NY Podiatrist Named County Commissioner of Health
 
Orange County Executive Steven M. Neuhaus announced on Thursday that Irina Gelman, DPM, MPH, PhDc has been named the County’s next Commissioner of Health. Dr. Gelman received her Doctorate of Podiatric Medicine from the New York College of Podiatric Medicine in 2010 and completed her residency at Coney Island Hospital in 2014. She earned her Masters of Public Health in 2012 from Nova Southeastern College of Osteopathic Medicine in Fort Lauderdale, FL.

Dr. Gelman is currently working on her PhD at Walden University. With a concentration on community health, Dr. Gelman’s dissertation will focus on telehealth initiatives in rural areas.

TX Podiatrist Discusses Summer Footwear
 
Podiatrist Jessica Rutstein, DPM of TSAOG Orthopaedics in San Antonio says flip-flops are fantastic, if your goal is serious damage to your feet, and your posture. First of all, she says your heels may not survive a summer in flip-flops. "Patients using very flat, very inexpensive flip-flops and they get plantar fasciitis for heel pain and arch pain, because they are wearing something that has absolutely no support whatsoever," Dr. Rutstein said.

And that's just the start. She says your toes are forced to make unnatural moves to keep the flip-flops on your feet when you are walking, and that leads to painful injuries. "Not only hammertoes, but people get tendonitis in the front of their ankle and on the top of their foot, and that takes ten to fourteen days sometimes to resolve." She says if you want an open-toed shoe for the summertime, go with a Birkenstock or some sandal that has a support strap behind the ankle. Or, stick with reliable tennis shoes.

NJ Podiatrists Among Six-Figure Earners
 
One of the benchmarks along the way to making it big is earning a six-figure salary. While six figures isn't much when you're living in the New York-New Jersey area, where cost of living is among the highest in the nation, breaching that $100,000 number in New Jersey is a lot easier and more likely if you have one of 95 jobs that pay as much, according to data from the Bureau of Labor Statistics. 
 
Some fields tend to yield higher pay, with four of the top five highest-earning jobs in the medical and health field. New Jersey podiatrists, for example, average $137,160.

Query: Locum Tenens
 
Can a podiatrist who has opted out of all insurances work as a locum tenens in a practice on the days that the owner is gone. The owner of the practice will still work one day a week. If so, can the locum tenens bill under the podiatrist who owns the practice.

Editor's comment: PM News does not provide legal advice. Your question has two parts 1) does the locum tenens physician need to accept insurances? and 2) can the hiring doctor still work in the practice? Since the policies of insurance companies vary, this response will only be in reference to Medicare. 1) Under Medicare, a locum tenens physician can work up to 60 days in place of a physician. While the locum tenens doctor must be registered with Medicare, there is no rule requiring the locum tenens to accept insurance. It is irrelevant, since billing is done under the owner's Medicare number and the locum is paid a negotiated salary by the owner.
 
2) The owner of the practice cannot work at the practice while employing a locum tenens and a locum tenens physician cannot be used to cover expansion or growth in a practice.

Dr. Robert Kaplan died of end-stage renal disease caused by amyloidosis. He graduated from the California College of Podiatric Medicine in San Francisco and completed 2 years of residency in N.J. and Oakland, after which he began his practice of podiatry on Pill Hill in Oakland. He was a lifetime member of the California Podiatry Association and served as president of his local chapter.

Dr. Bernard Spear, 94, of Bristol died on Wednesday, June 13, 2018, in Bristol after a short illness. Dr. Spear served in the United States Army in France during World War II. He was a graduate of the New York College of Podiatric Medicine. He loved his work as a podiatrist in Bristol where he practiced for 45 years.

I am writing to clear up the misinformation that EPAT technology is similar or the same as extracoporeal shock wave therapy (ESWT).  EPAT is FDA class one cleared and described as a vibration device.  Class 1 requires no medical evidence. This clearance does not indicate that EPAT uses sound waves or shock waves. If there were shock waves used in the EPAT technology, it would have to undergo Class 3 FDA criteria and regulations.  
 
ESWT uses shock wave technology, which requires double-blind, randomized, placebo controlled, and multiple center studies to obtain FDA approval. The Dornier Epos Ultra has been proven effective with the highest level of medical evidence criteria. The Dornier is the only FDA-Approved Class 3 (highest level) device that meets all five criteria with the Blue Cross Blue Shield tech report, and it is the false claims of lesser devices that created the insurance coverage withdrawal. 
 
Dornier invented ESWT as well as kidney stone lithotripsy. Comparing high-energy FDA-approved ESWT with the EPAT is false and misleading. They are entirely different modalities with different uses and outcomes.

The most important consideration in footwear selection in this case is to accommodate the lateral column. Most shoes become narrower at the midfoot, both medially and laterally. In this case, it increases pressure and shear both laterally and plantarly. Certain styles have a broader base of support inside and outside the shoe. Birkenstock has the right shape for met adductus. Finn Comfort and New Balance SL2 last (#1540, #928 are two styles in that last) may relieve shear and pressure to the base of the 5th. A new footwear option that has a broader shank area is KyBoot. 
 
Almost no shoe fully accommodates the styloid process at the base of the 5th in this foot. A shoe modification in which the lateral column inside the shoe is opened up and extended laterally is the best option since it can be done to most shoes.

I understand that these are nothing but drafts at this moment. I reviewed the diabetic therapeutic shoe template documentation requirements. All I have to say is that this is ridiculous and an inordinate amount of paperwork for something that is duly needed to prevent DFUs in the diabetic. In CMS’s great wisdom, they believe this will ensure that all the necessary documentation will be proper and correct. I opine that this will just prevent DME suppliers (DPMs) from ever dispensing shoes. The last go around of documentation requirements and continual audits have certainly lessened the number of shoes being dispensed by this profession in light of the surety bond and DME supplier certification exemption brought about by the APMA.
 
Unfortunately, we are on the front line of this issue and it is sad that this documentation template requirement will eventually stifle our ability to do what’s best for our diabetic patients. The APMA will need to step up again and try to get involved with this ill-sighted effort created by CMS DME consultants and not by DME suppliers/physicians as it should have been. This is a typical response from the federal government. This is not going to work the way they think it will; all in the name of saving money and decreasing audits. I would like to know how they think this will solve the problem at-large. Interestingly, DPMs only make up about 3% of the DME suppliers and we are being penalized because of the other 97%.
