NBPME Selects Meazure Learning to Develop APMLE Exam
 
Meazure Learning, a leading provider of end-to-end test development and delivery solutions for professional testing and higher-education organizations, has announced it has been selected by the National Board of Podiatric Medical Examiners (NBPME) to develop and deliver the American Podiatric Medical Licensing Exam (APMLE) in a multi-year agreement. Prior Authorizations are an Unnecessary Healthcare Burden: MI Podiatrist
 
According to Conway McLean, DPM, "One of the most frequent tactics utilized by the insurance companies is the prior authorization. Once termed ‘pre-certification’, this concept was developed decades ago to prevent doctors from ordering expensive tests or procedures that are not 'appropriate,' at least according to the insurer. The aim initially was the delivery of cost-effective care but became a method of withholding care or delaying payments. When multiplied by the millions of insured individuals, the interest alone amounts to a significant source of revenue for these corporations."

"There is little accountability or oversight in this process. And the implication that patients need protection from their doctors is offensive to many practitioners. Providers must jump through hoops, often spending twenty or thirty minutes on the phone, usually after a staff member has spent twice that in the process of the typical prior authorization. Commonly, the insurance agent requires that the doctor gets on the phone, taking them away from patient care," says Dr. McLean.

VA Podiatrist is Keynote Speaker at AASCP Conference
 
David T. Weiss, DPM of Richmond, VA was the keynote speaker at the American Academy of Stem Cell Physicians (AASCP) conference from June 3rd-4th in Miami, FL. Dr. Weiss presented his lecture, “The Use of Regenerative Medicine Modalities in The Foot and Ankle: Why Operate When You Can Regenerate?” to a group of international leaders in the regenerative medicine field.

Additionally, Dr. Weiss led a workshop demonstrating the proper stem cell injection techniques for optimal outcomes. Weiss has been presenting his case studies and research to different medical groups, hoping to raise awareness about regenerative medicine as an alternative to surgery.

IN Podiatry Office Manager Charged with Medicaid Fraud
 
An Indianapolis woman is accused of defrauding Medicaid of $556,797 for her alleged role in submitting false claims for hundreds of medical devices for patients. Leslie Smith was arrested June 8 by federal marshals and was charged with healthcare fraud. According to a complaint filed by the Inspector General’s office of the U.S. Department of Health and Human Services, Smith submitted more than 200 false claims to Medicaid for portable medical devices known as oximeters, which measure a person’s blood oxygen saturation.

Query: Callus Challenges
 
I have a patient who comes in with a callus and has systemic conditions that qualifies her routine foot care as medically necessary. The location and full description are documented. The shaving of callus is documented. I plan to code the procedure as CPT 11055. Do I only need to place the systemic code (such as I73.9) as the primary, and do not put any secondary codes (such as L85)?

Response: It appears that the patient that is described in the above post qualifies for “At Risk,” routine foot care. As a general rule, in order to qualify for coverage, the patient in question must have a covered systemic disease and class findings. There must be physical, objective findings that need to be treated by a physician, such as mycotic toenails, dystrophic toenails, non-dystrophic toenails and hyperkeratotic lesions such as a corn or callus. The premise for coverage is that if the patient or a lay person attempted to treat the pathology that was present, the patient runs the risk of an infection or the loss of a limb.
 
So, the systemic disease that the patient has is coded as ICD-10-CM Code I73.9, which is defined as the following: Peripheral vascular disease, unspecified. The patient has a callus that needs to be pared or trimmed. The most appropriate ICD-10-CM code that defines the callus is ICD-10-CM code L84 which is defined as: corns and callosities. The CPT code that is billed for the “shaving” of the callus is CPT 11055 ,which is defined as the following: paring or cutting of benign hyperkeratotic lesion (eg. corn or callus); single lesion. In order to complete the coding, billing, and documentation process, the associated complication(s) resulting from the systemic disease (ie. I73.9) must be documented in the medical record. These are the class findings that lead you to select the appropriate Q modifier (i.e. Q7, Q8, Q9).
 
The appropriate way to code the above scenario is the following:

In my opinion, the biggest threat to podiatry is the fact that we are still not considered physicians in all senses of the word. All of the other issues with intramural bickering, insurance problems, etc. are not specific to podiatry. All medical disciplines face these issues, and they all fight each other and many "eat their young," not just podiatry. But until we can be recognized as physicians legally, we will never be able to fully participate in all aspects of the healthcare industry.  
 
I don't know how we achieve that, whether it be a degree change along with the appropriate changes in our education, or a change in the way our post-graduate education is currently structured. Although many, if not most of us, are considered true "doctors" by our patients and MD/DO colleagues in our communities individually, we must be able to be legally classified as equals as an entire profession before we can expect to be reimbursed equally or be in positions of leadership as other physicians are. I think that this is also the main reason we are not attracting more young people to our schools.
