Flip-Flops More Dangerous Than Traditional Sandals: DC Podiatrist
“Flip-flops are more dangerous than traditional sandals and traditional shoes for a number of reasons,” says Dr. Lee Firestone, a podiatrist with Foot and Ankle Specialists of the Mid-Atlantic. He says these flimsy minimalist sandals leave the toes especially exposed to injury — the kinds of injuries he sees most Mondays at his practice in the Washington, DC area.

|
Dr. Lee Firestone |
“I find that people come in on Monday morning with a toenail that is half on and half off because they have caught it on something over the weekend,” he says, adding “they have layers of tape and Band-Aids holding the toenail in place, with a level of desperation on their faces.”

Bret Ribotsky: In accessing patients with heel pain, which treatments will work best?
|
|
Dr. Stephen Barrett |
Stephen Barrett: Staging and grading with diagnostic ultrasound is the key today. A study that I published involving 100 patients with and without heel pain gave us some data. There were three zones noted. "Zone A" is anterior and center of the plantar fascia. Zone B is anterior to the calcaneus. Under the calcaneus is classified as "Zone C." You can look at the symptomatic limb vs. the other leg to help determine what's going on. If "Zone B" is thicker than 4mm, you can almost be assured that there is a component of plantar fasciopathy. Those who measure 7mm+ are not going to respond to night splints; thus, staging and grading are very important.

Time Limits for FCA Cases Stand Under Supreme Court Decision
Hospitals and other healthcare organizations no longer have to worry that False Claims Act (FCA) lawsuits will hit very old claims because the U.S. Supreme Court ruled on May 26 that the suspension of the statute of limitations for certain fraud laws doesn’t apply. In the final word on the subject, the highest court in the land said the Wartime Suspension of Limitations Act (WSLA), which still is in effect, applies to only criminal statutes, not civil statutes such as the FCA. The WSLA suspends the statute of limitations for fraud perpetrated against the federal government, the court noted.
Washington, D.C., attorney Jesse Witten says the 9-0 Supreme Court decision restores “common sense” to the statute of limitations under the FCA. An FCA case may be brought (1) six years from the date of the violation, or (2) three years from the date of discovery of the violation, but not longer than a decade after the violation, he says.

5 Ways Providers Can Improve Marketing in the Mobile Age - Part 3
3. Solve for mobile.
Mobile devices have triggered local searches across multiple industries. According to Google, the number of "near me" searches has increased 34 times since 2011, and 80 percent of those searches came from mobile devices. It is essential that your content be visible on mobile devices, especially in light of the latest Google algorithm update that favors mobile-friendly sites in search results.

Query: Practicing in the U.S. Territories
Can someone please provide information on how a DPM in the states can practice podiatry in the U.S. territories such as the U.S. Virgin Islands?

Query: Incentives for Dispensing DME
Does anyone have recommendations about employee incentives for dispensing DME such as braces and diabetic shoes, as well as other non-insurance-covered products? The back office staff is more involved in the dispensing. Should front office staff receive incentives as well?

| CODINGLINE CORNER | |
Query: Osteogenesis Stimulator
Has anyone ever billed for the application or fitting of an osteogenesis stimulator? The manufacturer bills for the actual device (E0747), but their insurance department reps won't tell us what (if any) code there is for the fitting of the device done in our office. I'm at a loss. I've seen a couple of application codes, but they seem more like application of external fixators.
Jennifer P., Billing Specialist, Memphis, TN
Response: There are a few codes to select from: CPT 20974-20979 depending on whether it is an electrical or ultrasound bone stimulator used. The coding differs. However, often times the reps come into our office at a pre-arranged time and do the fitting and training while the doctor is elsewhere, or otherwise seeing patients; so since they did everything, there is no actual professional component for what you did. You did not provide or add any service relative to the dispensing, fitting, and instruction. If you DID provide additional services, that is another matter.
So, in the vast majority of such instances, there is nothing for you (the provider) to bill. There is not necessarily an exam performed, just the application of the unit by the rep. It just happens to have occurred in your office.

| RESPONSES/COMMENTS (CLINICAL) | |
From: William Greco, DPM
Here is a solution that I have used in the past. Apply a Steri-Strip with benzoin to protect the skin. Direct the force to pull the distal skin to (de-rotate) the skin in a distal plantar direction. This will work similar to serial casting as the skin in very moldable in the infant and may take several months of application. I have done it weekly or when the Steri-Strip fell off. You can teach the parents to apply this if you find them competent at the task.

From: Brian Kashan, DPM
I have been advertising with the Yellow Pages for 30 years. The original ROI (return on investment) was about 15-1. It was great. 30 new patients a month and reasonable costs. Well, more books came out from more companies, and the investment had to be split between books. It became difficult to track.
We tend to throw money at things if they appear to be working or have worked for us in the past. With the Yellow Pages, we were always told by the reps that you will lose your place...

From: David Secord, DPM
I appreciate Dr. Deutsch’s commentary on governmental inefficiency and bloat. The vitriol flowing within the text is well deserved.
It might be pointed out that we live in a republic, not a democracy and, at best, we have a representative democracy in Washington, DC, as we elect individuals to represent our views and interests at the nation’s capital and are usually disappointed with the results, as lobbyists and special interests hold sway in this realm of the professional politician. Both term limits and a Constitutional Amendment for a balanced budget would go a long way towards amending some of the problems in DC.
Nevertheless, I have no delusions that EMR usage, ...
