PODIATRISTS IN THE NEWS
Keeping Feet Dry Helps Prevent Athlete's Foot: MD Podiatrist

Having damp feet can also create a breeding ground for athlete's foot, as can long nails, which can house (and spread) the fungi in question. This means that, even if you do wear flip-flops in the shower, you should dry your toes thoroughly any time your feet get wet, Priya Parthasarathy, DPM, spokesperson for the American Podiatric Medical Association and Chief of Podiatry at Medstar Montgomery Medical Center, tells SELF.

Dr. Priya Parthasarathy

And if you forgo a shower altogether at the gym, remember that your feet and socks might still be damp from your workout. Change into fresh socks, Dr. Parthasarathy says.

PODIATRISTS IN THE COMMUNITY
KY Podiatrist is HS Commencement Speaker

With a turn of a tassel, 137 seniors graduated from Middletown Area High School. The commencement speaker was Dr. Carl Kihm, a 2001 MAHS graduate, who is a podiatrist, or a foot and ankle doctor. He trains surgical residents at the Norton Audubon Hospital’s Kentucky Podiatric Medicine Residency Program in Louisville. “Maybe you don’t know exactly what you’re going to do when you graduate, and where life will take you from here, but that’s okay,” Kihm said. “Sometimes life has to shake you a little bit to guide you where you’re supposed to be.”

When Kihm was a senior, he played on the soccer team. The team was advancing through the playoffs when he broke his ankle. His dream of being a state champion was shattered. Although the team lost, it planted the seeds of his new dream to become a podiatrist.

PODIATRISTS AND THE LAW
GA Podiatrist Sentenced To 36 Months Role In Kickback Scheme

A Georgia podiatrist was sentenced for his participation in a kickback scheme where he prescribed medication that was ultimately paid for by a federal program that provides low cost medication to members of the U.S. Armed Forces. Dr. Alap Shah, 44, of Columbus, Georgia, was sentenced to 36 months in prison followed by three years of supervised release. Shah was also ordered to pay a $300 special assessment and restitution in the amount of $55,300. Shah was convicted of one count of conspiracy to defraud the United States and receive healthcare kickbacks and two counts of receiving kickbacks.

PRACTICE MANAGEMENT TIP OF THE DAY
10 Ways to Strengthen Your Strategic Plan - Part 7

A strategic plan is not just a document written by physicians and managers and then filed away for safekeeping. It is a vision for the practice, owned by the practice. For a medical practice to succeed, everyone must engage with and live and breathe the plan. Here are 10 questions to assess your strategic plan.

7. Does your strategy require commitment and trade-offs? Hedging your bets is not a strategy. A full commitment to a defined course of action is the only path to a sustainable competitive advantage. This requires trade-offs, as you can’t be all things to all people. Remember to consider what you are NOT going to do and accept those self-imposed limitations to stay focused on long-term success.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Joseph L. Starr, DPM

Joseph L. Starr, 100, of West Hartford, formerly of Bridgeport, Connecticut, and Del Ray Beach, Florida, died on June 10, 2019, just a few days shy of his 101th birthday. Joe graduated from the New York College of Podiatric Medicine. He served in the Army Air Corps in World War II, and then was a podiatrist in Bridgeport, where he practiced for almost 40 years. He was a past president of the Connecticut Podiatric Medical Association.

QUERIES (CLINICAL)
Query: Lateral Column Fusions vs. Arthroplasties of the Metatarsal Cuboid Joints

I have an active slightly overweight active middle aged female patient with a non-contributory medical history who has developed painful osteoarthritic changes across the Lisfrancs joint involving 2nd met/cuni, 3ed met/cuni, 4th met/cub and 5th met/cub. Conservative treatment has failed to give her significant relief of pain. She gives no history of acute trauma recently or in the past. On clinical evaluation, the majority of pain is produced with range of motion/stress of the 2nd and 5th metatarsals at the base. Her forefoot to rearfoot is slightly varus. She has a cavus foot type.

AP and oblique views

These are well aligned joints, just arthritic and painful. I am concerned about fusion of the 4th and 5th metbase/cuboid in this foot because of the independent motion required by the lateral column. I was thinking about cleaning up the 4th and 5th met/cuboid joint and placing a joint spacer there to maintain some motion. I would appreciate comments on lateral column fusions vs. arthroplasties of the metatarsal cuboid joints.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Cedric Cooper, DPM, Kathleen Neuhoff, DPM

Give it a shot. You should keep it conservative non-surgical, in my opinion. Keep your overhead down with maybe two medical office assistants... one for receptionist duties and the other to assist. Do not use an EHR. It’s too expensive. Currently, it’s still legal to operate a podiatry or medical practice using paper only. Depending upon your state, offer services that may be considered aesthetic in nature. Try to find some retail sales that you may be able to offer to supplement income. Get one phone line only or it will get too expensive. I would like to hear how it’s going.

Cedric Cooper, DPM. Jackson, TN

I am not an attorney, but I would be sure your business model allows easy exit for each participant so if one person becomes unable to practice, the whole structure does not need to be re-done. Personally, I think you will be very successful. The cash only, non-covered portion of my practice has been growing steadily for 10 years. In addition, this year, I stopped taking Medicare assignment. This allowed me to raise my fees. After 5 full months, we are down $630, and lost less than 2% of our Medicare patients. However, my net income has increased significantly and would be up even more except that I am committed to retaining my 7 employees as long as they want to stay. We have several successful concierge practices in our area and I think many people are willing to pay to be treated as valuable patients. Just give them plenty of TLC!

RESPONSES/COMMENTS (NEWS STORIES)
From: David Zuckerman, DPM

Dr. Jacobs makes a good point, but I believe the point made by Dr. Horowitz makes a more important point. Podiatrists should play a major role in the assessment of risk falls which includes gait and biomechanics. Saying that every fall is probably due to an inner balance middle ear problem is robbing our patients of comprehensive fall assessment

Podiatric medicine is needed and should be part of the initial fall assessment  Sending patients just for etiology assessment that doesn’t include podiatry is robbing our seniors of complete comprehensive care
