CA Podiatrists Return from Vietnam Medical Mission
The Kaiser Permanente Global Health Project has just completed their annual Foot & Ankle Surgical Mission to the DaNang Orthopaedic & Rehabilitation Hospital. The goal of the project is to bring advanced surgical skills and technology to an underserved part of Vietnam. The project was started by Dr, Tom Kaschak in 1997. Since then, Dr. Kaschak has put together a yearly team of surgeons and other healthcare professionals including the Kaiser Permanente foot & ankle surgical residency programs to Danang for this volunteer surgical mission.

Dr. Glenn Weinraub pointed out that, "because of Tom's consistent commitment to the training of the orthopaedic surgeons in Danang, it's not really skills that they lack, but rather technology and advanced equipment." This year, over 40 reconstructions were performed for pathology, ranging from cerebral palsy to congenital deformities to traumatic deformities. The highlight of this year's mission was an official government ceremony whereby Dr. Kaschak was given a humanitarian award, acknowledging his devotion and care to the Vietnamese people.

Canadian Podiatrist Has Serious Concerns About Nail Salons
Toronto podiatrist Dr. Hartley Miltchin said he sees about six to eight people a week coming in with infections they suspect they've gotten from nail salons, and it has worsened in recent years as more low-price places pop up. He has serious concerns about the nail salon industry based on his experience treating patients out of his North York-based clinic...I'm going to make a major guess that 99 per cent of nail salons are non-sterile," he said.

"I know of cases where people have been hospitalized, they've lost the toe, they've lost the foot, and they've lost a leg from infections." Miltchin said podiatrists also sometimes cut nails and they are held to a very high standard of sterilization, using a process known as autoclaving, where instruments are treated with steam and pressure. "The same sterilization used to cut nails should be used in nail salons; it just makes sense," he said.

Flip-Flops Can Cause Heel Fissures: WA Podiatrist
Podiatrist Dr. Shawn Summers from Cascade Foot & Ankle in Yakima says he has treated countless cases linked to flip-flops. "As dry as the air is in Yakima, it can really dry the skin out, causing fissures in your heels and cracked skin, so you just want to be careful while you're wearing them," said Dr. Summers.

He says it all comes down to having the right support, something the flip-flop doesn't offer much of. But if you're just not ready to part ways with your flip-flops, Dr. Summers suggests looking for a type that isn't flimsy, which means you might have to spend a little more money. Look for a flip-flop that offers plenty of support for the arch of your foot.

VA Podiatrist Discusses Biggest Challenge in The Future
According to podiatrrist Dr. Mitchell Waskin, “The greatest challenge over the next five years is being able to remain an independent clinic. The increasing government regulations, decreasing reimbursements for our services (in the face of increasing expenses), and the tactics insurance companies use to avoid paying for needed medical services have prompted many doctors to work for hospitals or join large group practices. It has become almost impossible to have a small practice. Clinics like the Foot & Ankle Center need to be innovative in order to be successful and stay independent. We have achieved this by prioritizing high-quality care and offering a one-stop shop for medical and surgical foot care services.”

Patients Surprised By Out-Of-Network Bills Under ACA
The Wall Street Journal reports on unexpected medical bills under the Affordable Care Act resulting from patients going to out-of-network physicians or hospitals. Many policies on the health insurance exchanges have small networks in order to control costs, and while the law does set limits on out-of-pocket costs for enrollees, the cap doesn’t apply to out-of-network providers who charge patients for costs not covered by insurance. Dr. Jay Kaplan, president-elect of the American College of Emergency Physicians, defended physicians for billing beyond insurance, saying that insurance reimbursements are so low that providers have to bill the difference to stay in business.

CrossFit and Circuit Training Have Equivalent Benefits
The fitness benefits of CrossFit, a trademarked fitness program and competitive sport, match those of a generic circuit-training program in women, a new head-to-head comparison shows. The only significant difference between the two programs was a greater maximum bench press weight for the circuit-trained women.
But this finding, along with some trends toward differences in other parameters, suggest interesting contrasts, researcher Paige Wessel, from West Kentucky University in Bowling Green, told Medscape Medical News. With circuit training, strength improved more," said Wessell, who trains using a CrossFit program. But if you want endurance, "I'd say do CrossFit."

1. Claim your real estate. Make sure you have created local citations so that your business can be found on physician profile pages, directories, practices, and hospitals listings. Including Google My Business (creating a public identity on Google) is critical from a mobile aspect.

Query: Ingrowing Nail in 4 Month Old
This is a healthy 4 month old boy brought to the office by his mother. His hallux nail and distal hallux have created an ingrowing nail in the distal tip of the hallux. He did have a procedure to partially remove the offending distal border which apparently healed well but the problem remains. There is a very thin nail plate which has been debrided and reduced with a curette to prevent paronychias.

The distal portion of the hallux is overgrown and as the nail continues to grow, a more permanent resolution is sought by the family. At this point, the child is in no distress but there is a continued danger of infection. His other hallux is normal and there is no family history of similar problems. Any suggestions?
