NY Podiatrist Discusses Ganglionic Cysts
 
According to Douglas Tumen, DPM of Hudson Valley Foot Associates," a ganglionic cyst is a soft tissue fluid-filled cyst. Picture a balloon. You fill it with water and it starts getting bigger. The tendon right over the big toe joint is a common place to get these ganglions. These are filled with a viscous gelatinous fluid.

"It's usually not painful unless it's pressing on a structure. Usually they are freely movable. After local anesthesia, the ganglion can be punctured and drained. They usually resolve, but one in five will return,” says Dr. Tumen

UK Podiatry Team has Helped Hundreds in Lockdown 
 
Lockdown may have closed most of the University of Northampton's buildings, but one health team remained on call providing a vital, life-saving service for the community. The podiatry clinic is situated next door to Northampton General Hospital and doesn’t know the meaning of a ‘quiet day’. During business-as-usual periods, upwards of 100 patients can be seen each day by the team, supported by UON podiatry students. On average, 20 people receive a consultation each hour.

Lockdown meant the clinic closed its doors for general appointments, but part of the team remained on-call for patients. They either gave advice remotely or came into the clinic to provide direct care to those deemed at high risk of developing more serious complications and who required urgent treatment.

Study Examines Role of ACA on DFU Amputation Rates in People of Color 
 
Every 20 seconds a limb is lost due to diabetes, worldwide. In the United States, as in many other parts of the world, this occurs at least twice as frequently in people of color. At the American Diabetes Association 80th Annual symposium, investigators from the University of Arizona College of Medicine and the Keck School of Medicine of University of Southern California, evaluated the role of the Affordable Care Act (ACA) in minority patients being admitted to hospital with diabetic foot ulcers. 
 
“We show, for the first time, a substantial difference in major amputation between states that adopted ACA and those that didn’t,” noted Tze Woei Tan, Associate Professor of Surgery at the University of Arizona and the study’s lead author. “There was a 1% increase in states that didn’t expand Medicaid and a dramatic 17% reduction in major amputation among those that did.” The reasons for this are likely manifold, but the authors believe some of it may have to do with access to care outside of the hospital. “This study showed us something really fascinating,” noted David G. Armstrong, Professor of Surgery at Keck School of Medicine of University of Southern California and the study’s senior author. “People of color with wounds were more than 20 percent less likely to be admitted to hospital in states that expanded Medicaid than before expansion. This is compared to a 78% increase in hospitalization among non-adopter states.”

Query: Is Same and Similar Like This Across All Jurisdictions?
 
I am curious to see if Medicare DME claims are being treated the same across other jurisdictions. I have seen few payments for any “off the shelf” boots, braces, or even custom items. I see near immediate denials only to be informed that a “same and similar” device was dispensed in the past 5 years. Appeals and re-determinations seem to fall on deaf ears and the time it takes to appeal these claims is becoming more cumbersome, even with their online submission process. It is nearly impossible to use a temporary fitting walking boot or simple brace while waiting for a more permanent option (i.e. custom brace or other device) without receiving a denial.

Response: This scenario is exactly why APMA, AOPA, PFA, AOTA, APTA and others including AAOAS AOFAS have been petitioning CMS to get this 5 year rule changed. Every supplier should be enrolled into all four DME portals, and a same/similar (S/S) check should be run prior to dispensing anything. Again, the search for S/S should be run in at least one DME MAC in JA or JD and again in JB or JC. If these are clean or hit something, you must print them and save them. Then prepare your dispensing note to say why the previous dispensed device is inadequate (e.g. Cam walker dispensed two years ago for left ankle sprain is inappropriate for a patient requiring a custom AFO with greater deformity for PTTD and the custom device is required because the patient needs to be treated in all three planes and has a new diagnosis of…). Then, the previous device is inappropriate and places a clear and present danger to the patient. You can then appeal your initial claim rejection as a re-determination through the same portal. 
 
It’s interesting that MCR chooses not to share S/S info via the provider portal, but does so in their claims processing. Unfortunately, the rules are not fair and are slanted in favor of the carrier (what else is new). If you wish to proceed with a new device during a global period (5 years is absurd), be sure you properly complete an ABN. These are very tricky and must be detailed. I urge you to Google “How to Properly prepare an ABN.” Not only should you review those provided by CMS, but those of others in the DME industry.

RE: The Passing of Stanford Rosen, DPM

At the academy, we were very saddened to learn of the passing of our good friend Dr. Stan Rosen of Tuscaloosa, Alabama. Dr. Rosen was one of the original founders of The Academy of Ambulatory Foot and Ankle Surgery (now the Academy of Minimally Invasive Foot and ankle Surgery). Dr. Rosen, besides being an innovator in several MIS foot and ankle procedures, served many functions for the academy including terms as President and as Executive Director for many years. 

One of Dr. Rosen's lasting legacies was the LSU 3-day lecture/cadaver course which continues to this day. Dr. Rosen's always smiling face, outgoing personality, and willingness to help and instruct fellow MIS foot and ankle surgeons will be sorely missed. Our heartfelt condolences go out to his wife Joanie and daughter Priscilla.

I arrived in AL in 1973, just out of residency. Soon after, Stan Rosen befriended me, and took me to a Birmingham MIS meeting hosted by Abe Plon and Al Brown. Stan Rosen and the Academy CHANGED MY LIFE! Six months into practice, I became dismayed by the open traditional hackery that caused such pain and disability. I thought that there must be a better way. I found it under the guidance of Stan and the Academy.
 
Stan was my Earl Nightingale, always encouraging, a patient teacher, and giver of himself, sometimes to his own detriment. Thanks Stan, best regards to Joni and the girls. See you up there buddy!

Is the second toe long or is the metatarsal parabola the anatomical cause? If the 2nd metatarsal is long (or there is a short 1st met), as opposed to the 2nd toe being long, you may need to shorten the metatarsal, which I believe is the CPT 28308 code.
