PODIATRISTS IN THE NEWS
DC Podiatrist Discusses Plantar Fasciitis

If not treated, scar tissue can develop, inhibit healing, and lead to chronic plantar fasciitis. "It's not going to go away," says Dr. Howard Osterman, a podiatrist in the District of Columbia. "The problem is by ignoring it, you change the way that you walk and you create other issues. The earlier we treat heel pain, the better the chance we have of having it be an easy treatment."
Dr. Howard Osterman

The condition is not limited to competitive athletes. In fact, "it's probably just as common in non-runners as it is in runners," says Osterman, who treats it more than anything else in his clinic, Foot and Ankle Specialists of the Mid-Atlantic. The condition can also be caused by a lifestyle or profession requiring lots of standing, shoes with poor support, a quirky foot shape such as being flat-footed, or having high arches, or any combination of the above. People who are overweight or obese are also frequently diagnosed with plantar fasciitis since a foot strain can easily morph into the condition due to the added pressure on their feet, Osterman says.

INTERNATIONAL PODIATRISTS IN THE NEWS
Diabetics Need Regular Foot Exams: Aussie Podiatrist

Sydney podiatrist Dr. Abdul Habra warns diabetics to get regular foot examinations. Diabetics are often unaware of the risks they face due to neglect of their foot care. The significant rise in reported cases of diabetes is a disturbing trend, with Diabetes Australia reporting that 280 Australians develop diabetes every day. They also report that over 100,000 Australians have developed diabetes in the past year, with diabetes being the fastest growing chronic condition in Australia.

Dr. Abdul Habra

Habra said, “The need for regular foot examinations by diabetics cannot be underestimated. In many cases, diabetics have little feeling in their feet and, for their own safety, it is important for them to receive professional foot care by a podiatrist on a regular basis.”

PRACTICE MANAGEMENT TIP OF OF THE DAY
5 Ways Providers Can Improve Marketing in the Mobile Age- Part 2

2.  Provide essential content.

Ensure basic data such as your name, address, and hours of operation are visible on your local page. But go beyond the basics and share deep content that will help a patient make the right choice, such as your areas of specialty, your personal mission (for a doctor's page), insurance carriers you honor, and personalized physician videos. And, naturally, be sensitive to matters of tone in voice and content. Patients looking for service are not looking for cars. They seek wellness.

QUERIES (NON-CLINICAL)
Query: Does Yellow Pages Advertising Still Pay?

I advertise in both the major Indianapolis Yellow Pages and my more local West Suburban pages. I find most of my patients either have my magnet on their refrigerator, look me up on the Net, or are sent by referral. I am wondering how many out there find that the YPs are redundant and a waste of money? Any comments?

Do you have toe imaging for possibly underlying deformity? Consider addressing the distal 'bulb' of soft tissue rather than the nail plate. Also consider a second opinion with a plastic surgeon. This is an infant; you are responsible for whatever procedure you perform for many years to come.

First, lubricate the nail daily with A&D ointment. If a paronychia appears, apply hydrogen peroxide daily until cleared. Persistant paro will clear with Neosporin. Let this baby go and grow.

RESPONSES/COMMENTS (CLINICAL) - PART 1C
From: Dennis Shavelson, DPM

This is the youngest case of “disappearing toenail syndrome” (DNS) I have seen. We all see these nails in practice at different stages, but they are especially problematic in the latest stage when the nailbed unit covers with stratum corneum as in this 4 month old.

Disappearing Nail Syndrome

I have developed a formula for a multimodal plan for care. It involves shoe modification, an adhesive or RockTape strapping that I have developed, cosmetic reconstruction of the nail and a NuVail Rx that works about 60% of the time in well selected cases. When this doesn’t work, I suggest plastic surgery or lifetime cosmetic toenail reconstruction.

Disclaimer: I am a consultant to Innocutis, the marketers of Nuvail (poly-ureaurethane 16%)

RESPONSES/COMMENTS (NEWS STORIES)
From: Joel Lang, DPM

My practice experience has shown me that wearing any kind of backless shoe where the heel is not “contoured” will cause heel fissures in those patients who are prone to them. It is a mechanical problem where the round bottom of the calcaneus squeezes the plantar pad against a flat surface. The peripheral heel skin and fat pad are pushed outward. This causes stress on the skin circulatory system around the periphery of the heel and stretches the skin. Hyperkeratosis forms as a result. The thickened skin loses its elasticity and flexibility and cracking is inevitable.

The most efficient treatment is an aggressive (sometimes painful) debridement of the fissures down to live flexible tissue and the thorough removal of all hyperkeratotic tissue. Application of ointment (not crème) based lubricants are used – Vaseline is fine. Most important is the restoration of a compressive contoured environment for the heel. The best method I found was the use of an “M-F plastic heel cup”. I don’t know if they are even still made. The combination of restoring lubrication and removal of the causative stressors solves the problem. Occasional subsequent (less aggressive) debridement may be necessary.
