ID Podiatrist Reminds Public of the Dangers of Flip-Flops
 
One podiatrist is reminding the public of the dangers that flip-flops can bring, as the weather starts to warm up. Randal Wraalstad, DPM at St. Luke’s says flip-flops are great for the correct circumstances, such as walking from the pool to the cabana, or walking on the beach, but they also present certain dangers. He says every summer he sees people get injured from wearing them at the wrong times.

Dr. Wraalstad says he sees people injured from mowing their lawn in flip-flops, and doing other home improvements projects as well. “People always have some accidents with lawn mowers, and if they have exposed toes and it slips underneath the lawn mower, they can lose toenails, they can lose their toes, they can eventually lose their foot.”

Critical Steps for Successful Digital Vendor Selection
 
Selecting a digital vendor is a complex task in any industry, but it’s especially challenging in healthcare. By following a series of comprehensive and repeatable steps, you can ensure your next digital solution selection process not only succeeds but delivers the results your practice needs.
 
6  Perform deep due diligence - One of the major challenges of digital solution selection is that public information on vendors and solutions tends to be either very limited or very biased, frequently originating from the vendors themselves. That’s why a superficial shortlist isn’t enough; you should also be requesting in-depth demos from vendors that cover the specific features and requirements that matter to you.
 
Beyond demos, you should also be performing your own independent inquiry through peer networks and reference checks. This is another opportunity where marketplaces can be a great help: the right marketplace can connect you with similar practices that can provide references, additional lessons learned, and more candid information on the actual outcomes and success of a given solution.

Barry Block, DPM’s editorial, “The American Board of Foot & Ankle Medicine and Surgery” advocating for a single unified certifying board for podiatry puts the issue into clear and sharp focus and demonstrates that the profession wants this. It is now up to the boards to work out a compromise.
 
I encourage all of us, both the rank and file and the leaders in our profession to call upon the boards to set a schedule of meetings, to come together and finally, get this done once and for all, for the good of the profession.

I was having the same issue with sign language interpreting until I found an online solution at Jeenie.com. I have no financial interest in Jeenie.com, but have used their services a few times and it worked out well. It is a HIPAA-compliant app that remotely connects you to an interpreter. Sometimes, it may take a few minutes to get an interpreter, but it hasn't been an issue the few times we used it. 
 
They charge by the minute (like $2/min), no monthly fees, no start-up costs, and no minimums and maximum. I used them twice in the past 3 months and was billed $46 for an initial office visit with the patient, and then $6 for a post-op nail check. I just had my assistant hold the iPad up behind me so the patient could see both me and the interpreter. It's very simple and affordable. It has been a perfect solution for us; otherwise, we were paying an interpreter $220 for the $80 office visit.

Other than the fact that podiatry has never been a cohesive unit, I believe the biggest existential threat is participating with health insurance. There will be no end to the erosion of income over time. Podiatrists work like dogs and are being exploited by insurance companies. So who is to blame? Clearly, insurance companies because they figuratively rape doctors. BUT, this is not a new story. It is the continued participation/cooperation/acceptance of this madness that is also to blame. And that is something every podiatrist can change. Otherwise, they can continue working as the underpaid employees of the insurance companies that they are.

I find it interesting that a person commenting that the profession itself is its own biggest threat, once testified against ankle privileges for podiatrists on behalf of orthopedic surgeons in his state. At that time, I was vice president of that state association and heard his testimony firsthand. Was not the question asking for forces outside of the profession?

Dr. Tomczak points to an age old question that faces not just podiatry, but all fields of medicine. There's too often a disconnect between the pride of being in medical practice, the economics that drive that practice, and the realization that all of us have a limitation of education. At the end of the day the primary focus must be whatever is best for the patient.
 
I just had a paper accepted by The Journal of Wound Care (due October 2023) that addresses this very issue. It describes a simple vascular procedure that went wrong, causing...

I hope that Dr. Gordon Yun mentioned in the original article that orthotics, NOT rest, was the real answer to these problems, at least on a conservative level. If Dr. Yun did not emphasize that podiatrists use orthotics and they are important to treat these problems, then he missed an opportunity to highlight the profession. This information is what makes podiatry unique and different and why the public needs to think podiatrist when they are in pain vs. just resting the part.
