accurate cohort from which to draw global conclusions.

Many runners (not all) can acclimate themselves to minimalist shoes and we should 1) not rule out this type of footwear and 2) understand that minimalist footwear for running is likely here to stay.
