OBITUARIES

Harold Sarles, DPM, 84

Late in life, podiatrist Harold Sarles assumed the persona that matched his personality. He became the clown "Dr. Foot." Dr. Sarles studied to become a clown with the Red Devil Clown Unit of Yaarab Shrine Temple after he retired in Buffalo, N.Y., and moved to Marietta in 1988.

Dr. Foot wore the classic red wig, bulbous nose, greasepaint and, despite his name, regular clown shoes. He entertained at Shrine functions, birthday parties and the neighborhood block party. At Halloween, he greeted trick-or-treaters as Dr. Foot.

Dr. Sarles participated in the Normandy Invasion, the Battle of the Bulge and the crossing of the Rhine, and was awarded the Purple Heart in the process . When he wasn't clowning around or playing golf, Dr. Sarles was gardening, a talent he had to relearn for Southern soil and plants. By 1992, he was a certified Master Gardener.

Groups Push Congress on Health IT

The AARP, Business Roundtable and Service Employees International Union have sent Congress a set of jointly endorsed principles for implementing the use of health information technology, which the organizations say would help reduce medical errors, and save time and money for patients and healthcare providers.

“The digital age has transformed virtually every other sector of the economy,” said Business Roundtable President John Castellani in a written statement released jointly by the three organizations. “It is time to bring the tremendous benefit of technology to our healthcare system by enacting health IT legislation.”

The principles, created in conjunction with the organizations’ healthcare-improvement campaign Divided We Fail, include recommendations that Congress pass legislation requiring payers and healthcare providers to use secure, uniform and interoperable technology; create grants, loans or tax credits that would assist providers in purchasing such technology; and set a timetable for a uniform healthcare IT system to be universally used. Representatives from the three advocacy groups said they plan to schedule meetings over the coming weeks with lawmakers in the Senate and House.

QUERIES

Query: Short 1st Metatarsal, Status Post Total Joint Implant

Unfortunately, following a total joint implant of the 1st MPJ the 1st metatarsal was inadvertently made too short. A result from the surgery is the patient has lesser metatarsalgia and sesamoiditis. I have made orthotics with a metatarsal pad and a 1st ray cutout to allow the 1st metatarsal to purchase more effectively, which has provided some degree of improvement. We are discussing surgical correction and I am considering a Lapidus with bone graft for lengthening or external fixator with distraction to gain length. Does anyone have any experience or thoughts on this matter?

RE: Removable Ankle Brace More Effective Than BK Cast For Low-Risk Ankle Fractures

This appeared in this week's issue of Medscape News. For patients aged 5 to 18 years with low-risk ankle fractures, a removable ankle brace is more effective in recovery of physical function than a below-knee walking cast, according to the results of a randomized single-blind trial published in the June issue of Pediatrics.

The study also concluded: The convenience and efficiency of the brace may reduce costs, avoid ED [emergency department] visits for patients who receive a diagnosis of these injuries in a primary care setting, reduce time spent in the ED, and obviate the need for follow-up visits in busy general or pediatric orthopedic clinics."

This would seem to support the increased use of these devices by both the podiatric and orthopedic community. The full study may be read at: Pediatrics. 2007;119:e1256-e1263
