PA Podiatrist Corrects Flatfoot With Titanium Implant

When she was a little girl, Vanessa Wanshop dreamed of being a ballerina. Then one day her dance instructor noticed that when she took a stance that requires her toes to be pointed out to either side at 90 degrees, her knees would still be pointing forward. She had, doctors would later tell her, a classic case of flat feet. In addition to cosmetic concerns, Wanshop's flat feet caused her chronic foot, hip, and back pain for most of her life.

Dr. Jess Drezner

Then one day her husband, Eric, met Dr. Jess Drezner, a Spring Township podiatrist, at a social event, and the two got to discussing Vanessa's flat feet. Drezner said that for the past five years he had been performing a simple outpatient surgery that can correct most flat feet with a 20-minute procedure and a single titanium implant called HyProCure. That simple but important correction not only corrects flat feet, it helps bring the entire body back in line, Drezner explained.

Trial Date Set for VA Podiatrist Accused of Trading Rxs for Marijuana

A trial date has been set for a podiatrist accused of writing prescriptions in exchange for marijuana. 61-year-old Dr. Richard Caudell was arrested in April after an eight-month investigation. Police said he wrote prescriptions for oxycodone in exchange for marijuana. He faces three felony charges of distribution of oxycodone. Caudell's trial is set to start on August 10.

CMS to Accept Advanced-Payment ACO Applications

The CMS announced plans to accept applications for 2013 advanced-payment model accountable care organizations. As of August 1, the agency will accept applications for the program, which offers small hospitals and medical groups access to upfront payments under the Medicare Shared Savings program, the CMS said.

The Shared Savings program was created under the Patient Protection and Affordable Care Act, which is facing a legal challenge before the U.S. Supreme Court that could invalidate the entire law. A decision is expected this month. The CMS named the first five advanced-payment ACOs in April. The CMS is expected to name another round of participants who will enter into shared savings contracts as of July 1.

QUERIES (CLINICAL)

Query: Bilateral Pain Sub 2nd Mets

I have a 50 year old construction worker who presented with pain and discomfort to each foot. Though the feet are painful sub met 2nd B/L, he also suffers from extreme sciatica. He has been referred to a back specialist. In the interim, the feet are painful on palpation and manipulation. The slightest amount of pressure is excruciating.

Painful Lesion Sub 2nd Met

There is no evidence of direct trauma or incident involving the 2nd MTPJs. The MRI report indicates either local callus or a possible fibromatosis. Any suggestions on how to treat this would be greatly appreciated.

RE: Belated In Memoriam – Isadore "Buzz" Forman, DPM
From: Arden Smith, DPM

I was speaking with Warren Joseph, a fellow alumni of the St. Joseph’s Hospital surgical residency program in Philadelphia, and he informed me that he had only recently found out about the death of our director and chief, Dr. Isadore P. [Buzz] Forman, who had died about two or three years ago.

We were both saddened that there was not a word in any podiatric periodical about this man who was one of the true pioneers in podiatry and one of the “fathers” of podiatric surgery. Podiatry was Dr. Forman’s life, and he was probably responsible for teaching more podiatrists surgery than anyone else in Philadelphia. Dr. Forman, who died after a long battle with Alzheimer’s, had a small family and outlived most of his peers. He practiced with and was followed in practice by his son, Dr. William Forman.

After graduating from the Temple Chiropody School, Buzz fought in WW II and became an...

From: Connie Lee Bills, DPM

Not only are all healthcare expenses out of line, so is the cost of education. At $150K, it would be hard to pay the ticket with the $15 office visit.

RE: Unified Post-Graduate Training (Charles Lombardi, DPM)
From: Bryan C. Markinson, DPM

Dr. Lombardi is very passionate about the "history" he states I distort. It is as though he "hears" differently from what I write. I will let our respective comments speak for themselves as follows:

I stated that the "American Board of Podiatric Surgery and too many of its members were simultaneously telling insurance companies and hospital administrators and whomever else that the 'credential you can trust' is the consummate barometer of the 'qualified' podiatrist, no matter what he or she is treating." This means that they implied that a hospital or insurance company gets the best bang for the buck with DPMs with their credential. I said nothing about the ABPS stating they were the only board.

Dr. Lombardi responded "The ABPS never went...

RE: Vision 2015, Vision 20/20 (Ed Davis, DPM)
From: Samuel S. Mendicino, DPM

I read all of the posts regarding post-graduate training and it makes me chuckle. I just attended a conference, and at a roundtable discussion I heard many senior DPMs complain that “today’s graduates can’t even trim a toenail, diagnose a wart, or even strap a foot.”

Then I move to the next table and I hear how our students are all mainstreaming with allopathic/osteopathic students, and they should be taking the USMLE exams. Move one table over, and a group of residency directors who have high volume surgical programs are complaining that there isn’t enough time to teach all facets of surgery in the combined models; and what do you do with the resident who has poor surgical skills? At the next table, I see directors who teach at excellent medical facilities with phenomenal wound care clinics, and biomechanics experts complain about having to find all of these rearfoot and ankle reconstructions to meet MAVs.

The problem is what it has been for decades: we can not define who we are and...

RE: Barefoot Running More Efficient (Dennis Shavelson, DPM)
From: Robert Scott Steinberg, DPM

Here is what I have observed in my athletic patients over the last 37 years. When they present with foot, ankle, shin, or knee pain, they are almost always in soft and flexible (powder-puff) unstructured athletic shoes. If my treatment of them is with a prescription functional foot orthotic, they are going to leave my office with a list of shoes that I recommend for their foot type and condition. These will be either stability or motion control shoes. I dispense the PFFOs into the shoes, and off my patient goes. Months pass, but what brings them back in 6-8 months appears to be that they switch to inappropriate shoes, i.e., cushioned shoes. Today, I can add the other mistake they make is switching to a minimalist shoe. I prove my "theory" that unstructured shoes are the root cause of their re-injury by putting them back in the appropriate shoe, and watching their injuries disappear. That's scientific enough for me.
