Stilettos Can Lead to Foot Surgery: NY Podiatrist

How many women put on high stiletto heels even though they know they’re going to hurt? Turns out, a lot. But stuffing our feet into a pair of stilettos can do permanent damage. One podiatrist says 40 percent of her patients have pain caused by high heels.

“It can lead to surgery, severe pain, and you might not even be able to go and run,” Manhattan podiatrist Dr. Hillary Brenner said. In an online survey, nearly half of women admitted to repeatedly wearing uncomfortable shoes even when their feet hurt, and nearly 40 percent admitted to walking home barefoot after a night out because they just couldn’t take the pain anymore.

Scholl Professor Sues Associate Dean for Defamation

Severko Hrywnak, DPM, an assistant professor and Chair of the Academic Assembly at the Dr. William M. Scholl College of Podiatric Medicine has filed suit against Daniel Bareither, PhD, alleging defamation. Dr. Bareither is a professor of Basic Medical Sciences and Senior Associate Dean of Educational Affairs. A copy of the complaint appears here

Reached for comment, Dr. K. Michael Welch, president and CEO of Rosalind Franklin University of Medicine and Science said, "We just received this. While the university is not named as a defendant, our attorneys are investigating the allegations and, based on our current knowledge, we do not believe that these claims have any merit whatsoever."

Physicians' Medicare Claims Data Could be Made Public After Court Ruling

Doctors who fear big government intrusion into the practice of medicine may have just gotten something else to worry about. A federal judge made a decision last week that could result in individual doctors' Medicare claims data being made public.

U.S. District Judge Marcia Morales Howard on Friday lifted a 1979 injunction that had barred public access to a confidential database of Medicare claims data. The American Medical Association (AMA), which opposes lifting the ban on the grounds of physician privacy, may appeal the ruling.

Query: Fissuring Under 4th and 5th Digits

This 19-year-old female does not want surgery to desyndactylize this congenital anomaly. Her main complaint is two or three times a year (especially after swimming) the skin will crack underneath the fourth and fifth digit. I recommended various moisturizing creams for her.

RE: Recurrent Ganglion Cysts

I have a technique I've used maybe a half dozen times for larger ganglion cysts over a midfoot or hindfoot joint, or recurrent ganglions. Once I find the hole where the joint fluid is leaking from, I use a small Mersilene patch which I cut to fit the area, and sew over the fascia or joint capsule using non-absorbable suture. Mersilene is commonly used as a hernia repair patch. It's thin, cuts easily, and is perforated so it's easy to stitch down (It is also inexpensive.) Mersilene is popular in hernia repair because you get a thick, dense scar tissue - sounds perfect for sealing up a recurring ganglion, right?

RE: Recurrent Ganglion Cysts

Typically, I inject Depomedrol just proximal to the post-surgical wound. According to O.A. Mercado's videos, and other various reference textbooks, a "few drops of Decadron" intra-operatively is another technique. The post related to using "non-absorbable suture and to heavily cauterize it." I suspect that this might be causing further perpetuating aggravating factors such as the irritation of the surrounding structures, and causing the cycle to repeat from repetitive irritation of remnant sutures, perhaps overzealous necrotic cauterized tissue.

I would have selected an absorbable type suture to help nature maintain a smooth natural gliding action. If it's emerging from a (synovial) joint, then there may be iatragenic causes that are inducing tendon sheath ganglionic cyst(s). Finally, a firm compression post-op dressing helps with a mixture of long and short-acting corticosteroids.

Back then, DPM graduates found themselves fighting over a limited number of surgical slots, with the rest (majority) of us settling for...

RE: Educational Commission of Foreign Medical Graduates (ECFMG)

I recently read in the ECFMG Reporter about the development of GEMx (global education in medicine exchange). ECFMG is the authority for recognizing medical training as equivalent enough to take medical licensing exams. Successfully meeting the requirements allows you to qualify for residencies and take the exams to be licensed to practice medicine in the U.S.
 
I suggest that our APMA and/or CPME liaison with the ECFMG to see if a "third pathway" can be developed for U.S. DPMs to be considered at least as equivalent as foreign medical students.  Podiatry needs to think about merging with medicine instead of running a parallel, but substandard course. It's time to drop the blind loyalty to podiatry as an independent field. As chiropodists, we were unique and could be independent, but to evolve, the lines of the two professions must intersect. We should try every path to make this happen. The APMA needs to change its focus and get serious.

RE: Pronation from Flip-Flops Can Damage Feet: MI Podiatrist

It’s time to once and for all “bust the myth” that all pedal and postural mechanical problems result from primary pronation on the frontal plane of the rearfoot. In fact, although there are those who suffer from this malady, far more feet suffer from primary forefoot supination. Forefoot pronation is a viable treatment for most feet biomechanically. Dr. Bremer, stating that “Pronation is a condition that can lead to foot arch collapse, premature arthritis, bunions, and heel spurs" says so from a position that Dr. Dananberg, myself, and others have dispelled for decades, and is more and more accepted by the international biomechanics community.

I am tired of hearing from DPMs that biomechanics is too hard to understand, that it is pseudoscience, and that when a weight-bearing foot is “flat”, without examination or measurement, the culprit is pronation. Spreading this kind of false information on websites, in the media, and on the Internet without modern biomechanical knowledge and skill stands in the way of those of us who wish to practice podiatric biomechanics backed with science, skill, and the existing EBM.

We should be selling 1st ray cutouts, not RF varus posts, and we should be blowing away the wanna-be “pronator” biomechanists from our podiums, schools, residencies, and the social media. Every tri-plane joint pronates to good and bad ends. To hear my profession continue to try to convince the foot and postural suffering public that “pronation is bad” is a myth that needs busting.

In fact, Dr. Miltchin does teach his techniques along with other podiatrists at LSU Medical School. The AAFAS will be sponsoring its annual cadaver course in conjunction with this fine institution, next week. We cordially invite Dr. Sullivan to participate. It is important that, as a profession, we share our knowledge and experience with each other. I commend both Drs. Miltchin and Dr. Sullivan for their willingness to share over the years. They are both a credit to our profession.

Like Dr. Miltchin, I've had very few complications utilizing minimally invasive surgery for bunions. I have not had a non-union or hallux varus from a bunion deformity that I corrected utilizing minimally invasive surgery. I have been using MIS techniques for bunions for 7 years. It is theoretically possible that he has never had a complication from bunion repair utilizing minimally invasive surgery.

I personally believe that complications do not discriminate on incision size. I have had my share of complications with minimally invasive surgery, including non-union. MIS is just a technique. It's not the only way, and it is not free from complications.
