PODIATRISTS AND SPORTS MEDICINE
Achilles Tendon Tear Does Not Have to End Career: IL Podiatrist

It’s unlikely Kevin Durant will perform at the same level he did before rupturing his Achilles tendon during the NBA Finals, according to medical experts and academic research. The rehabilitation will be rigorous and take between nine and 12 months. Durant can expect to wear a cast or walking boot for six to 12 weeks. While the Achilles tendon in his right leg heals, his leg muscle will weaken. But it is possible for him to return as a productive player.

Dr. Lowell Weil

Lowell Weil, DPM, a team podiatrist for the Chicago White Sox and a consultant for the Chicago Bulls, pointed to NFL linebacker Terrell Suggs as an example of what’s possible. Suggs, a seven-time Pro Bowler, in 2012 returned to play and registered a sack just five months after surgery for a partial tear of his right Achilles tendon. “So you can still be extremely explosive, and he’s such a talented athlete,’’ Dr. Weil said of Durant. “If he works hard in his recovery, there’s no reason to say he won’t be able to get back to the pinnacle of the profession.’’

INTERNATIONAL PODIATRISTS AND FOOTWEAR
Aussie Podiatrist Provides Rules for Buying Healthy Footwear

There are some simple rules to follow when shopping for healthy footwear. “The two main issues with people’s footwear are poor fit and heel elevation,” says Dr. Hylton Menz, a podiatrist and professor of biomechanics at La Trobe University in Australia. Some of Menz’s research on older adults shows that too-short or too-narrow shoes can lead to foot deformities and increased risk for weakness and falls, as well as growths like bunions, corns, and calluses.

Dr. Hylton Menz

The healthiest footwear for both older and younger adults should fit well and have a low, broad heel, a thin and flexible sole, and some kind of lace, strap or Velcro to ensure the shoe stays firmly attached to the foot, he says.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Thomas J. Riess, DPM

Dr. Thomas J. Riess Of Marin County, died of Parkinson's related complications on Sunday, June 9, 2019. He was 72 and had valiantly fought against Parkinson's for more than half of his adult life.

Dr. Thomas Riess

By profession, he was a podiatrist, following in the footsteps of his father and practicing together in San Francisco and San Rafael, CA. He was interviewed on NPR and patented headgear with engineered visual cues which projected red lines to help individuals with Parkinson's walk.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Don Peacock, DPM, MS

These patient often do well without bone work. Most of these patients have gastrocnemius equinus and can get significant relief in mid-foot arthritic pain by performing a gastrocnemius recession. There will be reduction in biomechanical stress in the mid-foot. Look at some of Dr. Monroe LaBorde's research on gastrocnemius recession.

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Christopher Hood, Jr, DPM

Coincidentally, I will be lecturing on this topic at the APMA The National 2019 in Salt Lake City on July 12th during the “Surgical Blitz” series. Because of this, I can at least offer a literature based response.

When you look at the literature with respect to lateral column perceived arthritis in conjunction with medial and middle column arthritis and the treatment options, it tends to lean towards either no intervention or arthroplasty. It is common opinion that a fusion should not be performed either in...

RESPONSES/COMMENTS (BILLING & CODING)
RE: Your Claims Could be Deemed Unprocessable: RT LT Modifiers
From: Joseph Borreggine, DPM

The DME MACs issued a "Correct Coding - RT and LT Modifier Usage Change" joint publication on December 6, 2018, which provided instructions for billing right (RT) and left (LT) modifiers on separate claim lines. This is applicable when two of the same items or accessories (same HCPCS codes) are provided on the same date of service (DOS) and the items are being used bilaterally, and was effective for DOS on or after 3/1/19. These instructions have now been changed in several policy articles.

Bilateral items not billed on separate claim lines will be returned as unprocessable. Claims returned as unprocessable must be re-submitted. Unprocessable claims do not have appeal rights and cannot be re-opened or submitted for adjustment. Between May 29th and June 6th, 16.6% of JB claims would have been rejected with the outdated instructions, so be sure to review them.

Click here for the full list of HCPCS codes requiring RT and LT modifiers and impacted by the updated policy articles.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Dennis Shavelson, DPM

There are potential patients in all neighborhoods (different percentages) of varied means, needs, and levels of foot problems. Here is my top ten suggestions list:

1. Consider the concierge model.
2. Emphasize low volume (with more time per patient).
3. Charge high fees (comenserate with the higher level of service provided).
4. Create a hi-tech and high end environment.
5. Carry unique, quality cash products at various price points for immediate sale.
6. Be well versed, expert, and well practiced in 3 or more non-surgical “centers of excellence”. Make at least two of them “non-covered” by insurance.
7. Practice the wellness model, not the insurance/government model.
8. Hire staff capable of collecting cash.
9. Your staff must believe that your care is worthy of a cash patient.
10. Get a mentor-partner to educate, monitor, and adapt the practice until you are successful.
