KSUCPM Graduates Class of 2023
 
Kent State University College of Podiatric Medicine (KSUCPM) has graduated another class of highly educated doctors of podiatric medicine out into the world to treat and heal. Friday, May 19 saw 98 doctors walk the stage in Kent, Ohio and into their new lives as podiatric physicians.

The class of 2023 had a 100% match rate, and placed in 62 hospitals across the country. 20% of the class will be specializing in Ohio, with other top states being Illinois, Michigan, New York, and Pennsylvania.

UK Podiatrist Explains Why Feet Swell in Hot Weather
 
Here’s something  to add to the list of heatwave problems – swollen ankles and feet. According to Emma McConnachie, podiatrist from the College of Podiatry, the reason we get swollen ankles and feet is all to do with the blood supply. She explains: "As our bodies heat up, there is an increased blood supply to the skin and this causes fluid to move out of our vessels and into the surrounding tissue."

Don’t fear, there are some things you can do to help. Emma said, "Make sure to drink plenty of fluids to stay hydrated. Help your body move the fluid on by doing simple ankle exercises such as rotating your foot as if you were drawing circles with your big toes. Do this 10 times a few times throughout the day to help your body pump the fluid away." When you get home, lie down and elevate your feet to help the fluid drain away. You need to try to elevate them above your heart.

Black Patients With Plaque Build-up More Likely to Suffer Co-Morbidities: Study
 
A new study from Keck Medicine of USC has uncovered significant racial disparities in the diagnosis, treatment, and outcomes of peripheral artery disease (PAD) among Black and White patients in the United States. Black patients with plaque build-up in arteries in the legs are more likely to have a stroke, heart attack, or amputation.
 
"We discovered that Black patients are nearly 50% less likely to receive vascular interventions to potentially restore the blood flow than White patients, and consequently are at a disproportionately higher risk of a stroke, heart attack or amputation," said David Armstrong, DPM, PhD, a podiatric surgeon specializing in limb preservation with Keck Medicine and an author of the study.

VA Podiatrist Discusses Ivory Tower vs. Reality Medicine
 
Ben Pearl, DPM discussed academic institution medicine versus  private practice on Dr. Tyson Franklin's podcast  Podiatry Legends. Dr. Pearl said that as a consultant at the National Institutes of Health (NIH), he collaborated with physiatrists, physical therapists, biomechanics experts, and orthopedic surgeons. The collaboration was natural because you were working in that environment as a team and there was no economic competition. In private practice, that is not the case and physicians have more of tendency to work in their own "silos".

Pearl added that interacting with physicians on his podcast has stimulated ideas for modifying his own protocols for better patient outcomes. Physicians that do not embrace change will find themselves in a slow downward spiral.

We regret to announce the passing of Dr. Richard Dean Erbes of Estherville, Iowa on June 9, 2023 at the age of 71. Rick passed away peacefully at home surrounded by family.

Query: Therapeutic Injections of Dextrose 
 
I have been reading numerous articles on therapeutic injections of dextrose therapy, peri-neural injection therapy, and hydrodissection. Most doctors are injecting dextrose anywhere from 12.5 to 25 percent.
1) Does anyone have a source where this dextrose solution can be purchased?
2) If you are doing such injections - would you be kind enough to share your feedback with any pearls or thoughts on the topic?

It has been a while since I performed at-risk foot care in New York. However, all the information you require about coding is the policy article attached to L33636 – Routine Foot Care and Debridement of Nails. The policy article and LCD are quite lengthy and often misinterpreted by podiatrists, and you are not alone in this. The CPT codes used for routine foot care are the most frequently used codes by podiatrists and the number one set of codes audited by Medicare for podiatrists. Thus it is important that you thoroughly understand the answer to your question and apply it to those clinical scenarios you may face.
 
As to specifically answering your question: The policy article lists a myriad of ICD-10, including I73.89 (other peripheral arterial disease) as a covered diagnosis, (I73.9 is not listed). For this ICD-10, your name and NPI may be listed as the referring doctor, but you must have a narrative description of the vascular findings (class findings), and an appropriate Q modifier (7,8, or 9) must be amended to the CPT 1105X code. The ICD-10 which provides for the local findings of callus (L85) is not listed as required. Even if it were the L85, by and of itself, it would not merit coverage and reimbursement for at-risk foot care. I urge you to go the NGS website and find the LCD and attached policy article for more information.
